2 Coronary revascularisation in England A geographical profile of hospital admissions. Authors: Michael Goldacre, David Yeates, Leicester Gill, Nick Hall, Matt Davidson Published by: Unit of Health-Care Epidemiology, University of Oxford, 2013 This document provides a geographical profile of hospital admission for Coronary revascularisation in England. The geographical areas covered are the standard local authority areas of England. The period covered is 01/01/2009 to 31/12/2011. The data are from Hospital Episode Statistics (HES). HES includes data on all NHS hospital admissions (including admissions for day case care). The maps show: (a) The spell-based admission rates per resident population for each geographical area per year, expressed as an average annual rate. The spell is the hospital admission, representing the continuous time spent in a hospital Trust from admission to discharge. A person admitted several times for the condition is counted as many times as he/she has admissions. The data in the maps are shown as quintiles of geographical areas, with the onefifth of areas with the lowest rates shown in the lightest colour and the one-fifth of areas with the highest rates shown in the darkest colour. (b) The person-based admission rate per resident population for each geographical area per year, counting people once (and only once) per year in which they were admitted, expressed as an average annual rate. If the person has another admission for the condition in a different year, he/she is counted once again (and only once) for each different year of admission.
3 (c) The ratio of the number of spells with the condition (a, above) per person admitted for the condition (b, above), expressed as an annual average. The two error-bar charts provide a plot of the values of the average annual spell-based rate and the average annual person-based rate (counting people once, and only once, in each year) for each geographical area, each vertical line representing one geographical area. It shows the point estimate for the standardised rate for the area with its 95% confidence interval. At a glance, the error-bar charts show the spread of rates across England and the extent to which variation across the country is statistically significant. The first scatter-plot shows the number of spells per person plotted against the person-based rate. The second scatter-plot shows the average Index of Multiple Deprivation (IMD 2004) score for each geographical area plotted against the person-based rate (counting people once, and only once, in each year) for each area in England. The first table shows the individual values for each geographical area, with the areas ranked in alphabetical order. For each area, the information in the table provides data about spells showing the observed number of spells (Obs.); the expected number of spells (Exp.), obtained by applying national age-sex-specific rates to the age-sex distribution of the geographical population; the age-standardised rate, calculated by standardisation, expressed per population with the 95% lower and upper confidence limits for the rate; and the same measures for people admitted (counting people once, and only once, in each year) per population per year.
4 The second table shows the geographical areas ranked according to the standardised spell-based admission rate (ranked from highest to lowest rate). The third table shows the geographical areas ranked according to the standardised person-based admission rate, in which people are counted once per year (ranked from highest to lowest rate). Some technical notes: Dataset used: UHCE-built linked file for England,, cips file, analysis template 2 Diagnosis codes used: Specialty codes used: Operation codes used: K40.1-K50.9 Diagnosis position: Operation position: any Period covered: admissions from 01/01/2009 to 31/12/2011 Source of admission: any Exclusion of successive admissions: - Age/sex restriction: Ages: 0-119, Sexes: both Restrictions relating to presence of other diagnoses or operations: none Restrictions relating to length of stay: none Restrictions relating to type of hospital: those in England only, otherwise none Place of residence: that on HES record at the time of admission; in the analysis of person-based rates someone who moves home and has an admission from two different areas is counted twice once for each area.
