On behalf of the entire staff of Indiana Volleyball Camps, LLC, we welcome you to historic Bloomington - the official home of Indiana University and the Indiana Hoosiers! This summer, each camper will receive an exclusive Indiana Volleyball Camp T-Shirt! In addition, campers enrolling in The All-Skills Camp, The Elite Position Camp and/or the Hoosier Team Camp will receive an additional gift as a token of our appreciation for selecting us this summer! Note: Teams participating in the High School Team Challenge will not receive an Indiana Volleyball Camp T-Shirt as part of their registration. A copy of our Information for Campers, Rules and Expectations, Parent Consent, Medical Waiver & Concussion Awareness, Request for Refund and Request for Airport Transportation forms have been included in this packet. Parents should review all information with their camper(s) before coming to camp. Note: By registering for any camp in the 2017 Summer Camp Series, campers and their legal guardians agree to abide by the rules and policies set forth in the Official Camp Manual. CHECK-IN LOCATIONS Camp check-ins will be held at one of the locations listed below. Campers should check-in and check-out from the same location unless otherwise noted. Please refer to your particular session on the next page for the location of check-in. If attending an overnight camp, the camp check-in will be at the housing dorm. Note: If the camp you selected is an overnight camp but you are commuter (not participating in the overnight option), you will still check-in at the designated location (see next page) for your specific camp. 4 INFORMATION FOR CAMPERS AIRPORT PICKUP: Campers who need transportation to and from the Indianapolis Airport will need to make arrangements with Indiana Volleyball Camps, LLC no later than two weeks prior to camp. Indiana Volleyball Camps, LLC will make every effort to accommodate each request, but service is not guaranteed. Campers requesting airport transportation ($60.00 one-way, $ round-trip) will need to complete the Request for Airport Transportation form found in the Official Camp Manual. CAMP STORE: Indiana Volleyball Camps, LLC will be selling a variety of snacks and drinks along with exclusive Indiana Volleyball Camps gear and merchandise throughout the entire summer camp lineup. Campers are encouraged to bring money to camp each day. Campers participating in an overnight camp will have the opportunity to purchase late night pizza as well. CANCELLATIONS / REFUNDS: All cancellations and/or request for refunds must be received two weeks before the specified camp. The Request for Refund form (found in the Official Camp Manual) must be submitted for consideration. If the cancellation notice is received before the above-said deadline, full payment will be returned except a 25% administrative fee. The administrative fee is not refundable for any reason. If the cancellation notice is received after the above-said deadline, 0% of the paid balance will be returned and any deposits will be forfeited unless there is a medical circumstance in which proper documentation is required. Indiana Volleyball Camps, LLC reserves the right to change and/or cancel any camp at any time in which a full refund will be honored. CHILD SAFETY & PROTECTION: Indiana Volleyball Camps, LLC is committed to creating a safe environment that provides protection of all children and teens participating in the 2017 Summer Camp Series. For an appropriate level of supervision, a strict 12:1 ratio will be maintained. All members of the staff (including court coaches, camp mentors, camp counselors and overnight security) will have passed a legal and thorough background check before being allowed to work at camp in any capacity. In addition, all members of the staff will wear official Indiana Volleyball Camps, LLC identification while working at camp. Finally, all staff will have been properly trained to handle any and all emergency situations. Should a camper suspect any illegal or inappropriate behavior from another camper or staff member, it is asked they immediately report the activity to the Camp Director or a staff member. Indiana Volleyball Camps, LLC will adhere to the Indiana University Public Safety & Institutional Assurance Policy on Programs Involving Children (this policy can be found online at and is also included in the back of the Official Camp Manual). CONFIRMATION: It is strongly encouraged to register online for the 2017 Summer Camp Series. Upon processing each mail-in registration (Friday mornings), an initial notice will be sent via . Confirmation s are only sent once payment has been collected OR when the registration form is received. A follow-up confirmation notice along with the Official Camp Manual will be sent several days before camp. Once the registration is processed, all camp-related correspondence will be sent via . It is advised that campers and/or parents check their s frequently in the immediate weeks and days leading up to camp. DISCOUNTS: Indiana Volleyball Camps, LLC is proud to offer discounts for campers this summer. Indiana University Athletics employees and Indiana University employees are all eligible for a special camp discount. In addition, Indiana Volleyball Camps, LLC will provide a sibling discount for siblings registering in the same camp. A detailed list of available discounts is included in the Official Camp Manual. Please note that no other individual or group discounts will be honored. If a camper is not able to actively participate in the full camp session, they may request a pro-rated discount for camp. In this instance, the cost of camp is divided by the number of days (multi-day) or hours (single day) the camp is hosted. If a camper needs to request a pro-rated fee, they must do so no later than 14 days prior to the beginning of camp. Please contact Indiana Volleyball Camps, LLC to begin the process (IndianaVolleyballCamps@gmail.com). DISMISSAL POLICY: There will be no refund for any camper dismissed from camp. Grounds for dismissal include but are not limited to vandalism; leaving a facility or dormitory unsupervised; the use of alcohol, drugs or tobacco products; harassment and not following verbal instructions/rules. Indiana Volleyball Camps, LLC reserves the right to send a camper home if they do not conduct themselves in a manner that will enhance an atmosphere of proper and safe learning, good sportsmanship and willingness to follow directions.

5 ENROLLMENT: These camps are open to any and all entrants, limited only by number, age, grade level and/or gender. Space is limited for each camp and enrollment is granted on a first-come, first-serve basis. Early enrollment is strongly encouraged to ensure participation. Indiana Volleyball Camps, LLC is not affiliated with Indiana University, the Indiana University Department of Intercollegiate Athletics or the Monroe County School Corporation. All staff is independently hired by Indiana Volleyball Camps, LLC pending the approval of a legal and thorough background check. FACILITIES: The main site for all camps will be University Gym, the official home of Indiana Volleyball! Other possible facilities include Assembly Hall, Cook Hall, the School of Public Health (HPER) as well as Bloomington High School North. While all facilities are air-conditioned, the likelihood of cool conditions is unlikely with the number of campers expected this summer. FREQUENTLY ASKED QUESTIONS: Available online ( is a complete rundown of Frequently Asked Questions. Campers and/or their legal guardians are encouraged to consult the thorough rundown of questions before contacting Indiana Volleyball Camps, LLC with a specific question. If the question is not answered, please contact us. INSUFFICIENT FUNDS NOTICE: Once we receive notice from our bank that your method of payment was not accepted due to "insufficient funds" or any other null-and-void reason, we will notify you via with an electronic copy of the notice we received for your records. We expect that the remaining balance be paid in full as soon as possible. We will ask for the entire camp fee plus a $10.00 charge as your first notice. Should you still have an outstanding balance due 15 days of initial notice, we will raise the penalty cost to $25.00 plus the initial charge of camp as your second notice. Should you still have an outstanding balance 30 days from the initial notice, the penalty cost is risen to $50.00 plus the initial charge of camp as your third and final notice. In addition, Indiana Volleyball Camps, LLC will begin discussions with small claims court for proper collections. MEALS: Meals will not be provided during any day camps or the High School Team Challenge; campers will be expected to provide their own lunches. All campers will receive lunch and dinner during The All-Skills Camp, The Elite Position Camp and the Hoosier Team Camp. Overnight campers will also receive breakfast during the camp. Commuter campers will be responsible for their own breakfast if participating in an overnight camp. Note: It is the camper s responsibility to alert Indiana Volleyball Camps, LLC of any dietary and/or food allergies that exist. T o successfully plan healthy meals for everyone, please contact Indiana Volleyball Camps, LLC (IndianaVolleyballCamps@gmail.com) no later than 14 days prior to camp. If you re not sure what the menu will consist of, we advise bringing a lunch and dinner that satisfies your specific needs. A discount will not be honored should a camper bring their own lunch. MEDICATIONS / SPORTS MEDICINE: The Parent Consent, Medical Waiver & Concussion Awareness Form is REQUIRED for all campers (one per camper per summer). Campers will not be permitted to participate without this form completed and presented on the first day of camp. This form can be found online at Indiana Volleyball Camps, LLC reserves the right to deny any camper with pre-existing medical and/or physical history. Campers may only carry medications to camp if such medications are to treat a potentially life-threatening condition. Typically, we have only permitted campers to carry EPI-pens and inhalers for asthma. In order to bring any medication (prescription or over-the-counter) to camp, a parent or guardian must make a request to the Camp Director. Each situation will be considered on a case-by-case basis. To make a request, or to disclose the intended use of an EPI-pen or inhaler, please contact Shane Wilkinson by at IndianaVolleyballCamps@gmail.com. In order to adequately assess your request, please contact us no later than 14 days prior to the start of camp. Indiana Volleyball Camps, LLC is not responsible for any medication mismanagement or abuse. Note: A First Responder will be present at every camp to help monitor all physical activity and provide minor medical treatment if need be. All campers will need direct permission from parents/guardians before the First Responder can administer any treatment or medication ( Parent Consent, Medical Waiver & Concussion Awareness Form ). All employed First Responders are CPR/AED certified. Gatorade specialists will also be present to monitor hydration levels and needs of all participants.

6 OVERNIGHT ACCOMMODATIONS: Indiana Volleyball Camps, LLC is offering three overnight camps in which campers will be housed at either the McNutt Residence Center or the Foster Residence Center (depending on the camp). For added security, Indiana Volleyball Camps, LLC will maintain a strict 12:1 ratio during overnight camps. The camp staff will escort the group to and from the residence center and dining hall at all times. In addition, the camp s designated First Responder will stay overnight in the residence center should a medical emergency arise. All overnight security will have official Indiana Volleyball Camps, LLC identification when monitoring the halls throughout the evening. If registered for an overnight camp, Indiana Volleyball Camps, LLC will handle all rooming assignments for the camp. Campers are allowed to choose their roommates and every effort to honor requests will be taken, but not guaranteed. Campers are not allowed to leave the residence center for any reason without an escort. Should a camper leave the residence center without supervision for any reason, they will be immediately dismissed from camp with no refund offered. If a camper is attending a day camp and does not live within a commuting distance, please contact one of the listed hotels below to make a reservation at the camper s expense. The Holiday Inn Bloomington 1710 North Kinser Pike Bloomington, IN (812) The Hampton Inn 2100 North Walnut Street Bloomington, IN (812) PARKING: Parents or guardians are welcome to stay and watch camp. There are several parking spots surrounding University Gym, Assembly Hall, Cook Hall and Bloomington High School North to use during day camps. Free parking near the School of Public Health (HPER) will not be available. While Indiana University Parking Enforcement is aware of our camp dates, it is not guaranteed that a ticket will not be issued and Indiana Volleyball Camps, LLC will not reimburse the cost of a parking ticket. For campers participating in an overnight camp, parking passes will be available at check-in for $15.00/night. It is strongly advised that campers who drive themselves to camp should purchase an overnight parking pass to eliminate the risk of multiple parking tickets and/or their vehicles being towed. Should an overnight camper decide to drive themselves to camp, they must surrender their keys upon checking-in to camp. RECRUITING / SCHOLARSHIPS: While Indiana Volleyball Camps, LLC is not affiliated with the Indiana University Department of Athletics, NCAA recruiting rules prohibit us from offering scholarships or other financial aid to campers. In addition, it is not permissible for boosters to provide expenses for individuals to attend any of the camps listed in the 2017 Summer Camp Series. Expenses includes, but are not limited to, lodging, meals, transportation, and/or camp registration fee. NCAA rules allow our staff to have recruiting conversations with campers during camp. Campers are advised not to bring recruiting videos or other recruiting materials to camp. The staff of Indiana Volleyball Camps, LLC must continue to follow NCAA recruiting rules that establish when coaches may and may not have contact with a prospective student-athlete (a student who has begun ninth grade). Once camp has been officially dismissed, tours of Indiana University are available, but must be set-up ahead of time with Jason Oliver, Indiana Volleyball Assistant Coach & Recruiting Coordinator. T-SHIRTS: This summer, each camper will receive an exclusive Indiana Volleyball Camp T-Shirt. A camper must register and check-in to camp before receiving their t-shirt. Should a camper register and/or pay but not attend the actual camp, they will forfeit claim to the participation t-shirt. No refund will be offered. WHAT TO BRING: All campers should come dressed and ready to play no jewelry, shirts tucked in and hair pulled away from face. Water and Gatorade will be provided, but campers are encouraged to bring their own water bottles. While kneepads are optional, gym shoes are required. If a camper needs to borrow volleyball-specific training gear for the duration of camp (provided the training gear is necessary for participation in camp activities), arrangements can be made while supplies and last. Campers should contact Indiana Volleyball Camps (IndianaVolleyballCamps@gmail.com) no less than 14 days before the start of camp to make arrangements. Campers are encouraged to bring money to camp each day. 7 SATELLITE CAMP PROGRAM DATES: Dates are available starting Thursday, June 1st through Friday, July 21st. All satellite camps must be conducted over at least a two-day consecutive period, but no more than three consecutive days. Dates are solidified on a first-come, first-serve basis. Every effort to accommodate all requests will be made, but is not guaranteed. To solidify a date, please contact Paul Koncir at IndianaVolleyballCamps@gmail.com. DISCOUNTS: No discounts will be offered for the Satellite Camp Program. ENROLLMENT: All Indiana Volleyball Camps, LLC satellite camp opportunities must be open to any and all entrants. For example, a high school coach cannot limit satellite camp participation to only Varsity-level student-athletes. Participation can only be limited by number, age, grade level and/or gender. In addition, only those participants who have presented a signed Parent Consent, Medical Waiver & Concussion Awareness Form and paid in full may actively participate. FACILITIES: Indiana Volleyball Camps, LLC is not in charge of scheduling facility dates or securing the proper insurance to host a satellite camp. The primary individual/team hosting Indiana Volleyball Camps, LLC for a satellite camp should have the paid facility scheduled and proper insurance available. In addition, all necessary equipment and supplies (nets, pads, balls, training equipment, etc.) should be provided and available. MEDICAL COVERAGE: Each hosting individual/team must ensure that medical supervision will be provided and paid for during the entire duration of the satellite camp. It is expected that water and/or Gatorade will be available for all active participants and coaches. Indiana Volleyball Camps, LLC will not monitor medications brought to camp by any participants. Indiana Volleyball Camps, LLC shall not be held responsible for any medical emergency that might occur during camp, PAYMENTS: The cost of participation is variable by the number of participants, number of consecutive dates of camp and distance from Bloomington to account for travel expenditures (mileage, hotel, meals, etc.). The final cost of each satellite camp is configured on case-by-case basis and is ultimately decided on by Indiana Volleyball Camps, LLC. REFUNDS: In the unlikely event that a satellite camp is cancelled after payment has been collected from all participants, a refund in full will be provided. Aside from this occurrence, no other refunds will be offered for participating in a satellite camp with Indiana Volleyball Camps, LLC. T-SHIRTS: This summer, each active participant will receive an exclusive Indiana Volleyball Camp T-Shirt. A camper must register and participate in camp before receiving their t-shirt. Should a camper register and/or pay but not attend the actual camp, they will forfeit claim to the participation t-shirt. No refund will be offered.

8 CAMP STAFF The coaching staff from Indiana University Volleyball will serve as the primary instructors and camp directors for the entire duration of the 2017 Summer Camp Series. In addition, former and current Indiana University Volleyball student-athletes will serve as court coaches and overnight supervisors. If numbers warrant outside employment, Indiana Volleyball Camps, LLC will hire additional coaches with collegiate and/or Junior Olympic club experience. All employees will have passed an extensive background check and will wear identification throughout the entire camp duration. SHERRY DUNBAR-KRUZAN PAUL KONCIR JASON OLIVER Sherry Dunbar-Kruzan: With Head Coach Sherry Dunbar-Kruzan at the helm, the Indiana Volleyball program has enjoyed a rebirth and some of its highest success in program history. Entering her 11th season on the sidelines for the Cream and Crimson in 2017, Dunbar-Kruzan has continuously raised the bar of Indiana Volleyball. In 2016, the Hoosiers won at least 17 matches for only the fourth time since 2001 and were NCAA Tournament eligible for the first time since In 2010, Coach Dunbar-Kruzan led the program s first AVCA First-Team All-America honoree Ashley Benson and the Indiana Hoosiers to the Sweet 16, a first for IU Volleyball. The 2010 squad finished the historic season with a record and a 24th national ranking. Before arriving to Bloomington, Dunbar-Kruzan led College of Charleston to an impressive record while capturing the Southern Conference title on three separate occasions and was named the SoCon Coach of the Year twice. An Ellettsville, IN native, Dunbar-Kruzan was a four-year starter at Ball State University. Paul Koncir: Associate Head Coach Paul Koncir will be in his eighth season in Nationally known as elite trainer, Koncir has helped re-establish Indiana s presence in the nationally renowned Big Ten Conference. Koncir helped guide Taylor Lebo to the top of the Career Aces in the Rally Scoring Era with over 128 aces. In addition, he mentored Caitlin Cox to the top of the Single Season Digs and Career Digs categories. He also mentored Samantha Thrower to tie the national record for hitting percentage in a single match, going 15-for-15 in a match against Buffalo. No stranger to the head coaching ranks, Koncir led Towson Volleyball to multiple Colonial Athletic Association Championship appearances. Jason Oliver: Assistant Coach Jason Oliver is entering his second season in Oliver mentored former IU Setter Megan Tallman to the top of the Career Assists in the Rally Scoring Era. Before his arrival to Bloomington, Oliver was head volleyball coach at High Point University. Known as an elite trainer and recruiter, Coach Oliver transformed the High Point program and brought the Panthers to heights never before achieved. In 2010, High Point won their first Big South Conference Title in program history thus earning a spot in the NCAA Tournament. In his eight seasons as head coach, High Point recorded over 125 wins - making Coach Oliver the most successful head coach in program history. 9 RULES AND EXPECTATIONS FOR DAY-CAMP VOLLEYBALL CAMPERS While we hope each of our campers have a great and unforgettable experience with us this summer, safety is our primary concern. Campers adherence to these rules and expectations is extremely critical to making our camps safe, rewarding and fun for all participants. Expectations for Camper Behavior: Follow all instructions of camp staff and immediately consult your camp coach if you are uncertain about any of the instructions. Label all personal belongings, especially equipment. Indiana Volleyball is not responsible for any lost or stolen items while participating at camp. Notify a camp coach as soon as any problem arises, whether it be big or small. Behaviors Not Permitted: The use of profanity or inappropriate language, whether it be written or verbal. Leaving the camp area as designated by the camp director. Indiana University is located in the heart of Bloomington, so campers are not permitted to walk freely around campus or any surrounding areas. Inappropriate use of a cell phone or cell phone camera during camp. Indiana Volleyball Camps reserves the right to confiscate phones for the duration of camp. Damaging or stealing University or other s property. Camp fees do not cover payment for any lost or stolen items. While unacceptable conduct is not anticipated from our campers, violation of any camp rule or expectation could result in disciplinary action, including dismissal from camp. Refunds will not be issued to campers dismissed from camp. Should a camper suspect any illegal or inappropriate behavior, they are expected to report to the lead court coach immediately. We expect that parents or guardians of campers will be available (by cell phone) during the duration of camp in the event that the camp director needs to speak with them about a camper s behavior. Campers and parents/legal guardians should review this important information prior to arriving at camp. 10 RULES AND EXPECTATIONS FOR OVERNIGHT VOLLEYBALL CAMPERS While we hope each of our campers have a great and unforgettable experience with us this summer, safety is our primary concern. Campers adherence to these rules and expectations is extremely critical to making our camps safe, rewarding and fun for all participants. Indiana University is a large campus in the heart of Bloomington. There is adult supervision at all times in the residence center and during all structured camp activities. Campers may have down time during which an adult is available but may not be directly monitoring all activities (in the residence center, during a break or just prior to lights out). Expectations for Camper Behavior (On The Court): Follow all instructions of camp staff and immediately consult your camp coach if you are uncertain about any of the instructions. Follow the camp schedule and report on time for all sessions. Participate in all scheduled activities (including camp meals, if offered). Only the camp director may grant permission to be excused. If you are ill or need medical attention, please consult the First Responder. Attendance will always be taken. Keep your camp coaches informed of your whereabouts at all times. Label all personal belongings, especially equipment. Indiana Volleyball Camps, LLC is not responsible for any lost or stolen items. Notify a camp coach as soon as any problem arises, whether it be big or small. Expectations for Camper Behavior (Residence Center): Staying overnight in a college residence center is a unique experience and privilege at Indiana University. With this comes more responsibility for all overnight campers. Campers are expected to keep their room clean at all times. If there is an issue with a room, campers should contact the residence center supervisor and/or Camp Director immediately to report the issue. Campers should keep track of their personal room key and never loan it to another camper for any reason. When leaving their room (even if only for a small amount of time), campers should take the extra precaution of checking their door. Indiana University, Indiana Volleyball Camps, LLC and the residence center are not responsible for lost or stolen items. If there is suspicion of theft, campers should contact the Camp Director immediately. Campers should use the buddy system, which is your assigned roommate, when in transit. DO NOT leave the residence center without being accompanied by a member of the camp staff. If a camper leaves the residence center for any reason, they will immediately be dismissed from camp. Visitors, especially those of the opposite sex, are strictly prohibited from entering a dorm room...period. Campers are expected to be in their own rooms at the time deemed as lights out, which is normally 11:00 p.m. unless otherwise stated. Campers should NOT take anything from the residence center that does not belong to them. Furniture should not be moved and fire alarms should not be tampered with. The cost of cleaning, replacing or repairing any damaged or stolen University property will be forwarded to the campers who were assigned that specific room. No matter who is deemed guilty, all tenants of the room will be held responsible. Behaviors Not Permitted: The use of profanity or inappropriate language, whether it be written or verbal. Leaving the camp area as designated by the camp director. Indiana University is located in the heart of Bloomington, so campers are not permitted to walk freely around campus or any surrounding areas. Also, campers are not permitted to leave camp early without notifying the Camp Director. Inappropriate use of a cell phone or cell phone camera during camp. Indiana Volleyball Camps, LLC reserves the right to confiscate phones for the duration of camp. Damaging or stealing University or other s property. Camp fees do not cover payment for any lost or stolen items. Possession and/or distribution of indecent literature or images. Possession and/or consumption of alcohol, tobacco, illegal substances, matches, fire lighters, hot plates, fireworks or lethal weapons. While we understand volleyball is an emotional sport, harassment, bullying or intimidation of any nature is strictly forbidden. Excessive noise and/or horseplay. The use of a vehicle. If a camper drives to an overnight camp, they may be required to give their keys to the camp director at check-in. The keys will only be reissued at the conclusion of camp or if there is a family emergency in which guardians must call the Camp Director to coordinate camper dismissal and return. Violations of all state and federal laws.

11 RULES AND EXPECTATIONS FOR INDIANA VOLLEYBALL CAMPS STAFF PAGE 1 Each year, Indiana Volleyball Camps, LLC welcomes numerous campers from all over the country. As a member of our camp staff, you are essentially the heart and soul of our product. While all areas of our camps are crucial to the overall success, it is your role that serves as the most important component of what we are building at Indiana. Your efforts, diligence, high energy and enthusiasm will greatly influence whether each camper has a safe, beneficial and enjoyable camp experience. Briefly stated, your responsibilities as a camp staff member include, but are not limited to: 1. Ensuring the safety of each camper; 2. Following the direction of the camp director and carrying out the camp s supervision and instruction plan; 3. Creating an opportunity for each camper to have a remarkable experience by providing instruction that is appropriate for each camper s age and ability, and encouragement that enables each camper to gain confidence needed to learn new skills; 4. Serving as an appropriate role model for all campers in your care at all times. The Lead Court Coach leading your camp will discuss your responsibilities and assign you specific duties during the pre-camp staff meeting. Ensuring Camper Safety: Camper safety is the paramount goal of Indiana Volleyball Camps, LLC. In order to promote a safe environment for all campers, camp staff should: Always think safety first. If an activity appears dangerous, stop it right away. Read the camp s Emergency Action Procedures and make sure you know what to do if an emergency occurs. Read the Camper Rules and Expectations and review them with your assigned group of campers. Monitor camper behavior to ensure the campers are following the Camper Rules and Expectations. If you are staying in the residence center, review fire alarm evacuation procedures with the campers on your floor. Wear your staff credential whenever you are on duty. Please wear the staff uniform t-shirt while working camp. Maintain a watchful eye for any person who is in a designated camp area but is not a camper or other camp staff member. If you suspect any illegal or inappropriate behavior at any time, please report the activity to the Camp Director immediately. Take attendance as required by the lead court coach. You are responsible for everyone in your assigned group. When escorting campers by foot from one camp site to another, make sure they stay on the sidewalk and only cross the street - as a group - at a designated crosswalk. Immediately report all injuries to the facility First Responder and Camp Director. Carefully monitor hydration levels and mandate frequent water breaks...be persistent. Never be alone with a camper behind closed doors. If you need to speak privately with a camper, bring another camp staff member along. Report the details of the meeting to the Camp Director immediately following the conference. While you are working, only use a cell phone for camp-related business. Unnecessary cell phone use distracts. With that said, it is expected that all group leaders have their charged cell phones available and off silent mode at all times. Following the Camp Director s Instructions: The Camp Director is responsible for overseeing the operation of the camp, ensuring camper safety and supervising all camp staff. Camp staff should: Follow the instructions of the Camp Director, and, if necessary, ask for clarification of any instructions which are not clear. Attend all scheduled camp activities to which you are assigned and be on time. Promptly report any issues or problems to the Camp Director.

12 RULES AND EXPECTATIONS FOR INDIANA VOLLEYBALL CAMPS STAFF PAGE 2 Creating Opportunities for Remarkable Experiences: Creating remarkable experiences for our campers is an important goal of Indiana Volleyball Camps, LLC. Camps Since camp staff have frequent and daily contact with campers, they play an important role in reaching this goal. Camp staff should: Take an active approach to learn names quickly. Campers tend to be more involved when you use their actual name as opposed to hey you! Look for the camper who may have difficulty integrating into camp. Campers who attend camp on their own may need help meeting other campers, particularly at the start of camp. Start each instruction session with an explanation of the session s goal. Remember that not all campers learn the same way or at the same pace. Offer feedback and look for teachable moments (but remember, be vocal with your praise and discreet with your constructive criticism...this is not a tryout). If a drill or activity is not working, ask the Camp Director whether you should change it. Serving as a Role Model: Indiana Volleyball Camp, LLC s proud tradition has been built on the accomplishments and character of its coaches and staff. A mantra we operate under is, You win with people and this statement holds true for our camps. It s crucial to understand that you are held to a higher standard when you wear the Indiana Volleyball Camps, LLC logo and our campers view all of us as important role models. Camp staff should: Be positive and patient (in your spoken words and body language) with all campers, not just the most talented ones. Never use alcohol, drugs or any type of tobacco products at any time while working a camp. Never use foul or inappropriate language at any time while working at camp. Be mindful to immediately stop a camper s usage of foul or inappropriate language as well. Not bring outside guests to any camp site or into the dormitory until the camp has been dismissed. Child Safety and Protection: Indiana Volleyball Camps, LLC is committed to ensuring a safe environment that provides for the protection of children and teens participating in camps. This includes the obligation to perform background checks on all staff, properly train all staff, and immediately report any illegal or inappropriate behavior, including sexual behavior, and allegations of such behavior to law enforcement. Indiana Volleyball Camps, LLC will adhere to the Indiana University Public Safety & Institutional Assurance Policy on Programs Involving Children (a copy has been included in the employee packet and is also available online at By signing this document, I confirm that I have read all of the important information listed in this document and understand my role as an employee of Indiana Volleyball Camps, LLC. I confirm that I was given proper training as an employee and understand what to do if an emergency should arise. 13 2017 SUMMER CAMP SERIES EMERGENCY ACTION PROCEDURES PAGE 1 Chain of Command: The Camp Director, First Responder or Law Enforcement may determine when to remove campers from a playing surface. The use of cell phones is a timely way to communicate this decision to other camp staff. Once the decision to remove campers has been made, the Camp Director will notify the other camp staff of this decision. Weather Forecasts and Severe Weather Monitoring: Prior to each camp session, the Camp Director and First Responder will check local forecasts to determine whether severe weather is being anticipated. During the camp, the Camp Director and First Responder will continue to monitor local forecasts and Indiana University s alert system. IN THE CASE OF POTENTIAL SEVERE WEATHER AND/OR LIGHTNING, ALL CAMP STAFF WILL BE INSTRUCTED TO KEEP THEIR CAMP GROUPS INSIDE AND SECURE UNTIL THE FIRST REPONDER GIVES FINAL APPROVAL TO TRANSIT BETWEEN FACILITIES. IT IS IMPORTANT TO KEEP CAMPERS CALM AND ORDERLY AT ALL TIMES. LIGHTNING SAFETY PROCEDURE Field Clearance: If it is determined that lightning is occurring within 15 miles and is moving towards campus, the First Responder and/or Camp Director will notify camp staff that all campers should be cleared from the field and that they should seek shelter immediately. Transition between facilities may be delayed until conditions are deemed safe. Safe Shelter: Seek shelter in the nearest athletics facility. Avoid being in the highest point of the field and do not take shelter near or under trees and light poles. Wait for the Camp Director to provide further instructions prior to resuming an activity or returning to the field. Managing Victims of Lightning Strike: Survey the scene for safety (lightning victims do not carry charge and are safe to touch). Call 911 and contact the First Responder and Camp Director. If necessary, move the victim with care to a safer location (remember, do not seek shelter under a tree). Evaluate victim and begin CPR if necessary. Confirm that emergency service is on the way. Evaluate and treat for shock, fractures and/or burns. Notify Camp Director. TORNADO PROCEDURE In the event that severe weather is imminent or a warning has been issued, campers and camp staff should immediately take cover. The rule of thumb for safe cover is to find a substantial (concrete/brick) structure with a western exposure. Below is a list of athletics facilities and the specific areas within those structure that could be utilized: University Gym (locker-rooms on bottom level) Assembly Hall (court-level corridors, Cook Hall southeast entrance hallway) Cook Hall (practice facility-level, Assembly Hall northwest entrance hallway) HPER (lower level locker-rooms or lower level stairwell) Residence Centers (lower level bathrooms or emergency stairwell) Bloomington High School North (lower level bathrooms)

14 2017 SUMMER CAMP SERIES EMERGENCY ACTION PROCEDURES PAGE 2 FIRE PROCEDURE In the rare event that a facility catches fire, camp staff will immediately direct campers out of the building using the nearest and safest facility exit in a calm and orderly manner. Camp staff will take a detailed roster check to ensure all campers are accounted for. If a camper is deemed to be missing, contact the lead court coach immediately. The lead court coach for the specific facility will be in charge of calling 911. After 911 has been reached, the lead court coach will report to the Camp Director. Each facility has detailed fire routes posted and should be reviewed by all employees of Indiana Volleyball Camps. Once the situation is under control (campers accounted for, firefighters present, etc.), Camp Director will contact Indiana University. LOST CAMPER PROCEDURE 1.) Camp staff must account for their campers at all times. Camp staff should keep camp rosters with them and regularly check attendance against their roster, particularly in transition times when campers are moving from one location to another. - TEAM CAMP: While we expect high school coaches to assist in supervising their teams, the lead court coach must check with each high school coach to confirm all campers are present before any activity is started. 2.) Upon determining that a camper is unaccounted for (through attendance procedures, notification by another camp staff member or camper or by any other means), immediately notify the Camp Director. 3.) Camp staff and the Camp Director should immediately meet to exchange information on the camper s possible whereabouts. The Camp Director should also check camp records to determine: * Whether the camper reported to camp that day * Whether the camper left early 4.) At this initial meeting, the Camp Director should make note of the following information: * Time * Camper name and age * Where camper was last seen * Detailed camper description (height, weight, clothing, etc.) 5.) The Camp Director and/or a designee will immediately search the area where the camper was last seen and then the entire camp area. This should include meal locations and the residence center. 6.) If the missing camper was not located after steps 1-5 are completed, then: a.) CALL Camp Director should remain at current location and wait for police to arrive b.) Contact parents - Camp Director should contact parents to advise them and determine if a camper was picked up early without notification. c.) Contact Indiana University Athletics / Rebecca Pany. 7.) Camp Director should direct other camp staff to continue search until police arrive. 8.) As soon as police arrive, police will assume control over the search of the camper. Camp Director should have cell phone accessible at all times. When taking roll call before a camp session starts, it s safe to practice the CDD Rule : - Check - Double-Check - Do It All Over Again

18 HEADS UP CONCUSSION IN HIGH SCHOOL SPORTS A FACT SHEET FOR PARENTS What is a concussion? A concussion is a type of traumatic brain injury. Concussions are caused by a bump or blow to the head. Even a ding, getting your bell rung, or what seems to be a mild bump or blow to the head can be serious. You can t see a concussion. Signs and symptoms of concussion can show up right after the injury or may not appear or be noticed until days or weeks after the injury. If your child reports any symptoms of concussion, or if you notice the symptoms yourself, seek medical attention right away. What are the signs and symptoms of a concussion? If your child has experienced a bump or blow to the head during a game or practice, look for any of the following signs of a concussion: SYMPTOMS REPORTED BY ATHLETE Headache or pressure in head Nausea or vomiting Balance problems or dizziness Double or blurry vision Sensitivity to light Sensitivity to noise Feeling sluggish, hazy, foggy, or groggy Concentration or memory problems Confusion Just not feeling right or feeling down SIGNS OBSERVED BY PARENTS/GUARDIANS Appears dazed or stunned Is confused about assignment or position Forgets an instruction Is unsure of game, score, or opponent Moves clumsily Answers questions slowly Loses consciousness (even briefly) Shows mood, behavior, or personality changes How can you help your child prevent a concussion or other serious brain injury? Ensure that they follow their coach s rules for safety and the rules of the sport. Encourage them to practice good sportsmanship at all times. Make sure they wear the right protective equipment for their activity. Protective equipment should fit properly and be well maintained. Wearing a helmet is a must to reduce the risk of a serious brain injury or skull fracture. However, helmets are not designed to prevent concussions. There is no concussion-proof helmet. So, even with a helmet, it is important for kids and teens to avoid hits to the head. What should you do if you think your child has a concussion? SEEK MEDICAL ATTENTION RIGHT AWAY. A health care professional will be able to decide how serious the concussion is and when it is safe for your child to return to regular activities, including sports. KEEP YOUR CHILD OUT OF PLAY. Concussions take time to heal. Don t let your child return to play the day of the injury and until a health care professional says it s OK. Children who return to play too soon while the brain is still healing risk a greater chance of having a repeat concussion. Repeat or later concussions can be very serious. They can cause permanent brain damage, affecting your child for a lifetime. TELL YOUR CHILD S COACH ABOUT ANY PREVIOUS CONCUSSION. Coaches should know if your child had a previous concussion. Your child s coach may not know about a concussion your child received in another sport or activity unless you tell the coach. If you think your teen has a concussion: Don t assess it yourself.
