3 (310) (FAX) SGGQ4 2 OP 6/14/00: RECEIVED NOTIFICATION OF AIRCRAFT ACCIDENT FROM BILL MATTINGLY, SUPERVISOR AIRWORTHINESS UNIT, AT APPROX 09:50 HRS (PST). COORDINATED INVESTIGATION EFFORT WITH MS. TEALEYE C. CORNJO, NTSB INVESTIGATOR. NTSB WAS NOT PRESENT AT THE SCENE. AS1 GARY BARNARD, FAA IIC, REPRESENTED THE NTSB DURING THIS ON SITE ACCIDENT INVESTIGATION. AS1 GARY BARNARD, AND POI PAUL D'ALLURA ARRIVED AT THE AREA OF THE ACCIDENT LOCATION AT APPROX. 10:30 HRS (PST). THE ACCIDENT SCENE WAS AT THE TOP OF A MOUNTAIN RIDGE LINE, WHICH WAS INACCESSIBLE BY FOOT. THIS AREA IS VERY STEEP, MODERATE TO HEAVY VEGITATION TO INCLUDE TREES, WITH NO ROAD OR TRAIL ACCESS. THE LOS ANGELES FIRE DEPARTMENT WAS NOT ABLE TO PROVIDE TRANSPORTATION SUPPORT DUE TO FIRE-FIGHTING EFFORT REQUIREMENTS OF THAT DAY. THE ACCIDENT SCENE WAS NOT INSPECTED BY THE FAA. THE PILOT IN COMMAMD, MR. SATOSHI MORI AND THE CHECK-PILOT/CO-PILOT, MR. PIERGIORGIO GUSSO, WERE AIR LIFTED DOWN FROM THE MOUNTAIN TO THE NEAREST MAIN ROAD IN THE VACINITY OF THE BLOCK OF THE OLD ROAD (ACTUAL NAME OF THE ROAD). THE ACCIDENT SCENE WAS LOCATED APPROX 3 MILES WEST BY NORTH WEST OF THE INTERSTATE 5 AND CALIFORNIA 14 FREEWAYS INTERCHANGE IN THE SANTA CLARITA AREA OF LOS ANGELES COUNTY. THE NEAREST STREET TO THE ACCIDENT SCENE IS "THE OLD ROAD". THE ACCIDENT OCCURED APPROX 1 MILE SOUTHWEST OF THE BLOCK OF THE OLD ROAD, IN THE MOUNTAINS PARALLEL TO THE OLD ROAD AND INTERSTATE 5. MR. GUSSO WAS TRANSPORTED TO HENRY MAY0 HOSPITAL, VALENCIA, CA, FOR MEDICAL TREATMENT OF HIS ARM, AND WAS NOT AVAILABLE FOR INTERVIEWING AT THAT TIME. MR. MORI WAS AVAILABLE FOR INTERVIEW. AS1 GARY BARNARD AND POI PAUL D'ALLURA INTERVIEWED MR. MORI AND OBTAINED THE FOLLOWING STATEMENT INFORMATION : HE & INSTRUCTOR GUSSO WENT FOR A CHECK OUT IN THE HUGHES 300 HELICOPTER. CMNT CODE: GI991 MORI WAS BEING CHECKED-OUT FOR AN INSTRUCTOR POSIION WITH KING AVIATION. THEY WERE IN THE VALENCIA/NEWHALL AREA FOR PRACTICE AND TO LOOK FOR THE CABIN DOOR THAT FELL OFF OF THE HELICOPTER THE DAY BEFORE DURING A TRAINING SESSION WITH MR. GUSSO AND A STUDENT. MR. GUSSO INSTRUCTED MR. MORI TO LAND ON THE TOP OF A RIDGE LINE, WHERE MR. GUSSO BELIEVED THAT DOOR WAS LOCATED. MR. MORI SET UP FOR A LANDING ON THE HILL, INTO THE WIND. AS MR. MORI GOT CLOSE TO THE GROUND, HE FELT THE HELICOPTER LOSE LIFT AND HE DETERMINED THAT WIND HAD SHIFTED TO A TAIL WIND. AS MR. MORI ADDED POWER AND DECIDED TO FLY FORWARD AND TO THE LEFT DOWN THE HILL TO REGAIN AIRSPEED.

4 SGGQ4 20P AS MR. MORI BEGAN TO FLY DOWN THE HILL, HE FELT RESISTANCE TO HIS INPUTS. MR. MORI DETERMINED THAT MR. GUSSO WAS LOWERING THE COLECTIVE TO LAND. MR. MORI SAID THAT THE MR. GUSSO DID NOT TELL HIM HE WAS TAKING CONTROL. MR. MORI WAS TAKEN BY SURPRISE WITH THE CONTROL MOVEMENT(S). THE AIRCRAFT HIT THE GROUND WITH THE NOSE LOW, AIRCRAFT FACING DOWN HILL, AND THE AIRCRAFT SLID INTO SOME BRUSH AND THEN NOSED OVER INTO A LARGE BRUSH BUSH (SMALL TREE SIZE). MR. GUSSO'S HAND WAS INJURED BY THE SKID GEAR DURING THE CRASH. MR. MORI WAS NOT INJURED. BOTH PILOTS FREED THEMSELVES FROM THE AIRCRAFT AND PROCEDED TO WALK DOWN THE MOUNTAIN TO THE ROADWAY. THE LOS ANGELES COUNTY FIRE DEPARTMENT HELICOPTER ARRIVED AND TRANSPORTED BOTH PILOTS DOWN TO THE ROADWAY. MR. GUSSO WAS TRANSPORTED TO HENRY MAY0 HOSPITAL IN VALENCIA BY THE LOS ANGELES COUNTY SHERIFF'S DEPARTMENT. SHORTLY AFTER THE FAA ARRIVED. MR. MORI PRESENTED HIS PILOT'S CERTIFICATE AND MEDICAL AT THE INTERVIEW. ISIS RECORDS SHOW HE IS CURRENT. UPON COMPLETION OF INTERVIEWING MR. MORI, WE WENT TO THE KING AVIATION FACILITY AT VAN NUYS AIRPORT, VAN NUYS, CA AND INSPECTED AND PHOTOCOPIED THE MAINTENANCE RECORDS FOR THE AIRCRAFT DURING THE PAST 2 YEARS. NO DISCREPANCIES WERE NOTED IN THE MAINTENANCE RECORDS. NON-ROUTINE MAINTENANCE ENTRIES ARE NOT ENTERED BETWEEN INSPECTIONS. RELEASED AIRCRAFT TO MR. MICHAEL B. KELLEY, AVIATION INVESTIGATOR, FOR REMOVAL FROM THE ACCIENT SCENE. MR. GUSSO PROVIDED A WRITTEN STATEMENT WHICH WAS RECEIVED BY THE VAN NUYS FSDO ON JUN 16, MR. GUSSO'S WRITTEN STATEMENT IS BASICALLY THE SAME AS MR. MORI'S STATEMENT. 6/21/00: MR. GUSSO CAME INTO THE VAN NUYS FSDO FOR AN INTERVIEW OF THE ACCIDENT WITH AS1 GARY BARNARD, AND POI PAUL D'ALLURA. MR. GUSSO STATED THAT HE DID TELL MR. MORI "RPM" TO VERIFY TO MR. MORI THAT THE RPM WAS DECAYING, AND TO DO SOMETHING ABOUT IT. HE ALSO STATED THAT HE VERBALIZED HIS TAKING THE CONTROLS FROM MR. MORI (MR. MORI SAID THAT MR. GUSSO SAID NOTHING). MR. GUSSO INFORMED US THAT MR. MORI IS NO LONGER EMPLOYED BY KING AVIATION CENTER, VAN NUYS, CA. MR. GUSSO PRESENTED HIS PILOT'S AND MEDICAL CERTIFICATES DURING THIS INTERVIEW. ALSO, HE PRESENTED A SIGNED STATEMENT TITLED: "PILOT STATEMENT", DATED JUNE 14, HIS PILOT'S CERTIFICATE IS CURRENT IN ACCORDANCE WITH ISIS. A PHOTO COPY WAS TAKEN FOR THIS ACCIDENT INVESTIGATION REPORT. THIS ACCIDENT INVESTIGATION REPORT PACKAGE WILL BE TURNED OVER TO MR.
