KG Nordic Navigation I Ltd Maintenance of ship and Objective evidence discovered in an expanded ISM examination revealed that the company/vessel failed to fully implement the requirements of the Safety Management System. The PSCO discovered the following major non-conformities: Failure of the Master and crew to properly implement the company s environmental policy relating to compliance with MARPOL Annex I, multiple inconsistencies noted within the Oil Record Book including no record of accumulation or discharge of oily waste to or from the sludge tank from July to December No actions taken by the crew to ensure identified essential remained operational, taking into account severe cold weather Rescue boats Rescue boat was not in working order and ready for immediate use. During operational test of the rescue boat, the crew was not able to start the outboard motor. The rescue boat's fuel system was frozen due to water in the fuel line. The rescue boat is required per the Safety Equipment Certificate Emergency lighting, etc The emergency source of electrical power is a generator. The emergency generator failed to start using both the primary and secondary means of starting. In this condition the emergency generator is not capable of starting upon failure of the main source of power and connecting automatically to the emergency switchboard for providing power to all those services that are essential for safety in an emergency.

According to the posted operating instructions, "water mist pumps, suction and discharge valves, and system's delivery ball valves to be "ALWAYS OPEN". Upon further examination, the CO2 smoke detector above the number 1 auxiliary engine was covered with a plastic cap and the CO2 nozzle for the boiler on the next level of the engine room was covered with a rag.

Vessel's EPIRB and corresponding hydrostatic release have not been serviced as required and are both currently expired Shipborne navigational Vessel is required to have a functioning Automated Identification System (AIS) on board. The vessel's AIS is unable to broadcast vessel information nor able to display vessel specific information of other vessel's in the surrounding area Emergency equip. for 2-way Vessel is required to have onboard two VHF lifeboat commun. radios. Vessel does not have the required lifeboat radios onboard at this time Shipborne navigational Vessel is required to have a Long Range Identification Tracking (LRIT) system installed onboard. The vessel at this time does not have the requisite installation onboard Facilities for recept marine Vessel is required to have a NAVTEX installation safety info onboard the vessel. The vessel at this time does not have the requisite NAVTEX system installed Reserve source of energy Vessel is required to have an emergency source of power for its radio installation onboard. At this time the vessel has no source of power for its radio installation onboard.

A PSCO observed a crewmember open the discharge valve to the system while the lead PSCO examined the suction valve. The Chief and Second Engineer stated they have no required maintenance procedures and the last time the system was inspected was in dry dock (July 2013). The vessel operates with a periodically unattended machinery space. Americas Marine Management Services, Trans-Mambo Fire prevention Approximately six feet of pipe for the CO2 fire fighting system along the port side of the #1 cargo hold is missing, preventing delivery of CO2 into the cargo hold Rescue boats Ship's crew was unable to launch the rescue boat because the davit system could not fully clear the boat cradle Cargo and other hatchways Steel hatch covers throughout the vessel are wasted with holes, there is a hole in the main deck just forward of the #1 cargo hold hatch cover, and two cargo hold accesses located aft of the #2 cargo hold are severely wasted.

PSCO requested crew to perform a functional test of the detection system; however the crew was not able to demonstrate proper operation of the detector system Steering gear A means of communication shall be provided between the navigation bridge and the steering gear compartment. Phone communication between bridge and steering gear compartment was inoperative and radio communication was inadequate to relay steering commands Other (Crew) The PSCO believes that there are clear grounds for believing that the crew is not familiar with essential shipboard procedures relating to the safety of the ship. Engineering staff was unfamiliar with: CO2 alarm for evacuation of engine room; testing of fixed fire detection system. Chief Engineer was unfamiliar with steering alarms and oily water separator operation. Chief Mate was unfamiliar with cargo hold fire suppression system. Crew was unable to start the emergency fire pump without the maser departing the bridge to assist. The ship s crew was unfamiliar with basic SOLAS training and abandon ship procedures Maintenance of ship and Objective evidence discovered in an expanded ISM examination revealed that the company/vessel failed to fully implement the requirements of the Safety Management System. The PSCO discovered: nonfunctioning smoke/heat detectors; no testing procedures for fixed detection system; no maintenance performed as required for the OWS; unfamiliarity of crew with specific duties assigned - crew could not properly set up the emergency fire pump and start without masters assistance. Missing safety - no EEBDs were located in the engine room as required/indicated on the fire control plan.

The OWS was not able to cycle through start-up mode and enter into the operational mode. Crew attempted to operate more than 15 times, however the OWS would not operate for more than seconds before going into alarm mode. Grandville Shipholding SA Charterers Nippon Yusen Kaisha (NYK) Lines Pollution report - annex I PSCO discovered an unauthorized bypass of the oily water separator. Expanded MARPOL exam produced evidence indicating the ship was discharging oil directly overboard using an unapproved arrangement through the soot collection tank. This arrangement did not allow the oil content meter to monitor and stop the overboard discharge of effluent in excess of 15 parts per million as designed and approved.

The OCM was not able to monitor and stop the overboard discharge of effluent in excess of 15 parts per million as designed and approved Maintenance of ship and Company shall establish a safety and environmental policy which described instructions and procedures to ensure protections of the environment in compliance with international legislation. Objective evidence of the safety management system (SMS) procedures, records of testing, and manufacturers guidelines proved that engine department was not familiar with proper operations, testing requirements of the oily water separator/oil content meter as well as reporting requirements for OWS failures and defects. The SMS failed to address cold weather environment procedures for critical operations - the emergency generator coolant was frozen and the prime mover was not capable of operating without damage to the system. The rescue boat was not able to start in a reasonable amount of time due to ice in fuel line.
