5 III. HOTEL ACCOMMODATIONS Springhill Suites New Orleans Downtown/Covention Center Your hotel accommodations are as follows: 301 St. Joseph Street New Orleans, LA Tel: (540) Check In: 4:00 P.M. Thursday, July 5, 2018 Check Out: 12:00 P.M. Monday, July 9, 2018 (Early check-in has been requested for your group however it is not guaranteed) Guidelines: Only paid occupants are allowed to stay in the hotel room and to be listed on the hotel phone roster. Room keys will be pre-carded and will be distributed by Ms. Kentaya Beeler upon arrival. Your accommodation provides you with 2 Queen Beds Suite with a layout sofa bed in the living room accommodating up to 4 individuals in a room. All suite accommodations include a bedroom and living room area with a kitchenette with small refrigerator and microwave. Note: Special Accommodations on the occupant spreadsheet, it is purely a REQUEST and you will be contacted if your REQUEST can be fulfilled. (ex. Handicap) Daily Breakfast is provided. Guidelines: Only paid occupants are allowed to stay in the hotel room and to be listed on the hotel phone roster. If you would like optional service (i.e. Phone calls, Room Service, etc.) one occupant in the room will be required to present a credit card upon check-in. However, any administrative complaints or requests should be addressed to Ms. Kentaya Beeler (914) cellular.

6 IV. ENTERTAINMENT Prior to arrival into New Orleans, s will be distributed as an update of the entertainment available during the festival. V. Name changes are not allowed Southwest Airlines allows online check-in for groups. Each passenger is required to present a source of Photo Identification to check-in. *** Tranquilpathways Travel is only responsible for the flight you are booked on. If you miss either fight segment it is not our responsibility to arrange additional accommodations and all group flights are non-refundable/non-transferable. AIRPORT SHUTTLE Airport shuttle is provided from New Orleans Airport to the Springhill Suites. As luggage is collected please look for your group leader so that we can all board the airport shuttle together as a group. Please minimize luggage so that several vans will not have to be used to courier your group causing separation.

7 DISCLOSURE DISCLOSURE to be completed and returned (888) (Fax) I, acknowledge the payment schedule and agree to full payment of the entire cost of the trip. I have been advised that all deposits and payments are non-refundable unless you opted to pay for travel insurance. I have read and agree to comply with all trip guidelines. In the event that I do not fulfill my payment requirements I agree to the garnishment of all deposits and will remit funds dependable by my roommate(s) to prevent forced legal actions.
