3 Health & Medical Forms The New York State Department of Health requires a completed medical form for each child PRIOR to the start of camp. Our medical forms meet the New York State Board of Health requirements and are required for campers to attend camp. In addition to completing the medical forms, please provide a copy of your child s immunization record and verification of a physical exam after August Log-on to the Parent Dashboard to complete the online Camper Medical Information Form and to print required Camper Health Care Forms. Camper Medical Information Form Required: Parents/Guardians complete this form online. Camper Health Care Form Printed forms to be signed by parents/guardians and completed/signed by physician: Form 1: Over-the-Counter Medication Consent Form. Required for all campers Form 2: Prescription Medication Consent Form. Only for campers that require prescription medication at camp. Options for Returning Completed Medical Forms to Mohawk Day Camp 1. Scan and upload through the Parent Dashboard 2. Fax: nurse@campmohawk.com 4. Mail: 200 Old Tarrytown Road, White Plains, NY Medications All medications must be administered by our Nurses. Epi-pens can be administered by Nurses and other trained camp staff on an emergency basis. Medications (prescription and over-the-counter) must be sent in original containers along with an authorization from you and your doctor regarding dosage and time of dispensing. Additionally, for campers who require prescription medication at camp, a Prescription Medication Consent Form must be completed and signed by the pediatrician and parent/guardian. Although Mohawk sprays for ticks, we suggest that parents check their children for ticks daily, consistent with Health Department recommendations. For information regarding ticks and Lyme Disease, visit the Westchester County Health Department website at NO MEDICATION will be administered at camp without written permission from you and a completed form from your doctor. Nurses Office: :45am - 4:15pm Dietary Needs & Food Allergies Mohawk s Director and Food Service Team are available to meet with families regarding any special dietary needs. Our Food Service Team adheres to strict allergen control protocols, and has comprehensive ingredient information for all foods provided at camp. If you have further questions, please call the camp. Mohawk has been peanut and tree nut aware for decades! Campers and Parents are not allowed to bring in treats food or beverages of any kind either to camp or on the bus. This is for the safety of campers with severe food allergies, and to avoid a choking hazard on buses! Plenty of food and water is provided by the camp during the day. Lunch Mohawk provides a hot lunch for Full-Day and Mini-Day campers including weekly pizza and a weekly cookout. Substitute choices are available daily (including a variety of sandwiches, yogurt, bagels and pasta). SunButter is also available. Snacks Mohawk provides daily snacks. Campers may not bring in any food including candy, cookies, gum, etc. Protecting children with severe allergies is critical and we cannot verify ingredients of incoming foods. Finishing breakfast on or eating aboard the buses is prohibited for the same reasons. Water Bottles The Westchester County Dept. of Health strongly discourages water bottles because campers lend them to friends, which can transmit disease. Also, water kept in these bottles breeds bacteria. If you feel your child must have a water bottle, make sure it is clearly marked with your child s name and give careful instructions not to share it. Mohawk provides cold drinking water throughout the camp at each activity area! Birthdays Mohawk provides birthday cakes for a small fee. The cake is large enough to serve all campers and counselors within the group. Arrangements should be made at least two weeks in advance, preferably before camp begins. You may complete the Birthday Celebration Form online through the Parent Dashboard. Per the Children s Camp Code, Subpart 7-2 of the New York State Sanitary Code: 1. Mohawk Day Camp is permitted to operate in New York State. 2. Mohawk is required to be inspected twice yearly. 3. Reports on these inspections are filed at: Westchester County Department of Health, 145 Huguenot Street, New Rochelle, NY Phone: (914) Health & Medical Forms Dietary Needs & Food Allergies
4 Aquatics Mohawk Day Camp follows the American Red Cross Learn-To-Swim program. Campers are placed in small swim groups at the start of camp based on their abilities as determined by a Water Safety Instructor. Campers will advance in swim groups during the summer and make progress through the camp season. Our deep water test is designed to ensure that campers are safe and comfortable in deep water, and is conducted by a qualified Water Safety Instructor. Campers must be able to swim the width of the pool comfortably (without struggling) using the crawl, return using a backstroke and tread water comfortably for one minute. Campers who passed the deep water test during the previous camp season may need time to progress to deep water again. Some campers swim in deep water at home or at a club but might not be ready for our deep water requirements, which are designed to ensure our campers safety. Often, campers who pass a deep water test will continue to have instruction in shallow water because the best teaching is done when an instructor is in the water with the camper. In these situations, campers are brought to deep water for specific instruction or for a free swim opportunity. Swim Evaluations / Communication Swim placement and evaluation information will be posted to your camper's Swim Group on the Mohawk 360 app. This is where you can also direct message the aquatics team with any questions you might have. Behavior Expectations Mohawk Day Camp is committed to providing a safe and welcoming environment for all of our campers. To ensure safety and comfort for all, we ask individuals to act appropriately at all times. Our code of conduct does not permit language or actions that can hurt or frighten another person or that falls below a generally accepted standard of conduct. Specifically this includes: Angry or vulgar language including swearing, name calling, and shouting. Physical contact with another person in an angry or threatening way; Any demonstration of sexual activity or sexual contact with another person. Harassment or intimidation with words, gestures, body language or other menacing behavior. Behavior which intends to or results in the theft or destruction of property. Carrying or concealing any weapons or devices that may be used as weapons. Staff are trained and expected to respond to any reported violation of our code of conduct. Please do not hesitate to notify our camp office or director if you need assistance. Mohawk will investigate all reported incidents. Dismissal from a program or termination of enrollment may result from any violation of the code of conduct. No refunds will be given. Food from Home Food from home is not permitted at camp or on the buses. To ensure the safety of campers with serious food allergies, we require that parents do not send in any food to camp or on the bus for any reason. Special dietary needs must be reviewed with our Food Service Manager. Mohawk Gear Camp Today Photos & Videos Rainy Days All campers will receive a Mohawk backpack, t-shirt and sweatshirt included in tuition. Specialty Mohawk apparel can be purchased directly from the Gift LABS online store. A link to their website is provided on the Parent Dashboard or visit Identification of Personal Belongings: Be sure everything is labeled with your camper's first and last name. We will provide you with 30 personalized stick-on-clothing labels for your camper from Label Daddy, which you will receive at Welcome Day. Visit the Parent Dashboard for the Label Daddy link to receive 25% off any additional label orders. Lost and Found: Although we make every effort to be helpful, we cannot be responsible for lost clothing or other personal items. This is why we strongly suggest you put your child s name on everything and do not send anything valuable to camp. To keep camp families connected and informed of all the fun their campers are having at Mohawk, photos will be posted daily to the Camp Today section of the Parent Dashboard for you to enjoy with your family. Campers will take part in a full scheduled program of activities regardless of the weather. We are prepared with a rainy day schedule for each group, which focuses on crafts, indoor games, and special events. Be sure to dress campers appropriately for the weather (i.e. rain coat and boots). Campers who are in 2nd grade or older may be bused to an offsite, indoor location on rainy days. Aquatics Behavior Expectations Food from Home Mohawk Gear Camp Today Rainy Days
5 Transportation AM Bus Pickup Please have your child ready on time. This will reduce everyone s travel time. Bus Counselors are instructed to wait for only 1 minute to keep routes short for our campers. Families who live in apartments are expected to be waiting for pickup at the building entrance or assigned corner in NYC. Note: Transportation times should become consistent after the first 2-3 days of camp. PM Bus Dropoff We ask that you or another responsible person signal the Bus Counselor that you are home to receive your child when he or she is dropped off. You should present your personalized, time-stamped Bus Pick Up Pass from Mohawk 360. You may also screenshot and send this to an alternate caregiver that may assist you with pick up. What happens if you are not home when your child is being dropped off? Don t worry! Your Bus Counselor will try to contact you via cell phone. If you are not at home, the route will continue and then return to your home. The Bus Counselor will only drop your child off with a neighbor if we have parental permission! Bus Counselor Information Your bus counselor will message you via Mohawk 360 the week before camp to introduce him/herself. Your bus pickup and dropoff times will be ed to you on Friday, June 15. Parent Pickup and Dropoff Arrivals At Regular Arrival Time 8:45-9:15am Enter the Lower Lot and turn right up the slope. Your camper will be received by staff, and escorted to his or her Headquarters. Attendance is taken as campers arrive during this time. During the Camp Day Park in the Upper Lot. Check in at the Welcome Table. Do not send your child to his or her Headquarters or Group independently, even if it is within view! This is critical for your camper s safety for two reasons: first, we must record that your camper has arrived and we will escort your child to his or her group. Second, groups may be at an activity area, therefore not at their Headquarters. Early AM Drop-off 7:30-8:45am Regular and occasional early drop-off is available. For staffing reasons, please call ahead if you are dropping off your camper early unless you have made arrangements to do so daily. Enter in the Lower Lot. Our staff will receive and supervise your camper until it is time to meet up with his or her group. Departures Half-Day and Mini-Day Dismissal 12:00pm for Half-Day Campers: Parent Pickup enter via the Upper Lot. 2:15pm for Mini-Day Campers: Optional Mohawk Transportation available if pre-arranged by June 18 (for most locations). Parent Pickup is in the Upper Lot. Early Pickups Call before 12:00pm (and preferably, much earlier) on the day of pick up. Tell your child s Bus Counselor about your plans in the morning, and then you must call our office. Transportation changes should be messaged directly to your bus counselor via Mohawk 360. To avoid confusion, please do not send in paper notes with the bus counselor. Transportation: Busing, Pickup and Dropoff Absences If your child is not coming to camp on a given day, please message your Bus Counselor directly via Mohawk 360. It is not necessary to call the camp office. Important Campers can only be transported in their assigned vehicle. Mohawk does not transport campers for playdates on different routes. For the safety of all of our children, all campers must be checked out by staff before leaving Mohawk Before 3:00pm Park in our Upper Lot. Pick up your camper at the Welcome Table. From 3:00-4:15pm Park in our Upper Lot. Pick up your camper from the upper Dinosaur playground. Late Pickup 4:15-6:00pm Please call our office at least one day before late pickup to ensure we have proper staffing. Park in our Upper Lot. Campers will be picked up from our upper (Dinosaur) playground. If raining, campers will be picked up from our Cottage building, across from the upper playground.
