9 RESPONSIBILITY FOR THE PRIVILEGE Page: of 2 PARAGRAPH PAGE 1. Responsibility Responsibility A. Travel programs are extended to SkyWest by our codeshare partners. Travel is a privilege and not a right of employment, and program fee payment does not guarantee ongoing travel privileges. Participation or opting in to partner privileges is optional, but once they have been elected, fees paid or committed to be paid are non-refundable. The terms of the programs and policies are subject to modification or termination, in whole or in part, by our partners or by the Company at any time and for any reason. B. Employees of SkyWest Airlines have access to worldwide travel through the leisure travel programs. Leisure travel privileges also extend to employees eligible pass riders so employees can share their travel experiences with those closest to them. Leisure travel is a privilege and with it comes responsibility. C. It is the responsibility of the employee to review the Employee Travel Manual and learn how the leisure travel programs work. In the manual, employees will find information about SkyWest and partner airlines' pass travel policies and procedures, including information to help employees prepare for their trips, procedures to keep in mind as they travel, and other travel tips and resources. This important information should be shared with their eligible pass riders. The employee is responsible to ensure their pass riders including anyone using Buddy Passes use leisure travel privileges appropriately, exercise good judgment, and follow SkyWest and partner airlines policies and procedures. Employees are held accountable for the conduct of anyone using their travel privileges. D. SkyWest reserves the right to deny travel privileges or apply corrective action, up to and including termination, to anyone misusing travel privileges. Misuse is a direct violation of Company policy and includes, but is not limited to, misrepresentation of time off to the Company while engaging in leisure travel and/or profiting from the sale, barter, or trading of travel privileges, including Buddy Passes, whether directly or through a broker. The travel privileges provided to employees and their eligible pass riders are a privilege not a right of employment. Corrective action for abuse or misuse of travel privileges may include suspension of travel privileges, charges assessed for full revenue fare, and/or termination of employment. NOTE: When your travel privileges are suspended due to misuse or a violation, you may not travel on other SkyWest employee s flight privileges. E. SkyWest s priorities are to deliver revenue customers to their destination safely and to operate flights on time. Customers expect flights to operate on time, and on time performance is part of the Company s agreements with its partner airlines. The Company depends on employees to achieve this goal and to ensure that non-revenue travelers do not distract other employees from that mission. As employees, we should remain focused on our customers at all times and assist our frontline employees in any way possible to maintain the high standard of performance and service SkyWest is proud to provide to its customers. 11 RULES OF THE ROAD Page: of 4 PARAGRAPH PAGE 1. Non-Revenue and Reduced-Fare Travel Rules Non-Revenue and Reduced-Fare Travel Rules A. Employees and their eligible pass riders must adhere to all non-revenue policies and procedures. Violation of these rules of non-revenue travel may result in suspension of travel privileges, charges assessed for full revenue fare, and/or termination of employment. The following rules apply to all non-revenue and reduced-fare travel on SkyWest Airlines and all other airlines. 1) Restrictions a) Non-revenue and reduced-fare travel provided to SkyWest employees and their eligible pass riders is for leisure and authorized bereavement/emergency travel only. It must not be used in connection with any business activity in which employees or their eligible pass riders may be engaged. b) Employees and eligible pass riders who have purchased a revenue ticket for a flight are not allowed to also list as a non-revenue passenger on that flight or any other flight with the same routing within a 24 hour period from the departure time of the flight that a revenue ticket has been purchased. Employees are not allowed to use their employment status to request to have fees waived or policies altered. c) All employee passes and reduced-fare tickets are rendered void and invalid for travel if the employee leaves the Company or if their travel privileges are suspended. Refunds for tickets can be obtained by following established refund procedures. Refer to Ticket/Pass Refunds for additional information. NOTE: Annual travel fees to enroll or opt in to partner travel programs are not refundable, not able to be canceled, and will not be prorated in the event of employment separation or when travel privileges are deactivated for a specific purpose. d) Travel privileges and all associated travel for the employee and all eligible travelers is only valid through the last day of active employment with SkyWest Airlines. 2) Conduct a) Employees and eligible pass riders must: (1) List themselves as early as possible and remember fellow non-revenue travelers when listing for flights. When listing, remember: (a) (b) (c) (d) (e) Do not create multiple listings on the same flight. Do not list on multiple flights on the same day of travel; please pick one flight. Do not list on consecutive days to the same destination or to destinations within the same vicinity. Cancel your listing as soon as possible if your plans change and you no longer plan to travel on a flight. When you are traveling with checked bags and need to change your listing, use the same PNR to change your listing do not create a new PNR. 01 Leisure Travel Programs

12 RULES OF THE ROAD Page: of 4 (2) Dress appropriately. Refer to Travel Dress Code and Guidelines for additional information. (3) Check in on a timely basis prior to flight departure. Check-in requirements vary by airline, airport, and country and should be verified prior to travel. Non-revenue passengers who check in for a flight less than 30 minutes prior to departure time may be placed at the bottom of the standby priority list. (4) Present government-approved identification upon check in for all flights (travelers over the age of 18). (5) Pay applicable baggage charges. Employees and eligible pass riders are charged for excess or overweight bags, and some airlines may assess fees for checked bags, pets in cabin, unaccompanied minors, etc. (6) After going through security, place your company badge out of sight for the remainder of your leisure travel. (7) Respect colleagues who are on duty by cooperating and complying with all rules and regulations. After checking in for a flight, walk away from the counter. Do not badger or disturb the Customer Service agent. When all revenue passengers have been processed, agents make an announcement for standby passengers and distribute boarding passes based on the order of the standby priority number listing. (8) Board as directed by the Customer Service agent and quickly occupy their assigned seat. (9) Be respectful of revenue passengers. Employees should not identify themselves as a non-revenue traveler to any customer. (10) Give their full attention to ground and flight crews during all announcements. Listen and do not talk. Employees may feel as though they have memorized briefings, but there may be passengers seated around them that have not. Announcements are a matter of safety and a Federal Aviation Administration (FAA) requirement. (11) Keep noise and activity to a minimum when traveling with children to ensure they do not interfere with the comfort of other passengers or the service offered by the flight attendant(s). (12) Be courteous when choosing snacks and beverages. (a) Flight attendants must do everything possible to accommodate revenue passenger's first-choice snack, meal, and beverage selections. Be very discreet and understanding when your choices cannot be accommodated due to availability. (13) Assist flight crews in their cabin clean-up at the conclusion of a flight. Employees should use discretion and not impede the deplaning of revenue passengers. (a) Employees may assist flight attendants by: (i) (ii) Making certain there are undamaged copies of the passenger safety card, SkyMall magazine, and a sick sack in each seatback pocket. Clearing crumbs and debris and discarding any trash. (iii) Handing any pillows or blankets back to the flight attendant. 01 Leisure Travel Programs

13 AE AS/UAX DC RULES OF THE ROAD Page: of 4 (iv) Placing seat belts neatly across seats. Each airline has a desired way of crossing seat belts prior to passengers boarding the aircraft. Seat Belt Placement for AE, AS, DC, and UAX Seat belts should be crossed at angle toward the front of the seat, placing the large end of the buckle on top. Seat belts should be folded horizontally against the seat back. Seat belts should be uncrossed and pulled forward on the seat cushion. (14) The nature of standby travel requires that employees exercise responsibility to ensure they have adequately planned enough travel time to return home prior to the start of their shift. Missing a shift due to employee travel places a burden on fellow employees and, depending on circumstances, may be considered abuse of non-revenue travel benefits. 01 Leisure Travel Programs

Employee participation in SkyWest's major partner travel programs is based on individual carrier eligibility requirements, including applicable enrollment fees. B. Travel is available to employees and their eligible pass riders once the employee has successfully completed a clearance process. 2. Travel Authorization (TA) A. SkyWest s internal travel database is called the Travel Authorization (TA). B. An employee s TA is a complete list of those approved by the Travel Center to participate in the employee s travel privileges. C. Employees can view their TA at SkyWest Online > Travel > Travel Profile. D. Employees have the flexibility to define separately which members of their TA are eligible for travel on each partner airline, according to the proper process and timeline for each airline. E. New hire employees have 90 days following their hire date to customize their TA for each partner airline. 1) For United Airlines, changes may be made within 90 days, but once the opt in process has taken place, the profile is set for the remainder of the travel year. 2) For Delta Air Lines, changes may be made within 90 days, but once the PPR fee is paid, the profile is set for the remainder of the travel year. 3) For American Airlines, changes may be made within 90 days, but once the enrollment process has taken place, the profile is set for the remainder of the travel year. 4) For Alaska Airlines, changes may be made within 90 days following their hire date. 01 Leisure Travel Programs

16 3. Adding Authorized Travelers TRAVEL ELIGIBILITY AND AUTHORIZATION A. To add authorized travelers to the TA: 1) Go to SkyWest Online > Travel > Travel Profile. 2) Select the Add Traveler button. Page: of 8 3) Enter the information for the person to add to the list of authorized travelers. 4) For those specific relationships that require documentation to be submitted, the employee will be prompted to upload that documentation during this process. Documents must be uploaded in pdf or jpeg formats only. B. The Add Traveler online authorization must be submitted for each traveler and unique relationship. C. An employee's travelers are authorized for travel only after proper documentation (when applicable) is received through the TA Add Traveler upload process and verified by the Travel Center. NOTE: Employees should allow 14 business days for the approval and processing of documents. Approved TA changes take 24 to 72 hours to download into mainline partners' systems, with the exception of American Airlines, which, at this time, can take up to three weeks to download. 1) Eligible pass riders are denied travel privileges when proper documentation is not provided or received within a two-week period after making the request to add the traveler. D. When the Travel Center has approved the changes and verified the documents, the status of the traveler changes from pending to active on the employee's TA. 4. TA Modification Requirements A. All employees must ensure their TA is correct and up-to-date at all times. B. Removing pass riders 1) Removing pass riders may be requested at anytime. 2) Restrictions may apply, and supporting documents may be required. 3) Employees are not reimbursed fees for any pass riders removed during the travel year. C. Adding or reinstating eligible pass riders 1) Refer to the individual partner airline sections of this manual for specific enrollment instructions and enrollment/opt-in time periods. 01 Leisure Travel Programs

17 D. Life Changes TRAVEL ELIGIBILITY AND AUTHORIZATION Page: of 8 1) Notice of any life-changing event that impacts the eligibility of a listed pass rider must be made within 30 days of the change. Changes include, but are not limited to: Divorce (divorce decree required) Marriage of a child Death of a spouse/domestic partner, parent, dependent, or travel companion Change in student status Change in military status for dependents 2) The Travel Center must be notified to nullify travel privileges for a pass rider who no longer meets eligibility requirements. Failure to remove a pass rider who no longer meets eligibility requirements is considered fraudulent use of travel privileges, and the employee will be subject to corrective action. E. All required documentation must accompany the Add Traveler, Edit Existing Traveler(s), Remove/Suspend Traveler(s), or Update Student/Military Status request submitted to the Travel Center. 1) Required documentation will be prompted for upload while completing the request process. 2) Eligible pass riders are not eligible for travel privileges when proper documentation is not provided or received within a two-week period after making the request to change or alter the traveler. F. SkyWest's partnerships with other airlines make it imperative that eligibility is verifiable and accurate at all times. G. Tickets issued to a previously eligible family member must not be used if that family member is not currently on the employee s TA. 5. Eligible Family Members A. Qualifying Spouse 1) A qualifying spouse is defined as a person who is currently and legally married to a SkyWest employee. A copy of a state-issued marriage certificate is required. 2) Honeymoon travel with a new spouse a) Employees who plan on traveling with their new spouse immediately following their marriage can arrange temporary honeymoon travel authorization for their spouse by submitting an online request and a copy of their marriage license. The documentation must be received by the Travel Center approximately two weeks prior to the marriage date so travel authorization can be assigned. b) On the day of travel (after the marriage), the new spouse may travel with their driver s license and a copy of the marriage license. Travel privileges are temporary and expire two weeks after the marriage unless a certified copy of the state-issued marriage certificate is received by the Travel Center prior to the expiration date. c) Permanent non-revenue travel privileges for a new spouse take effect after the online request and a certified copy of the state-issued marriage certificate are received by the Travel Center for verification and approval. 01 Leisure Travel Programs

18 B. Domestic Partner TRAVEL ELIGIBILITY AND AUTHORIZATION Page: of 8 1) An employee who is living in a committed, spouse-like relationship with a domestic partner must provide legally acceptable evidence of their relationship in order to qualify the domestic partner for travel privileges. 2) Proof of domestic partnership may be established by providing: a) A copy of a state-issued Certificate of Domestic Partnership (city- or county-issued certificates are not accepted) or b) A notarized Affidavit of Domestic Partnership and proof of financially interdependent domestic partnership as specified in the Affidavit. (1) Refer to the Domestic Partner Checklist to assist in identifying the required documentation. 3) The policy regarding documentation for domestic partners is governed by SkyWest s plans and policies. Any false or misleading statements made in order to receive travel privileges for which the employee does not qualify may subject the employee to corrective action up to and including termination of employment. 4) Children of the employee s domestic partner who satisfy dependent children eligibility requirements are eligible for travel privileges. 5) Travel privileges for domestic partners and their dependents are limited to those airlines whose interline contracts allow travel for domestic partners. 6) Termination of Domestic Partnership a) The employee must submit a Notice of Termination of Domestic Partnership to the Travel Center within 30 days of termination of the partnership. C. Dependent Children A copy of the form must be provided to the former partner. NOTE: An employee cannot designate a new domestic partner for a period of six months following the filing of the Notice of Termination of Domestic Partnership with the Travel Center. The six-month restriction is waived if the partnership is terminated by death. 1) For purposes of travel eligibility, dependent children are defined as unmarried, minor dependent children for whom the employee provides support, and who meet one of the following criteria: a) A natural born child of the employee or the employee's domestic partner. A copy of the child's state-issued birth certificate, containing the employee or domestic partner as a parent, is required. b) A legally adopted child of the employee or the employee's domestic partner. A copy of the child's state-issued birth certificate and the court adoption record is required. 01 Leisure Travel Programs

A copy of the child's state-issued birth certificate verifying the employee s spouse as the parent and a state-issued marriage certificate to the employee are required. (1) Ex stepchildren do not qualify for travel privileges unless legal adoption has taken place prior to age 18 and proper documentation is provided. d) A child for whom a court appoints the employee, spouse, or domestic partner as legal guardian. A copy of the child's state-issued birth certificate and the court documentation is required. (1) Employee must alert the travel center and make necessary updates to travel profile should anything change or be altered in the future with the guardianship prior to reaching age 18, appointment or documentation. (2) Privileges for a child under guardianship are treated as a natural born or adopted child for purposes of partner travel eligibility and may continue as such until they age out of mainline partner privileges or get married. D. Permanently Disabled Adult Children 1) A permanently disabled adult child qualifies as an eligible pass rider if the child: a) Is unmarried, b) Has been continuously incapable of self-sustaining employment because of a developmental or physical disability occurring prior to age 24, and c) Is continuously dependent upon the employee for support. 2) Interline travel privileges for permanently disabled adult children are limited to those airlines whose interline contracts allow travel for permanently disabled adult children. E. Full-Time Students 1) Children who meet one of the above criteria for a dependent child may extend the age at which they qualify for travel privileges if they are full-time students (must carry at least 12 credit hours per term) at an accredited institution. 2) A copy of their current college registration or a letter from the registrar is required twice annually in order to maintain travel privileges. a) Proof of spring registration submitted in December authorizes travel privileges for the student through the following August 31. b) Proof of fall registration submitted in August authorizes travel privileges for the student through December 31. c) Employees are responsible to submit updates on a regular basis and may upload documentation at Travel Page > Travel Profile > Update Student/Military Status. F. Military Dependents 1) Children who meet one of the above criteria for a dependent child may extend the age at which they qualify for travel privileges if they are active military. 01 Leisure Travel Programs

NOTE: The definitions of dependent children and student/military status vary by partner airline. Refer to the specific sections of this manual for additional information. Parents 1) Parent eligibility requirements a) Eligible employee parents include the following: (1) A biological father or mother. A copy of the employee s state-issued birth certificate containing the parents names must be submitted to confirm parents' names. (2) A stepparent who is married to one of the employee s biological parents. A copy of a state-issued marriage certificate must be submitted along with a copy of the employee s birth certificate that includes parents names, to confirm the biological parent's name. (3) An unmarried surviving spouse of a biological parent. A copy of a state-issued marriage certificate must be submitted along with a copy of the employee s birth certificate that includes parents names, to confirm the biological parents name. (4) Adoptive parents who legally adopted the employee prior to their 18th birthday. (a) Biological parents are not eligible for travel privileges when the employee is adopted. b) Guardians, foster parents, and in-laws are not eligible for travel privileges. 2) Assigning parent travel privileges a) Alaska Airlines (1) Employees may assign two parents per employee. Parents may be any combination of parents or stepparents per travel year. b) American Airlines (1) Employees may assign two parents per employee. Parents may be any combination of parents or stepparents per travel year. c) Delta Air Lines (1) Employees may assign parent travel privileges to as many as four parents who meet the eligibility requirements. 01 Leisure Travel Programs

23 TRAVEL DRESS CODE AND GUIDELINES Page: of 2 PARAGRAPH PAGE 1. SkyWest Airlines' Travel Dress Code Travel Dress Guidelines SkyWest Airlines' Travel Dress Code A. SkyWest Airlines travel dress code for non-revenue travelers matches the travel dress code of the partner airline for which the flight is operating. For example, a SkyWest flight operating as Delta Connection requires travelers comply with the Delta Air Lines non-revenue travel dress code. B. Employees are responsible for educating their pass riders regarding dress requirements of the airline on which they are traveling. C. Regardless of the airline, non-revenue travelers are representatives of SkyWest Airlines and should dress appropriately. D. Refer to the specific sections of this manual for the travel dress code of the partner airline. Alaska Airlines Travel Dress Code American Airlines Travel Dress Code Delta Air Lines Travel Dress Code United Airlines Travel Dress Code 2. Travel Dress Guidelines A. Travelers who are improperly dressed may not be accommodated on a flight. B. Travelers should dress for the most restrictive airline travel dress code when an itinerary involves multiple airlines. C. The travel dress code may be more restrictive for premium classes. Travelers may not be accommodated in first or business class if not properly attired. D. Ultimately, proper travel attire is at the complete discretion of the gate agent working the flight and all non-revenue passengers are expected to respect the decisions made, in a professional manner, and without incident. 01 Leisure Travel Programs

Bereavement/Emergency Travel Policy Bereavement/Emergency Travel Eligibility Bereavement/Emergency Travel Procedure Bereavement/Emergency Travel Policy A. In the event of the death or imminent death of an immediate family member, an employee currently participating in a dedicated partner travel program may qualify for bereavement/emergency travel on that airline. B. Bereavement/emergency travel priority and eligibility is in accordance with and subject to approval by the mainline partner. C. Bereavement/emergency travel is a privilege. Misuse of bereavement/emergency travel may result in corrective action up to and including suspension of pass privileges and termination of employment. NOTE: One emergency travel instance is extended per specific family member. If emergency travel is utilized in the event of imminent death, it cannot be used another time for the same family member. 2. Bereavement/Emergency Travel Eligibility A. Refer to the specific partner airline sections of this manual for bereavement/emergency travel eligibility. Requests for bereavement/emergency travel must be made by a manager to the Travel Center. 1) The manager must submit an to travelcenter@skywest.com requesting bereavement/emergency travel on behalf of their employee. 2) The must include: Names of those traveling Name of the immediate family member whose death is imminent or who is deceased Relationship to the employee Travel information including flight numbers and dates B. The Travel Center requests the upgrade from the partner airline. C. Upon approval, the Travel Center books travel on the appropriate airline and responds to the manager. 01 Leisure Travel Programs

All bereavement/emergency travel requires follow-up supporting documentation. 1) The employee must provide supporting documentation within 14 days of the travel request. 2) A death certificate, obituary, or funeral program must be submitted by mail, fax, or to the Travel Center. In the case of an imminent death, the employee may provide a note from the doctor on the doctor s stationary confirming the impending death. 01 Leisure Travel Programs

Active SkyWest employees who are employed a minimum of six months are eligible for the SkyWest Zonal Employee Discount (ZED) agreements. B. Previously-issued ZED tickets are only valid for travel through the last day of active employment with SkyWest Airlines. C. Employees with more than two parents on their travel profile will be required to choose the two parents they would like to have ZED privileges on each booking platform (myidtravel and ID90T) at the beginning of each calendar year. 2. ZED Defined A. Zonal Employee Discount agreements between airlines allow employees to purchase reduced-rate, standby tickets for leisure travel on other airlines. B. Zonal Employee Discount fares are fixed/standard fare levels (High, Medium, Low) applicable to global zones based on mileage. C. Zonal Employee Discount fares are based on nine global zones, each representing a set number of miles. Zone 1 covers segments up to 450 miles. Zone 9 extends up to 9,999 miles. As long as you access the website from the link on SkyWest Online > Travel Tab > Ticketing > ZED Tickets-myIDTravel, a unique login is not required. B. ID90Travel can be accessed at SkyWest Online > Travel >Ticketing > ZED Tickets ID90. 1) The initial login is your employee number (just the digits with no leading zeros). 2) The initial login is the employees first initial and last name, all together and all lowercase. Furloughed employees are eligible for continued travel privileges for a specified period of time defined in the official furlough letter, following active employment. An employee s spouse/domestic partner, dependent children, Travel Companion/Enrolled Friend/Registered Guest, and parents also qualify for continued travel if they are listed on the employee s Travel Authorization (TA) at the time of furlough. B. Eligibility for continued travel is subject to the following: 1) Employee must be in good standing (no counseling statements for the past six months) and work through the end of their active employment. 2) Employee must return ID badges to their supervisor at the end of active employment. 2. Travel Restrictions a) When ID badges are not returned, continued travel privileges are immediately terminated. A. The following restrictions apply to furloughed employee travel: 1) Activation or membership fees required for participation in travel programs must be paid in full with the understanding that employees have continued travel following the date of furlough for a period of time specified in the official furlough letter. 2) All travel must be completed before the end of the specified continued travel period. 3) Furloughed employees are not eligible for bereavement/emergency travel. 3. Return to Active Service A. When a furloughed employee, who has previously paid partner airline travel fees, returns to active service within the effective period of that fee payment (prior to the new opt-in period for UA, enrollment period for AA, or the employee's anniversary date for DL), no new fee is required. Active seasonal employees are eligible for bereavement/emergency travel and interline travel. 3. Seasonal Furlough Employee Travel Privileges A. Seasonal Furlough employees are eligible for continued travel privileges for 90 days following the end of their seasonal employment with certain restrictions. B. Seasonal Furlough employees must be in good standing (no counseling statements for the past six months) and work through the end of the season. C. Seasonal Furlough employees must return ID badges to their supervisor at the end of seasonal employment. 1) When ID badges are not returned, employee continued travel privileges are immediately terminated. 4. C. New hire employees are eligible for travel privileges as soon as eligibility is verified and the required documentation is received, and the employee has successfully completed a clearance process. D. SkyWest employees are eligible for coach class travel only. 4. Travel Year and Enrollment A. Alaska uses the calendar year as the travel year. B. There is no enrollment requirement for Alaska travel privileges. 5. Membership Fee A. There is no membership or enrollment fee to use Alaska travel privileges. 6. Employee Identification Number for Travel A. Employees use their five-digit SkyWest employee number when required. 7. Eligible Pass Riders A. The following are eligible pass riders: Employee Spouse/Domestic Partner Qualifying, unmarried dependent children Employee's parents B. Two parents are allowed to access travel privileges on Alaska each travel year. When there are more than two parents listed on your SkyWest Travel Profile, the first time the Alaska listing tool on SWOL is accessed each calendar year, you will be required to choose two parents you would like to have Alaska travel privileges for the remainder of the travel year. 1) All parents on your travel profile will show as No under the AS column, until the annual designation is made. 8. Dependent Children A. 37 9. Domestic Partner and Imputed Income ALASKA AIRLINES LEISURE TRAVEL Page: of 4 A. Travel for domestic partners and their pass-eligible dependent children are charged imputed income tax. This means the fair market value (i.e., the base yield fare) of the travel provided is included in the gross income of the employee and applicable FICA and federal, state, and local income taxes are withheld on the additional income. NOTE: When imputed income is included in an employee s paycheck, the employee s net paycheck is less than it would be without the imputed income as FICA taxes and applicable income taxes are withheld based on the imputed income amount. 10. International Taxes A. Any international taxes and fees are collected through payroll deduction and inactive employees are billed through Accounts Receivable. 11. Guest Passes (Buddy Passes) A. SkyWest employees are not eligible for Alaska Guest Passes. 12. Bereavement/Emergency Travel A. Bereavement/Emergency travel is not available on Alaska Airlines. 13. Alaska Retirement Privileges A. SkyWest employees are not eligible for Alaska retirement privileges. International Travel Guidelines A. The following guidelines apply to international travel: Page: of 8 1) Employees traveling internationally are responsible for securing proper international documentation prior to travel. All documentation must be available for the agent at check-in. 2) International fees and taxes are country/destination specific and are billed through an invoice on SkyWest Online after travel has taken place. 3) Many international departure cities require that additional fees be paid before a passenger can depart from the city. These fees range from approximately $6 to $35 depending on the city and often vary by season or day of the week. Unaccompanied Minors (UMNR) Non-Revenue Policy and Procedures A. UMNR fees apply for employees and their eligible pass riders. B. The following restrictions apply when a minor child is traveling non-revenue and unaccompanied. 46 B. In Flight Etiquette 7. Baggage USING ALASKA LEISURE TRAVEL Page: of 8 1) Non-revenue passengers are expected to be courteous and considerate of airline personnel and especially revenue passengers, who make travel privileges possible. 2) Every effort will be made by the Customer Service agent to accommodate employees on their desired flight, but revenue passengers always have first priority. 3) Inconveniences, such as flight delays, being asked to relinquish an assigned seat, or being denied a meal, should be accepted quietly and graciously. 4) In all instances, employees and their eligible pass riders should sit in their assigned seats and not request other passengers relocate for their convenience. a) Employees and their eligible pass riders may be separated on the plane. Employees should prepare for this or take another flight that will better accommodate their needs. A. Non-revenue travelers are not charged for the first two checked bags. All excess baggage and overweight charges must be paid. B. On full flights, a Customer Service agent may ask non-revenue travelers to gate check their carry-on bags to allow sufficient space in the overhead bins for revenue customers. 8. Pet Travel A. American-dedicated SkyWest employees who would like to utilize American travel must enroll in the program each year. B. Enrollment is based on the calendar year (January 1 to December 31), unless otherwise communicated. C. Employees pay $75 annually to enroll. Each eligible pays $50 annually with no fee cap. NOTE: Enrollment is optional for American-dedicated employees, but once elected, fees paid or committed to be paid are non-refundable. In addition, once elections for the travel year are acknowledged and submitted, they are no longer able to be canceled or changed unless a life-changing event occurs. 5. Eligible Pass Riders A. The following are eligible pass riders: Employee Spouse/Domestic Partner (Domestic Partner is subject to imputed income) Qualifying, unmarried dependent children Parents One Registered Guest, when employee is unmarried (subject to imputed income) 6. Dependent Children A. A qualifying child who meets one of the criteria of a dependent child is eligible for non-taxed travel on American flights: 1) Through age 18, if unmarried. 2) Unmarried full-time students from age 19 until 12:00 a.m. on the child s 24th birthday. B. Unmarried children from age 19 until 12:00 a.m. on the child s 24th birthday that are not enrolled in school are eligible for travel on American flights but will be subject to imputed income. NOTE: Children of domestic partners are always subject to imputed income. C. The SkyWest Travel Center must be notified to nullify travel privileges for any dependent child who has a change in student or marital status. 1) Failure to remove a child who no longer meets eligibility requirements is considered fraudulent use of travel privileges, and the employee will be subject to corrective action. 7. Children ages 5-14 traveling alone are required to use the Unaccompanied Minor (UMNR) service. The UMNR fee is waived for pass travelers. UMNR is also available for children but is not required. C. UMNRs may travel system-wide on American flights under the following conditions: 1) Age 5-7 years: Nonstop flights only. 2) Age 8-14 years: Nonstop, direct, and connecting flights through hub cities only. 03 American Airlines Leisure Travel Program

American-dedicated SkyWest employees board at top priority on SkyWest-operated American Eagle flights. American-dedicated employees must check in as AAC and must indicate that they are SkyWest employees on a SkyWest-operated flight to the gate agent. The gate agent then manually boards SkyWest employees first on SkyWest-operated flights. 1) When eligible travelers are flying with the employee on SkyWest-operated flights, they travel at the employee s priority. Otherwise, they stay in order of where they appear on the list with the AAC priority. B. American-dedicated SkyWest employees on mainline and other American Eagle flights board at AAC. AAC priority allows American-dedicated SkyWest employees to board any American flight ahead of American Guest Travelers and other airlines. C. When eligibles are flying with the employee, they board at the employee s priority. D. Eligibles not with employees also have the AAC priority and are accommodated where the priority appears on the standby list. 5. Travel Dress Code A. Dress code is casual so a relaxing journey can be enjoyed. If attire is appropriate and in good taste for the revenue customers, then it should be acceptable. NOTE: Employees and eligibles listed through the Travel Center must check in with a ticket or gate agent and not at the kiosk or online. 64 4. Pass Year and Renewal Dates DELTA LEISURE TRAVEL A. Eligibility for all employees is renewed on an annual basis. B. The one-year travel period is referred to as the pass year. C. Delta's pass year resets on the employee's anniversary date. Page: of 8 D. Employees can decide to participate in the program at any time throughout the year. NOTE: Enrollment and/or renewal is optional, but once elected, fees paid are non-refundable, although they may be applied when switching between dedicated and non-dedicated status in the same pass year. In addition, once elections for the pass year are submitted and approved on your SkyWest Travel Authorization (TA), they are no longer able to be changed unless a life-changing event occurs. 5. Annual Fee for Travel Privileges (PPR Activation Fee) A. Employees who choose to enroll in Delta travel benefits must pay an annual PPR activation fee. 1) Delta-dedicated employees a) The PPR activation fee is $100 per employee, plus an additional $50 per pass rider with a maximum cap of $200, plus a 7.5% excise tax. b) Non-Delta-dedicated employees are entitled to have two parents (any combination of natural or stepparents) activated under their $50 PPR activation fee. To avoid an interruption in travel privileges, the fee must be paid on or before the employee s anniversary date. a) When an employee does not pay the fee prior to their anniversary date, all Delta leisure travel pass privileges for the employee and their eligible pass riders are suspended on the employee's anniversary date and remain invalid until the payment is made. 04 Delta Air Lines Leisure Travel Program

65 DELTA LEISURE TRAVEL Page: of 8 b) When the payment is received and processed, travel privileges are reinstated, but this may take several days. c) Employees must not allow travel privileges to be invalidated if any of their eligible pass riders plan on using travel privileges on or near the employee s anniversary date. C. Employees pay their PPR activation fee online through Delta TravelNet. TravelNet can be accessed on SkyWest Online > Travel > DL TravelNet. D. A credit card is the only acceptable form of payment. E. Gate agents are unable to view and verify eligible travelers on an employee's TA. If an employee waits to pay the fee at the counter, the maximum amount of $ is charged, regardless of the number of family members on the travel authorization. 6. Employee Identification Number for Travel A. All employees/retirees are assigned a Delta Primary Pass Rider (PPR) Number. B. Delta-Dedicated Employees 1) All qualifying Delta-dedicated employees are assigned a Delta PPR number beginning with 400, followed by their five-digit SkyWest employee number (0 s before the number if there are not five digits in the number) with a 0 at the end. Each person in the employee s PPR file is issued a personal ID number at the end of the PPR (known as a two-digit designator). Example: Emp #12345 Emp: Spouse: C. Delta Non-Dedicated Employees 1) SkyWest Delta non-dedicated employees who are not eligible for Delta-dedicated travel are assigned a Delta PPR number beginning with 410, followed by their five-digit SkyWest employee number (0 s before the number if there are not five digits in the number) with a 0 at the end. D. All PPR numbers are located on SkyWest Online > Travel > Travel Profile. Anyone traveling must know this number. It is the employee's responsibility to ensure their eligible pass riders know their PPR number and individual two-digit designator. 7. Eligible Pass Riders A. The following are eligible pass riders: Employee/Retiree Qualifying Spouse/Domestic Partner/Travel Companion Qualifying unmarried dependent children Employee's parents 1) Dedicated employees may have up to four parents on the travel profile. 2) Non-dedicated employees may have any combination of two natural and/or stepparents activated each travel year. 04 Delta Air Lines Leisure Travel Program

This must be done through your Travel Profile on SkyWest Online > Travel Page > My Travel > Travel Profile. B. Requests to add or reinstate an eligible pass rider for travel privileges must be submitted one month prior to the employee's anniversary date. 1) Changes are effective on the employee's anniversary date. NOTE: Your anniversary date is too late to make changes. Example: An employee with a date of hire of October 31st must submit all changes by October 30th. 8. Travel Companion (Delta Air Lines Dedicated Employees Only) A. Employees/retirees may designate another individual as their Travel Companion. 1) A Travel Companion is eligible for unlimited non-revenue space available (NRSA) travel privileges on Delta and Delta Connection, similar to those of a spouse or qualified domestic partner. 2) A Travel Companion travels at a yield fare cost. 3) A Travel Companion travels at the same boarding priority as the employee/retiree. 4) Employees/retirees have the sole responsibility to inform their Travel Companion of Delta's travel policies and ensure they comply with all Company travel regulations. B. Eligibility and Enrollment 1) Employees are eligible to designate a Travel Companion if they are eligible for Delta-dedicated travel privileges. 2) Employees cannot simultaneously have both a Travel Companion and a spouse/domestic partner. a) Employees may request to switch between a spouse/domestic partner and travel companion or change to a new Travel Companion, 30 days prior to the employee's anniversary date to become effective on the employee's anniversary date. The deadline is the day BEFORE the hire date anniversary. b) When a Travel Companion is added in place of a spouse, the spouse forfeits all Delta-dedicated travel privileges, including emergency and interline travel. 3) A new hire employee or an employee who has never had a Travel Companion may designate a Travel Companion at any time as long as the current year's PPR Fee has not been paid. No supporting documentation is required.. No supporting documentation is required. 4) Employees may add a Travel Companion to their TA via SkyWest Online > Travel > Travel Profile > Add Traveler. Travel Companion is an option in the drop-down list. 5) Once designated, the Travel Companion remains authorized until changed. 6) A Travel Companion can be removed from an employee's TA at any time. 7) A new Travel Companion may only be designated once per travel year. When a new Travel Companion is requested after the employee's anniversary date, it must be determined if the current year's PPR fee has been paid. 67 DELTA LEISURE TRAVEL Page: of 8 8) When an employee is no longer eligible for Delta-dedicated travel privileges or adds a spouse or domestic partner to his/her Travel Authorization (TA), travel privileges for a Travel Companion cease. 9) An employee may designate another Travel Companion immediately if a designated Travel Companion dies. Employees must contact the Travel Center for documentation requirements. 9. Dependent Children A. A child who meets one of the criteria of a dependent child is eligible for travel on Delta Air Lines flights: 1) Through age 18 if unmarried. a) Travel for any dependent child ceases at 12:00 a.m. on the child's 19th birthday unless the child is classified as a full-time student or in active military. 2) Through age 23 if unmarried and a full-time student at an accredited institution. a) Travel for a full-time student ceases at 12:00 a.m. on the child's 24th birthday. 3) Through age 23 if unmarried and in active military duty. a) Travel for an active military dependent ceases at 12:00 a.m. on the child s 24th birthday. B. The SkyWest Travel Center must be notified to nullify travel privileges for any dependent child who has a change in student/military status or marries. 1) Failure to remove a child who no longer meets eligibility requirements is considered fraudulent use of travel privileges, and the employee will be subject to corrective action. 10. Domestic Partner and Imputed Income A. Travel for domestic partners and their pass-eligible dependent children are charged imputed income tax. This means the fair market value (i.e., the base yield fare) of the travel provided is included in the gross income of the employee and applicable FICA and federal, state, and local income taxes are withheld on the additional income. NOTE: When imputed income is included in an employee's paycheck, the employee's net paycheck is less than it would be without the imputed income as FICA taxes and applicable income taxes are withheld based on the imputed income amount. B. Any international taxes and fees are collected through payroll deduction and inactive employees are billed through Accounts Receivable. C. To get an estimate for the amount of imputed income, multiply $0.036 by the number of miles flown for domestic travel, $ for international travel, and $ for Alaska/Hawaii. NOTE: Delta Buddy Passes are not valid for travel on SkyWest-operated flights for other mainline partners. C. All Delta-dedicated employees are eligible for the Buddy Pass program. 1) New hire employees are eligible for the Buddy Pass program after one year of employment. 2) Retirees are not eligible for the Buddy Pass program. D. Each employee is allocated six one-way or roundtrip authorizations per year. 1) Delta Buddy Passes are allocated on the employee s anniversary date and do not roll over from year to year. 2) Buddy Passes not authorized (issued to a pass rider and ticketed) by the employee s anniversary date are not available when the new allotment is allocated. E. Delta s TravelNet website is used for Buddy Pass travel administration. Flight Days Allotment A. Domestic Flight Days 1) Delta-dedicated employees/retirees, spouse, domestic partner, eligible dependents, and parents receive unlimited flight days within Delta's domestic network (North and South America). B. SkyWest-operated Delta Connection S2 Boarding Priority Flight Days (All employees) 1) Employees, their spouse/domestic partner/travel Companion, and dependent children are eligible for annual travel at an upgraded S2 boarding priority on SkyWest-operated Delta Connection flights. a) Employees with less than 10 years of service are awarded three S2 flight days per year. b) Employees with 10 or more years of service are awarded six S2 flight days per year. NOTE: Retirees and parents are not eligible for S2 priority. 2) An S2 pass is valid for travel in one direction within a 24-hour period. Travel back to the origin city on the same S2 pass is not permitted and requires the use of an additional S2 pass from the employee's allotment. Connections using an S2 pass must be on the next available flight. 3) Employees are penalized $150 per person per flight for S2 travel in excess of the allotted days. 4) After listing for a flight on Delta s TravelNet, S2 priority is selected by the pass rider at the time of travel at the Delta kiosk. C. Transoceanic International Travel (Delta-dedicated employees only) 1) Employees/retirees, their spouse/domestic partner, dependent children, and parents are allotted six Delta transoceanic flight days free of segment fees or mileage charges per year. a) Travelers are responsible for taxes and other fees. 2) One flight day is defined as one transoceanic flight departure. 04 Delta Air Lines Leisure Travel Program

70 DELTA LEISURE TRAVEL Page: of 8 3) Employees/retirees, their spouse/domestic partner, dependent children, and parents who travel beyond their allotted six transoceanic flight days pay a flat $150 service fee (plus applicable taxes) for each Delta flight segment utilized beyond the allotted travel days. 4) Payment for flight segments in excess of the allotted flight days is automatically deducted from the employee's paycheck after completing travel. 5) Employees list for all Delta transoceanic flights on TravelNet using their PPR number. D. S2 and free transoceanic international flight day allotments are renewed on the employee s employment anniversary date. E. Flight days must be used during the employment year to which they are allotted. The allotment may not be carried over from one employment year to the next. F. Employees are responsible for tracking their usage of flight days. Flight days may be checked via TravelNet. G. Route Systems 1) For pass and reduced-rate travel purposes, Delta has four route systems: Domestic System European System Asian System Transoceanic System 13. Bereavement/Emergency Travel All routes within and between the United States, Puerto Rico and the U.S. Virgin Islands; Canada; Mexico; the Caribbean Islands (including the Bahamas and Bermuda); Central America; and South America. All routes within and between Europe, the Middle East, and India. All routes within Asia. All routes between the Domestic System, the European System, the Asian System, and Africa. A. An employee must be eligible for Delta-dedicated travel to qualify for bereavement/emergency travel. B. The employee, spouse/domestic partner, eligible children, and parents of the employee who are listed on the employee's TA are eligible for bereavement/emergency travel. C. Travel Companions are eligible for bereavement/emergency travel for the death of an immediate family member of the employee. Travel Companions pay a yield fare. D. Bereavement/Emergency Travel reservation will be created by the SkyWest Travel Center and then must be sent to Delta Airlines for ticketing. Seventy-two hours should be given, for the ticketing process, whenever possible. E. Bereavement/Emergency Travel standby boarding priority is S1A. F. Refer to the Bereavement/Emergency Travel Procedure to request bereavement/emergency travel. 14. Delta Retirement Privileges A. The following guidelines apply to international travel: 1) Employees traveling internationally are responsible for securing proper international documentation prior to travel. All documentation must be available for the agent at check-in. 2) Transoceanic international fees and taxes and transoceanic service charges are payroll deducted. 3) Many international departure cities require that additional fees be paid before a passenger can depart from the city. These fees range from approximately $6 to $35 depending on the city and often vary by season or day of the week. Employees should check with Delta s ticket counter upon arrival in all international cities to determine the amount of the fee and whether it must be paid in local currency upon their departure from that city. 4) Children under age 18 traveling to another country may need written approval from both parents. 5. Unaccompanied Minors (UMNR) Non-Revenue Policy and Procedures A. The accompaniment policy may be different for Delta codeshare airlines. Employees should check with the operating airline for information regarding their UMNR policy. NOTE: Children age 5-7 years will only be accepted on nonstop or through flights with no change of aircraft. Age of Child Under 5 Years Restrictions Children under 5 years of age may not travel unaccompanied on a pass Years May travel unaccompanied on nonstop flights domestic only (the domestic route system includes Canada and the Caribbean) Delta accompaniment services are not available to non-revenue/reduced-rate passengers and no service charge will be collected. EXCEPTION: Employees participating in the Airline Ambassadors, Make A Wish Foundation, or other similar humanitarian efforts may be the accompanying adult for a confirmed child passenger in these instances. d) On-duty employees are not allowed to be the accompanying adult for any non-revenue/reduced-rate minor child. 04 Delta Air Lines Leisure Travel Program

73 USING DELTA LEISURE TRAVEL Page: of 8 e) Minor children accompanied by adults with a lower boarding priority/seniority are processed from the standby list at the same boarding priority/seniority as the accompanying adult. f) No approvals or exceptions are made for children under the age of 15 to travel unaccompanied on a transoceanic flight (including flights to and from Mexico, Central or South America). g) Parents/adults may escort an unaccompanied minor child to the gate after obtaining clearance from a ticket counter agent. D. Multiple Infants Traveling with an Eligible Pass Rider 1) One infant must be listed in the same reservation as the accompanying adult non-revenue passenger using the same process as listing an adult non-revenue pass rider. Travel Dress Code A. NOTE: Seat request cards cannot be sent to a mobile device for non-revenue travel. 8) When checking bags, take the printed seat request to the ticket counter, and an agent will tag the bags. 9) Check-in through can be done 24 hours in advance of flight time. 10) It is never permitted to check-in for multiple flights departing from the same city. 11) Employees must not use online check-in until they are certain they intend to travel and are physically near their departure city. 12) In the event travel plans change, the employee must cancel their check-in by canceling the reservation on TravelNet or by calling Canceling check-in is critical to prevent a delay as agents attempt to locate the employee. 84 UNITED LEISURE TRAVEL 4. Travel Year, Travel Enrollment Period, and Renewal Dates Page: of 6 A. The United travel year runs from February 01 to January 31, unless otherwise communicated. B. Beginning January 2019, the United travel year will run on a calendar year, from January 01 to December 31. C. The opt-in period may be determined as a full calender year or a portion thereof. D. All employees are eligible to enroll or renew their participation in the program annually during the travel enrollment period at the end of each year. 5. Opt-in NOTE: Opt-in for dedicated and non-dedicated employees is an optional process, but once elected, fees paid or committed to be paid are non-refundable. In addition, once elections for the travel year are acknowledged and submitted, they are no longer able to be canceled or changed unless a life-changing event occurs. A. Employees must make any changes to their Travel Profile prior to opting in for United travel benefits. B. Non-dedicated employees must opt in (elect) to receive United travel benefits. Employees must opt in and pay any applicable opt-in fee during each opt-in period if they wish to receive and maintain United travel privileges. Privileges expire at the end of each opt-in period and do not automatically roll over into the next. 1) New hire employees must opt in within 90 days of their hire date. C. All employees must opt-in their Enrolled Friend(s) each year, as they do not automatically roll over into the next year. D. Dedicated employees must opt out or make the opt out designation for the spouse during the opt in period, in order to have the option to add a second Enrolled Friend during that travel year. E. Employees with more than two parents on their SkyWest Travel Profile must designate which two parents should be opted in for the upcoming travel year. Employees dedicated to the United Express operation pay no opt-in fee for themselves, spouse/domestic partner, children, and parents. B. Employees not dedicated to the United Express operation pay $200 (per employee/eligible) opt-in fee for themselves, spouse/domestic partner, children, and parents. C. UA-dedicated employees choosing to do so may enroll and opt in up to two Enrolled Friends each calendar year. The opt-in fee for the first Enrolled Friend is $100. NOTE: The opt-in fee is not prorated for new employees. F. The decision to opt in and the deadlines established during the opt-in period are final. When fees remain unpaid after the deadline, they are automatically deducted from the employee's paycheck. G. The opt-in fee is nonrefundable. Employment termination or nonuse of the United Airlines Leisure Travel Program for any reason does not grant the employee a refund. 7. Employee Identification Number for Travel A. All employees are assigned a six-digit United identification number that begins with either an N or an A. B. Employees identification numbers are located on SkyWest Online > Travel > Travel Profile. C. Employees use the identification number for accessing employeeres to check flight availability, status, and to create a standby listing. D. Employees may use their confirmation number to check in on or at United s airport check-in kiosks. 8. Eligible Pass Riders A. The following are eligible pass riders: Employee/Retiree Qualifying spouse/domestic partner/enrolled Friend(s) Qualifying, unmarried dependent children Employee s parents B. Requests to add or reinstate an eligible pass rider for travel privileges should be submitted during the opt-in enrollment period. 9. Enrolled Friend Program A. United's Enrolled Friend Program provides eligible employees and retirees the opportunity to register an Enrolled Friend for leisure pass travel privileges for a calendar year. An Enrolled Friend is a known friend or family member who is registered by an eligible employee or retiree. Enrolled Friends receive space available leisure pass travel privileges on United and United Express, although applicable service and/or segment charges apply. B. Single employees may add two Enrolled Friends. Employees/retirees with a spouse or domestic partner may add one Enrolled Friend. An employee with a spouse or domestic partner may choose to remove their spouse or domestic partner from their United travel privileges and add a second Enrolled Friend. This designation must be done during the Open Enrollment period or you must utilize the chat feature to chat with the Travel Center during the Open Enrollment period each year to commit to this designation in writing in order to hold the second Enrolled Friend position open for an individual to be added at a later date. 1) Enrolled Friends are eligible for unlimited, non-revenue space available (NRSA) travel privileges on United and United Express. 05 United Airlines Leisure Travel Program

C. Non-dedicated employees may only opt in one Enrolled Friend in lieu of a spouse. D. Eligibility and Enrollment 1) Employees and retirees can register a new Enrolled Friend(s) each calendar year, or they can keep the same Enrolled Friend(s) from the previous year. All employees must opt-in their Enrolled Friend(s) each year, as they do not automatically roll over into the next year. 2) Because of the fees associated with the Enrolled Friend Program, employees must designate and opt in their Enrolled Friend(s) each year during the opt-in period. 3) A new hire employee or an employee who has never had an Enrolled Friend(s) may designate an Enrolled Friend(s) at any time. Employees are permitted one Enrolled Friend per calendar year and a second Enrolled Friend (if eligible), regardless of opt-in period. 4) Employees may add an Enrolled Friend(s) to their Travel Profile via SkyWest Online > Travel > Travel Profile > Add Traveler. UA Enrolled Friend #1 and UA Enrolled Friend #2 are options in the drop-down list. 5) An Enrolled Friend(s) can be removed from an employee s Travel Profile at any time. However, a new Enrolled Friend(s) cannot be added until the next annual travel enrollment period at the end of the year. 6) When an employee is no longer eligible for travel privileges, travel privileges for the Enrolled Friend(s) cease. 7) When an Enrolled Friend passes away, a new Enrolled Friend may be designated immediately. The employee must contact the Travel Center for documentation requirements. 10. Dependent Children A. A child who meets one of the criteria of a dependent child is eligible for travel on United Airlines flights: 1) Through age 21 if unmarried. a) Travel for any dependent child ceases at 12:00 a.m. on the child's 22nd birthday unless the child is classified as a full-time student. 2) Through age 25 if unmarried and a full-time student at an accredited institution or in active military duty. a) Travel for any student or active military dependent ceases at 12:00 a.m. on the child's 26th birthday. 05 United Airlines Leisure Travel Program

The SkyWest Travel Center must be notified to nullify travel privileges for any dependent child who has a change in student/military status or marries. 1) Failure to remove a child who no longer meets eligibility requirements is considered fraudulent use of travel privileges, and the employee will be subject to corrective action. 11. Service Charges A. United charges a fee for all travel in premium cabins (First/Business Class). Estimates of these charges can be found when listing for a United flight. B. Travel service charges may be paid in advance by credit card at the time of listing or by payroll deduct. C. United charges base segment fees for non-dedicated employees/eligibles and for any parent and Enrolled Friend(s). 12. Imputed Income A. The Internal Revenue Code does not recognize a domestic partner relationship in the same way it does a marriage for tax purposes. Under current law, employees are taxed on travel privileges provided to a domestic partner who does not qualify as a dependent under the Internal Revenue Code section 152(d). B. The fair market value of travel privileges for a domestic partner is reported as additional income to the employee. This is considered as non-cash or imputed income, which is added to the employee s other income and taxed accordingly. C. The employee s net paycheck will be less than it would be without the imputed income since the imputed income is subject to applicable income tax withholding. D. The employee's W-2 Form includes total imputed income along with other compensation in box 1. Retired employees are issued a Form 1099-Misc to report the imputed income. 13. Bereavement/Emergency Travel A. An employee's employment status must be active and they must elect to opt in for the current travel year to qualify for bereavement/emergency travel. B. The employee, spouse/domestic partner, and eligible children are eligible for bereavement/emergency travel. C. Refer to the Bereavement/Emergency Travel Procedure to request bereavement/emergency travel. D. United Airlines provides NRPS travel to the employee and their eligible pass riders to the location of the bereavement/emergency travel request. NRPS travel is booked at PS7B priority. The return segments are NRSA travel. 14. United Retirement Privileges A. When listing a UMNR for travel, essential information must be entered into the record at check in. Such information includes: The name, address, and phone number of the individual bringing the child(ren) to the airport The name, address, and phone number of the individual picking up the child(ren) at their destination Required SSR (special service request) entries (UMNR SSR and SSR comment, which includes child's gender and age) D. Unaccompanied minors age four and younger are not permitted to travel space available. Children 5 through 14 years of age who are traveling space available may travel unaccompanied in Economy on flights within the U.S., Canada, Mexico, U.S. Virgin Islands, and Puerto Rico and in Economy on all other routes, as long as the following apply: 1) The child must be brought to the airport of departure by a parent or responsible adult who remains with the child until departure. 2) Airport procedures for NRSA UMNRs are the same as revenue UMNRs, including: a) The parent or adult must furnish satisfactorily completed form of evidence to the Customer Service agent at check-in verifying the child will be met by another parent or responsible party at the destination (allow sufficient time to complete the form). b) The Customer Service agent ensures paperwork is accurate and completes a UMNR envelope. c) A button or sticker is attached to the UMNR's outermost garment. 3) The flight must be nonstop from boarding point to destination. The discount will be applied when the ticket is purchased. E. All revenue fare rules apply and will vary based on the type of fare purchased. F. Employees, retirees, and their eligible pass riders traveling on a myuadiscount ticket may not standby for flights contained in their myuadiscount ticket itinerary. G. For more information regarding the myuadiscount (revenue-confirmed ticket) Program, refer to the United Discount Leisure Travel Program Manual. (Not to be used for company business travel.) 11. Billing and Refunds A. Employees using the United Leisure Travel Program are held accountable for travel expenses incurred. B. Canceled reservations automatically generate a refund for any prepaid travel (First/Business Class, international taxes). The refund should post to the credit card within two to three business days. C. Refunds for prepaid First/Business Class, where coach was the class traveled in, must be manually requested by going to > Reservations > Refund Policy > eticket Refund. 1) Complete all required information including the ticket number(s). Ticket numbers can be located: a) On the reservation confirmation . b) By going to > Reservations > Change/View Existing Reservations. Access the reservation with the PNR number and last name and select View Receipt to obtain the ticket number. 2) The manual refund should post to the credit card within seven business days. D. Payroll deduction for travel expenses is available and the preferred method of billing due to the fact that you are not charged if you do not end up taking the flight(s) for any reason. E. Failure to pay invoiced charges may result in suspension or revocation of travel privileges and the submission of charges to a collection agency. F. Employees are not permitted to establish or change the price of dependent, parent, or Enrolled Friend travel or add to the price because of any personal expenses incurred by the pass rider.
