1 SSO Summary Report For overflows between: 1/1/16 And 12/31/16 Overflow 387 Town of Camillus County owned Camillus Forcemain 01/08/ /13/ ,183,400 Ground Approximate six inch hole in forcemain. 388 Town of Onondaga Town owned sewer on Barker Hill Rd. 01/13/ /13/ ,000 Ground Grease and rags blocked sewer line approximately three feet from pump station. 389 Town of Dewitt Town of Dewitt owned force main 01/15/ /15/ Ground Two inch force main from Town of Dewitt owned Tow Path Commons Pumping was leaking. 390 Town of Geddes County owned 02/16/ /16/ ,000 Onondaga Lake Rain and snowmelt in conjuction with power outages and fluctuations in the area resulted in brief power losses. The main Motor Control Circuit Breaker tripped in the station during the power outages, resulting in the pumps not being able to run by either generator or incoming power. Flow overtopped the diversion structure and was conveyed to the lake. 06/08/2017 Page 1 of 5

Rain and snowmelt resulted in high flows one of the two existing pumps experienced mechanical problems. Portable pump was utilized due to elevated sewer levels until pumps at Ley Creek could catch up to the surcharged condition. 392 Village of Solvay County owned manhole at Milton Ave & Chemung St 02/17/ /18/ ,000 Ground Water main break contributed to collapse of County owned manhole, resulting in a partial obstruction and not all flow being able to be carried through the sewer main. 393 Town of Geddes County owned 02/24/ /25/ ,000 Onondaga Lake Rain and snowmelt accompanied with one pump being out of service for mechanical issues. 394 Town of Camillus Town owned sewer on Richlee Drive 05/31/ /31/ Basement 43 Richlee Drive Roots grease and rags partially restricted flow in Town owned 8-inch Vitrified Clay sewer. 396 Village of Camillus Village owned Sewer on MacLaughlin Street 06/06/ /06/ ,000 Basement 114 Maclaughlin Street Grease and grit build up in Village owned eight inch sewer restricted flow resulting in basement 397 Town of Salina Town owned sewer in ROW along the Thruway 06/27/ /27/ Basement 115 Park Place Grease build-up in Town of Salina owned eight-inch sewer main restricted flow resulting in basement 398 Town of Clay Town owned sewer on Amalfi Drive 07/16/ /16/ ,000 Basement 5414 Carina Circle Grease build up in Town of Clay owned asbestos cement sewer restricted flow resulting in basement Oak Orchard 06/08/2017 Page 2 of 5

Baldwinsville-Seneca K 401 Town of Camillus Town owned sewer on Hinsdale Road 08/19/ /19/ Ground / Geddes Brook Watershed Town of Camillus owned eight inch sewer partially blocked resulting in oveflow from manhole, possible pipe failure. 400 Town of Lysander Town owned Whispering Oaks Force Main 08/19/ /19/ Seneca River National Grid auger hit force main due to inaccurate location of infrastructure during pole installation. Onondaga County is investigating the cause of the inaccurate utility markout. Baldwinsville-Seneca K 402 City of Syracuse James Rigi Trunk Sewer 08/30/ /30/ ,000 Did not discharge to receiving water Sewer lining contractor was installing liner on 12 inch diameter James-Rigi Trunk Sewer. Back up pump system failed to operate causing surcharge condition upstream of sewer plug. Water backed into the basement of 829 Coughlin Ave and on the floor of 219 Lamson Street. Once the pumnp failure was observed, the plug was deflated to allow water to flow downstream. 403 Town of Geddes County owned West 09/18/ /19/ ,000 Onondaga Lake Total of 1.58 inches of rain inches of rain falling between 8:40 and 10:35 PM inches of rain fell between 8:40 and 9:10 PM, as recorded by Weather UnderGround's Solvay, NY rain gauge. 06/08/2017 Page 3 of 5
4 406 Town of Geddes County owned West Side pumping station ,050,000 Onondaga Lake 2.09 inches of rain on 10/20/16 combined with Motor Control Center #2 failure at station which caused three of the six pumps to be out of service. 407 Town of Geddes County owned ,440,000 Onondaga Lake 5.62 inches of rain beginning on 10/20/16 of which 3.62 inches came down in periods of heavy showers during 10/21/ Town of Clay Town owned sewer on Davis Rd North Basement 7961 Davis Rd North Grease and rags partially restricted flow in Town of Clay owned 10-inch Asbestos Cement sewer, in conjunction with 5.62 inches of rain beginning on 10/20/16 of which 3.62 inches came down in periods of heavy showers during 10/21/16. Oak Orchard 409 Village of Manlius Village owned sewer on Military Drive 2.00 Basement 107 Military Drive Possible roots and rags restricting flow in Village of Manlius owned 8-inch sewer, in conjunction with 5.62 inches of rain beginning on 10/20/16 of which 3.62 inches came down in periods of heavy showers during 10/21/ Town of Salina Town of Salina owned sewer on Chestnut Hill Dr Basement 108 Chestnut Hill Drive Abandoned sewer lateral connection failed and partially blocked flow on Town of Salina owned 8- inch vitrified clay sewer in conjunction with 5.62 inches of rain beginning on 10/20/16 of which 3.62 inches came down in periods of heavy showers during 10/21/16. 06/08/2017 Page 4 of 5

Flow has remained below 30 MGD since 11:00 PM on 10/22/16 and no overflow has been observed. 412 Town of Salina County owned Ley Creek Pumping 10/29/ /29/ ,996,000 Ley Creek Bypassed wastewater pumping station directly to Ley Creek in order to remove and repair bad section of Ley Creek Force Main. 413 Village of Manlius County Owned West Field "Back- Lot" Sewer 10/31/ /31/ Limestone Creek Watershed Roots and grease restricted flow resulting in surcharge and overflow from manhole. 414 Town of Geddes County owned West 11/04/ /04/ ,000 Onondaga Lake Power interuption caused pumps to trip out. By the time OCDWEP personnel could get onsite and get the station running on generator power, eight minutes of overflow time occurred. 415 Town of Geddes County owned West 11/27/ /27/ ,000 Onondaga Lake Main MCC breaker tripped causing loss of all pumps. 417 Town of Salina Town owned Sewer Right of Way 12/31/ /31/ Basements 517 & 519 Sunflower Drive Grease partially restricted flow in a Town owned eight inch vitrified clay tile sewer, resulting in basement backups. 06/08/2017 Page 5 of 5
