2 **** The Form I-765 is used to apply for Employment Authorization from the United States Citizenship and Immigration Services (USCIS). If your petition for employment is approved, you will receive an Employment Authorization Document (EAD) which is a document that shows you are authorized to accept employment in the United States. You must be present in the United States to apply for the EAD. You are only permitted to work after you receive your EAD. Your start and end dates are indicated on the document. To avoid a denial of your application, ensure you use the most current version of the Form I-765 found only on the USCIS web site Please notice that the current form available on the USCIS website may have in the upper righthand corner an expiration date that has already passed however, if it is the most recent form available on the website, then it is the correct form you should continue to use until it is updated by USCIS. We highly suggest that you include a cover letter to USCIS to indicate that you have used the most recent form I-765 currently on the USCIS website despite the expiration date listed on the form. You do not need to have a Social Security Number or a job to apply for 12 Month Post- Completion Optional Practical Training Employment Authorization with USCIS. Please Notice that you may now simultaneously request for an SSN on the I-765 while applying for the EAD. Do not forget to endorse your I-765!!! Not signing invalidates your document and USCIS will return your application packet to you.

3 Start Here ***I am applying for Permission to accept employment. ***If you have lost your EAD and are reapplying within your authorized USCIS dates, check Replacement. Question 1. Full Name- Always type Family Name (Surname) in all CAPS. Use both upper and lower cases for your first and middle name. If your name has more characters that can fit into the provided space, use black ink and put in your name legibly. Question 2. Other Names used. Only complete this part if you have used other names. For example if you have changed your name after marriage Question 3. US Mailing Address should be where you want your EAD mailed to. Please notice that you will receive your I-797C Notice of Action and possibly an RFE at this address therefore, it needs to be valid during the USCIS processing time which is usually 90 days. If you will be moving, make sure to report a change of address on the USCIS website. Please do not use the Office of Graduate and International Student Services as your address. Question 4. This should be the Country of Citizenship as indicated on your passport. Question 5. Place of Birth- In this order- Town/City-State-Country. Question 6 Date of Birth should be in the correct order- Month/ Day/Year. Question 7 Check the Gender that applies to you.

4 Question 8 Check your marital status. Question 9.a. If you have ever been issued a Social Security Card check YES, if not check NO Question 9.b. If you checked YES to 9.a. provide your correct SSN. A Social Security Number consists of 9 digits. Question 10. Check YES if you have never applied for an SSN then proceed to answer question 11. Check No if you have applied for an SSN in the past and skip question(s) 11, 12.a, 12.b., 13.a., 13.b. and proceed to answer question 14. Question 11. If you checked YES to question 10, then you MUST check YES. Question 12.a. If you answered YES to questions 10 and 11 then you MUST answer questions 12 and 13. Enter your Father s Family Name (Last Name) in all CAPS. Question 12.b. Enter your father s Given Name (First Name). Question 13.a. Enter your Mother s Family Name (Last Name) in all CAPS. Question 13. B. Enter your Mother s Given name (First Name).

5 Question 14. I-94 Number consists of 11 digits. It is found on the Form I-94 Arrival/Departure Record. Enter numbers only. You may not enter any letters or special characters. You may request a copy of your Form I-94 from the United States Customs and Border Patrol website here: If you entered the United States prior to 04/30/2013 which was before the CBP automated the Form I-94 you may still be in possession of a hard copy of the I-94. In this case, your I-94 number can be found on the left side of the document. Please notice that it is very important that you keep the card safe as it verifies your immigration status/ legal entry into the United States. However, if it is lost, you may file a Form I-102 to request for a replacement of the document here: Question 15. Check YES only if you have previously applied for Employment with USCIS. If your petition was granted, enter the USCIS processing office, the Start and end Dates on the expired EAD, and include a copy of the card. If it is lost, indicate that it is lost. If your petition was denied, include a copy/documentation of your denial notice. Check NO if you have never applied for Employment Authorization from the United States Citizenship and Immigration Services. Question 16. The Date of last entry can be found on your I-94. Month/Day/Year. Question 17. Place of Entry also known as Port of last Entry can be found on your I-94. Please notice that this can be within and also outside the US. There are currently 15 air US Preclearance Locations in 6 Countries: Dublin and Shannon in Ireland; Aruba; Freeport and Nassau in The Bahamas; Bermuda; Abu Dhabi, United Arab Emirates; and Calgary, Toronto, Edmonton, Halifax, Montreal, Ottawa, Vancouver, and Winnipeg in Canada.

6 Question 18. Indicate your Status at last entry. It is recommended you enter F-1 Student, or other appropriate status. Question 19. It is recommended you enter F-1 Student, or other appropriate status. Question 20. Type in ( c )(3)(B)-- as this is the code for Post-Completion Optional Practical Training. ( c )(3)(C)-- applies to 24-Month STEM OPT Extension. Question 21. *** Only complete this if you entered (c)(3)(c) in question 20 Enter your degree (level and major) as it appears on your I-20. Enter your employer name as it is listed in E-Verify. Notice that your employer MUST be an E- Verified Company. Enter the Employer s E-Verify Company Number ***Skip questions 22. &22.a. as they are not applicable to a student applying for 12-month Post-Completion Optional Practical Training and 24-month STEM OPT. Proceed to answer question 23.b. Question 23.(b). Have you ever been arrested or convicted of any crime. Check the appropriate box. If you checked YES, you must submit proof of any arrests or convictions, certified copies of all arrest reports and any other relevant documents. Also, make sure that you refer to Item 5., Item H. OR Item I of the I-765 filing instructions as it explains who may file form I-765 and also includes details as to providing court dispositions for your arrest and or convictions.

7 Certification ***Applicant s Signature. Make sure you sign the document!!! Not signing invalidates your I-765 and USCIS will return your packet to you!
