AMERICAN EAGLE DEICING AUDIT This audit form is to be utilized when accomplishing an audit of station facilities, equipment, personnel, manuals, etc. in accordance with the American Eagle Ground Deicing Manual, Section 1, Quality Assurance requirements. Discrepancies must be recorded at the end of this form. A list of completed deice audits can be found in Web DRS in the Express tab under Express Manual Revisions. MISCELLANEOUS INSTRUCTIONS TO THE AUDITOR Names of personnel in audited functions must be recorded. Observe overall compliance and document findings. Include same as a discussion item in the debriefing. A finding is a deviation from the standard established by the FARs, Company manuals, or this document. The Manager on Duty debriefing must encompass all findings as well as performance excellence. The meeting should be as positive as possible. All findings will be reviewed by top management. The audit will be documented with the submission of an Audit Report. The Audit Report may be a letter format and will contain, at a minimum: The name of the station and the date of the audit. Form WO-005 must be completed for each deicing entity. The auditor s name and contact telephone number(s). A list of findings and/or concerns containing the appropriate manual citations. Finding: A condition that is directly contrary to the procedures contained in this manual Concern: A condition, if not monitored or addressed, that may result in a finding A due date of written response for any corrective action(s) required. A list of individuals to which the report was copied. The carrier responsible for conducting the audit will ensure that follow-up actions result in closure to findings as prescribed by their internal audit procedures. If unsafe conditions exist, which, in the opinion of the auditor, require immediate attention for continued operations, the Manager, Safety must be notified. NOTE An acceptable audit does not relieve responsibility for maintaining consistently acceptable quality control standards and conformance. Audit results shall not be released except as provided by this form and the Quality Assurance/Safety Audit Program. General Information Y N O I N/A Does the General Manager or designated individual have a process in place to ensure an adequate supply of deicing/anti-icing fluid is available to support the deicing effort? (DIM ) Are SDSs immediately available to those who may require information regarding the storage, handling or emergency protection procedures relating to the fluids? (DIM 1-1-7) Does the Manager, Ground Support Equipment develop and maintain maintenance procedures for deicing/anti-icing equipment. (DIM1-1-8) Are all employees responsible for winter operation duties properly trained and qualified with proper documentation readily available? (DIM 1-1-7) Does the annual training include a comprehensive training outline and learning assessment (with all participants attaining a Passing score), and are all personnel qualified to operate deice equipment? (DIM Chapter 5) Has the General Manager assigned a Deice Coordinator who will monitor forecasted weather conditions and adjust assets (personnel and equipment) and/or vendor instructions to prepare for potential deicing operations? At joint use stations (American Eagle/American, or American Eagle /another Part 121 carrier), the Deice Coordinator may be identified by another title or description and may be employed by the joint use carrier. ( DIM through 1-1-9) Is the Deicing Report updated upon station opening and as conditions change. Is equipment G. status, deicing fluid quantities and specific deicing locations entered as required. The name and telephone number of the WOC is also required. This report is mandatory from October 1 st - April 30 th, regardless of weather conditions. From May 1 st September 30 th, the Station Manager will ensure the Deicing Report is updated whenever the local temperature drops below 50 (DIM 1-1-8) Is the manual containing the ground deicing/anti-icing procedures for ground personnel H. current and accessible to employees? (DIM ; ) I Is deicing/anti-icing fluid tested prior to each winterization season? (DIM ) The primary American Eagle glycol tester for fluid condition is the MISCO Model PA203 Palm Abbe Digital Refractometer (TES9638). Other glycol testers may be used if a calibration or functional check program is identified and followed to verify the equipment s accuracy per J. manufacturer s recommendations. All refractometers maintained by the station or vendor will be clearly numbered, either by starting with the number 1, marked with its corresponding truck number or Company-issued serial number. Deicing vendors can also use an alternate numbering method. (DIM 3-1-6) II. Safety and Precautions Y N O I N/A Company-approved fall protection harness and lanyard must be worn by personnel on any open bucket deicing equipment. The lanyard must be attached to an approved anchor point. (DIM 2-1-5) Eye protection must be worn when spraying fluid from open bucket deicers. (DIM 2-1-7) Management Responsibility: Ensure/verify that safety equipment and PPE are used as appropriate during deicing operations. (DIM 2-1-2) Ensure the deice unit maintains a 10 foot clearance between the aircraft, a minimum of 5 foot clearance exists between the bucket and the aircraft, and the speed of the truck when operating bucket does not exceed 4 MPH. 3 II. Safety and Precautions (Continued) Y N O I N/A Is the decision of the method of providing electrical power to the aircraft made jointly between the flight crew and ground crews? (DIM ) If deicing is taking place with aircraft engine(s) running, are propellers feathered or engines idled, crew indicated "Parking Brake Set" and deicing completed with a clearance of 20 feet from engines? (DIM ; ) III. Storage and Transfer Facilities/Equipment Y N O I N/A Are tanks used to store glycol fluids clearly labeled with the fluid type? At a minimum, the label must indicate the chemical base and type of fluid (for example, Propylene Glycol, Type I or Ethylene Glycol, Type IV). (DIM ) Have tanks been examined annually for corrosion and/or contamination? If corrosion or contamination was evident, have the tanks been properly repaired or replaced? (DIM ) Have fluids been checked to ensure that no degradation/contamination has taken place? (DIM ; ) Are all fill ports and discharge points conspicuously labeled to prevent contamination due to inadvertent product mixing and capped when not in use (storage vessels & containers)? (DIM ) Has the deicing/anti-icing equipment been checked for adequacy in accordance with the manufacturer's recommendations? (Leaks, safety defects, serviceability, general condition, etc.)? (DIM ) IV. Procedures Y N O I N/A Ensure the pilot-in-command is notified prior to any deicing. Ensure deicing is completed whenever requested by the pilot-in-command. For deicing/anti-icing carried out without the flight crew present, a qualified individual shall confirm the treatment applied and follow procedures for providing the Pilot Notification of Aircraft Deicing/Anti-icing Form. ( DIM and 7-1-9) Ensure that the Type I fluid is mixed based on weather conditions (maintaining the minimum required 18ºF buffer) and is applied at 140º-200º (DIM 3-1-4; 3-1-5) Ensure fluid is applied to all critical surfaces, and enough fluid is applied to wet the entire surface, then letting the snow, ice or frost melt. (DIM 3-1-4; 4-1-6) Are agents using as much fluid as needed for each deicing/anti-icing operation keeping in mind that the critical surfaces must be completely free of snow, ice, and frost?. (DIM through ) Ensure Type IV is used at 100% concentrate and are applied equally to both sides of the aircraft in a uniform and symmetrical manner. (DIM ; ) Prior to towing the deicer, driving the deicing truck, or deicing an aircraft, is a safety briefing with the deicing team members taking place to discuss the policies and procedures outlined in this manual for the particular aircraft being deiced as well as specific local procedures and is an operational test of boom to truck headset communications being conducted prior to each use? (DIM 4-1-3) When brooming aircraft prior to fluid application, ensure the agent is using a soft bristle broom labeled Deice Use Only, never strikes the aircraft to remove ice and extreme caution is used not to G. damage pitot tubes, antennas and static wicks and conducted only by qualified personnel. (DIM ) H. Ensure that proper deice sequence is being followed. (DIM through ) Ensure all critical areas are properly sprayed (elevator, leading edge, all wing areas, fuselage, propellers and spinners when not running, landing gear and gear doors and aileron area). On DH-8 aircraft, ensure the spring tabs, located on the tail section, are completely free of any contamination. I. The deicing agent must see the movement of these parts when sprayed directly with deicing fluid. If the agent does not see any fluttering type movement in both spring tabs while spraying, the crew must be notified immediately. ( DIM through ; 6-5-2) October 17, 2015 FORMS

Procedures (Continued) Y N O I N/A Ensure all caution areas (engine air intakes, static ports, pitot tubes, main landing gear proximity switches, fresh air vents, air conditioning inlets and engine/apu exhaust area) can be identified by the deicing crew and are not directly sprayed. (DIM ) K. Are all deicing records completed and retained as required? Instructions in manual (Chapter 5) In the event preliminary pre-deicing/anti-icing radio communication is not possible determine if the Crew is on board the aircraft. If the Crew is on board, proceed as indicated with normal L. communications. If the Crew is not on board, complete the required Pilot Notification of Aircraft Deicing/Anti-icing. (DIM ) M. Ensure the ground crew is communicating fluid type, mixture ratio for Type IV fluid only, start time of the final deice/anti-ice application, current time and verification that the post application check has been completed, and that the aircraft is clean and ready for departure. (DIM ) N. Is a tactile inspection being conducted on all applicable aircraft. (DIM ) October 17, 2015 FORMS
