18 Accident Investigation ASIs from the Louisville FSDO respond to all aviation accidents within our district. Depending on the scope of the accident there may also be investigators from: The FAA Office of Accident Investigation and Prevention. The National Transportation Safety Board. 19 FSDO Role in Accident Investigation ASIs gather, record, and evaluate the facts, conditions, and circumstances leading up to an accident. This data is used by the FAA to determine the actions required to prevent similar accidents. This data is used to determine the adequacy of the FAA s areas of responsibility. This data is shared with the NTSB. The NTSB is responsible for the determination of the accident s probable cause. September 5, 2013 Kentucky Aviation Conference
20 FAA Areas of Responsibility The FAA s nine responsibilities in accident investigations are to determine whether: (1) Performance of FAA facilities or functions was a factor. (2) Performance of non-faa owned and operated air traffic control (ATC) facilities or a navigational aid was a factor. (3) Airworthiness of FAA-certificated aircraft was a factor. (4) Competency of FAA-certificated airmen, air agencies, commercial operators, or air carriers was involved. (5) Regulations were adequate. (6) Airport certification safety standards or operations were involved. (7) Airport security standards or operations were involved. (8) Airman medical qualifications were involved.
