Initial climb - Miscellaneous/other On August 2, 2016, about 1322 Pacific daylight time, an Arion Lightning LS-1, N341AL, was substantially damaged after it collided with a building during a touch and go at Van Nuys Airport (VNY), Van Nuys, California. The private pilot was fatally injured. The personal flight was operated under the provisions of Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed and no flight plan was filed for the local flight that departed Santa Monica Municipal Airport (SMO), Santa Monica, California at According to an acquaintance, the pilot had planned to complete several touch and go maneuvers at VNY on the day of the accident. Preliminary Federal Aviation Administration (FAA) air traffic control audio indicated that the pilot requested to land on runway 16L. After a touch and go, he reported that he heard something "banging around" and asked the tower controller if he could see it. The controller informed the pilot that his left main landing gear was "dangling" from the airplane. After some further discussion, the pilot stated that he wanted to land at VNY, which was his final radio communication. Several witnesses reported that the airplane was flying erratically during its departure from runway 16L. Two witnesses listened to the exchange between the pilot and the controller after they observed a wheel fairing hanging from the left main landing gear of the departing airplane. Seconds after the pilot's final communication, they observed the airplane enter a hard left turn. One witness described the turn as a "fighter pilot maneuver." The airplane's nose dropped through the horizon during the steep turn, which was immediately followed by a quarter spin to the left and then a nose down attitude. According to the FAA, the accident site features indicated that the airplane impacted a utility pole, a truck, and then slid on the ground into a section of alleyway between two buildings where it came to rest. The airplane came to rest about one eighth of a nautical mile from VNY. The initial impact point (IIP) was identified by a broken wooden utility pole and multiple severed wires. The end of the debris path was marked by the main wreckage, which came to rest in fragments about 15 feet beyond the IIP. An odor of fuel was detected at the accident site, which was accompanied by a large fuel stain on the ground beneath the main wreckage. One wooden propeller blade remained attached to the propeller hub and the other blade was sheared at the propeller root. A wreckage examination will take place at a later date. Landing-landing roll - Landing gear collapse On July 1, 2016, about 2206 central daylight time, a Czech Aircraft Works Sportcruiser, N236SC, sustained substantial damage during an off airport landing on Interstate Highway 35 (I-35) near Wyoming, Minnesota. The sport pilot, the sole occupant, received minor injuries. The airplane was owned and operated by the pilot under the provisions of the Code of Federal Regulations Part 91 as a personal flight. Night visual meteorological conditions prevailed at the time of the flight, which was not on a flight plan. The flight departed the Starbuck Municipal Airport (D32), Starbuck, Minnesota, sometime between 2000 and 2100 and was en route to the Cambridge Municipal Airport (CBG), Cambridge, Minnesota. The pilot stated to a Federal Aviation Administration inspector that he could not find CBG because it was dark. The airplane was low on fuel and he could not see an airport beacon, so he landed the airplane on a road, which turned out to be I-35. During the landing roll, he attempted to maneuver the airplane into the ditch between the northbound and southbound lanes of the highway to avoid the traffic on the highway. The airplane's right wing hit the post of a wire road divider causing substantial damage to the airplane. The pilot stated that the airplane did not have any mechanical failures while airborne. A postaccident examination of the airframe and engine revealed no evidence of mechanical malfunctions or failures that would have precluded normal operation. Page 2 Copyright 1999, 2016,

Maneuvering - Loss of engine power (partial) On August 5, 2016, about 0745 eastern daylight time, an Liming Louis L HPL-1 experimental airplane, N9095U, registered to the pilot/owner, sustained substantial damage during a forced landing after a partial loss of engine power near Owensville, Ohio. The private pilot, who was the sole occupant, was not injured. The personal local flight was being conducted under the provisions of Federal Code of Regulations Part 91. Visual meteorological conditions prevailed throughout the area and a flight plan was not filed. The flight originated at 0730 from the Brown County Airport (GEO), Georgetown, Ohio. Prior to departure from GEO, the pilot reported that he fueled the aircraft with M94 fuel. After takeoff, the pilot flew to the north for about 9 miles and began circling a private residence at 1,500 feet AGL. Suddenly, the engine power went to idle. The pilot checked to make sure that he had not bumped the idle control with his left knee and attempted to increase the engine RPM. The engine RPM did not increase and the pilot chose a grassy field to land while the engine remained at idle. Upon initial touchdown in the rough vegetated field, the airplane's speed was relatively high. The airplane became airborne and the the left wing struck a small stand of trees, spun around about 360 degrees, and struck the ground before coming to a stop. Substantial damage was incurred to the left wing and right wing strut and the pilot exited without injury. Fuel samples were taken from the GEO source and no contaminates were found. Initial climb - Loss of control in flight On August 6, 2016, about 1533 central daylight time, an experimental PZL Bielsko SZD 55-1, a single-seat glider, sustained substantial damage when it impacted terrain during takeoff from the Chicago Glider Club Gliderport (IL59), Channahon, Illinois. The pilot was fatally injured. The glider was owned and operated by the pilot under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed for the flight, which was not on a flight plan. The glider departed from IL59 about 1532 on a local flight. The pilot of the tow airplane reported that he initiated the takeoff from runway 27 (2,000 ft by 250 ft, grass/turf) and that the takeoff was normal. The tow airplane lifted off about halfway down the runway and began climbing. He stated that when the tow airplane was about 20 ft above ground level (agl), he began to feel a "heavy increasing drag from the glider and shortly thereafter, felt the glider release from the tow." Witnesses reported that during the takeoff roll, the glider's right wing dropped and hit the ground after the "wing runner" let go of the right wing. The wings leveled momentarily and then the left wing hit the ground while the glider was veering slightly to the left. The witnesses stated that the pilot leveled the wings briefly before the glider pitched up approximately 30 to 40 degrees. The tow airplane was still on its takeoff roll as the glider pitched up. The glider continued to climb at a 30 to 40-degree pitch attitude until the tow rope was released from the glider, which was on a southwest heading. The glider's pitch attitude leveled out, and it appeared that the glider started a right turn, but then the glider entered a stall/spin to the left. The witnesses stated that the glider had reached 100 to 200 ft agl when it entered the stall/spin. The glider impacted a field with tall grass in a left wing low, steep nose-down attitude about 200 ft south and 1,600 ft from the approach end of runway 27. The entire aircraft was located at the point of impact. The entire span of the left wing remained attached to the fuselage and it exhibited crushing and impact damage along the leading edge of the outboard section of the wing. The nose and cockpit remained attached to the fuselage; however, it was crushed and broken, and displaced to the right. The outboard section of the right wing was separated from the rest of the wing at the aileron bellcrank and was lying forward of the right wing in the direction of travel. The tail was broken aft of the fuselage and the tail boom and empennage were displaced to the right of the fuselage. The empennage remained intact and exhibited no damage. The wing and horizontal stabilizer attach points were attached properly and were secure. The flight controls, including the spoilers, were checked for continuity from the flight controls to their respective surfaces. Flight control cables and control tubes were traced and all breaks were consistent with overload. No preimpact flight control continuity anomalies were detected. There was no water found in the ballast tanks. The chin tow cable release was found in the spring-loaded closed position. The chin release lever was operated by hand and it moved to the tow release position. The surface weather observation at the Joliet Regional Airport (JOT), Joliet, Illinois, located 6 miles northeast of the accident site, was wind 330 degrees at 6 kts, 10 miles visibility, sky clear, temperature 29 degrees C, dew point 16 degrees C, altimeter inches of mercury. Landing-landing roll - Loss of control on ground The pilot of the tailwheel equipped airplane reported that during the landing roll in gusty wind conditions, the airplane veered off the runway to the left, impacted an irrigation ditch, and nosed over. The airplane sustained substantial damage to the left lift strut. According to the pilot there were no preimpact mechanical failures or malfunctions with the airframe or engine that would have precluded normal operation. The private rated pilot and flight instructor were both seriously injured. The airplane was substantially damaged. The airplane was registered to and operated by Sonex Ltd under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed for the flight, which operated without a flight plan. According to preliminary information obtained by investigators, the pilots were performing touch-and-go landings at KFLD. Shortly after takeoff, the pilots reported a loss of engine power. During the forced landing, the airplane collided with a transmission line and impacted terrain. An inspection of the airplane revealed substantial damage to the fuselage and both wings. Takeoff-rejected takeoff - Loss of control on ground On July 10, 2016, at 1645 central daylight time, a Buckeye Dream Machine powered parachute, N3519T, was substantially damaged during an aborted takeoff at Ohio County Airport (JQD), Hartford, Kentucky. The student pilot received serious injuries. The powered parachute was registered to a private individual and operated by the pilot as a 14 Code of Federal Regulations Part 91 personal flight. Visual meteorological conditions prevailed and no flight plan had been filed for the flight. The flight was originating at the time of the accident. According to the pilot, he prepared for takeoff from a parking lot at JQD. He started the engine and began the takeoff roll. During the takeoff roll the engine started to "sputter" and the pilot decided to abort the takeoff. While attempting to abort the takeoff, he collided into a chain link fence. Initial examination of the aircraft by a Federal Aviation Administration inspector revealed that the left frame support tube was broken from the main tube structure. The pilot, the sole occupant, was fatally injured. The airplane was registered to and operated by the pilot under the provisions of Title 14 Code of Federal Regulations Part 91. Visual meteorological conditions prevailed for the flight, and no flight plan was filed. The local flight originated from DLS at about Moments before the impact, a witness observed the airplane in a nose-low attitude with the right wing pointed up and the left wing pointed directly towards the ground. A second later, the airplane impacted the terrain. A witness and another person tended to the pilot where they waited for the emergency medical services. The pilot was then transported to the hospital where he succumbed to his injuries approximately an hour and a half later. The airplane was recovered to a secure location for further examination. Takeoff - Loss of control in flight On August 6, 2016, about 0820 central daylight time, an experimental amateur-built Wilson Bugatti-DeMonge 100P airplane, N110PX, impacted terrain during takeoff from runway 35L at the Clinton-Sherman Airport (CSM), near Burns Flat, Oklahoma. A subsequent ground fire occurred. The airline transport rated pilot was fatally injured. The airplane was destroyed during the impact and ground fire. The airplane was registered to Le Reve Bleu LLC and operated by the pilot as a 14 Code of Federal Regulations 91 test flight. Day visual meteorological conditions prevailed in the area about the time of the accident, and the flight was not operated on a flight plan. The local flight was originating from CSM at the time of the accident. Witness at the airport reported that the airplane lifted off. During the climbout, the airplane banked to the right and then to the left. The airplane's left bank steepened, it descended nose down, and subsequently impacted terrain inverted. The 66 year-old pilot held a Federal Aviation Administration (FAA) airline transport pilot certificate with multi engine land, single engine land, and instrument airplane ratings. He held a FAA Second Class Medical Certificate issued on May 12, This medical certificate was issued limitations: Must wear corrective lenses. Not valid for any class after 05/31/2017. The pilot reported on the application for that medical certificate that he had accumulated 10,700 hours of total flight time and 25 hours in the six months prior to the medical examination. N110PX was an experimental amateur-built, twin engine, single seat, tailwheel airplane built as a replica of the Bugatti-De Monge 100P airplane. According to airworthiness documents, the airplane was constructed to duplicate the original airplane's systems, dimensions, and structure. The airplane was powered by two Suzuki reciprocating engines and they each drove a Hercules fix-pitched wooden propeller. The propellers rotated in counter directions to each other. The airplane's maximum gross weight was listed as 2,939 pounds and its empty weight was 2,470 pounds. The airplane received its Special Airworthiness Certificate in the experimental category on August 4, At 0753, the recorded weather at CSM was: Wind 040 degrees at 9 knots; visibility 10 statute miles; sky condition clear; temperature 23 degrees C; dew point 21 degrees C; altimeter inches of mercury. CSM was a public, towered airport, which was owned by the Oklahoma Space Industry Development Authority/State of Oklahoma. It was located about two miles west of Burns Flat, Oklahoma. The airport had an estimated elevation of 1,922.1 feet above mean sea level. Two runways, 17R/35L and 17L/35R serviced the airport. Runway 17R/35L was a 13,503 feet by 75 feet runway with a concrete surface. Runway 17L/35R was a 5,193 feet by 75 feet runway with a concrete surface. Airport operations personnel examined the runway after the accident and no liberated airplane parts were found. The airplane wreckage was found about 415 feet and 75 degrees from of the intersection of E1140 Road and N2120 Road and about 1,900 feet and 335 degrees from the departure threshold of runway 35L. The airplane came to rest inverted about a 330-degree heading. A depression was observed in the ground about 110 degrees and 23 feet from the wreckage. Sections of clear plastic were found in the depression and the surface of sections of the depression contained a blue color transfer consistent with the color of the airplane. The airplane, forward of its empennage, was discolored, deformed, and charred, with sections consumed by fire. The rudder's skin was consumed by fire. The lower section of the right main landing gear separated from its strut and the lower section was found resting on vegetation northwest of the wreckage. Wooden propeller blades were found splintered in sections resting on the ground in the area around the wreckage. An on-scene examination of the wreckage was conducted. The rudders control cables were traced from the rudder to the rudder pedals. The elevators' push/pull tubes were attached to the control arms for each elevator. Forward of the empennage, the tubes were found to be consumed by fire. The aileron's control tubes were found connected to each aileron. These tubes were found to be consumed by fire outboard of the fuselage. Control continuity for the elevators and ailerons could not be established due to the fire damage. The propulsion drivetrain was traced and no preimpact anomalies were detected. The engines sustained thermal damage and could not be rotated by hand. However, no indications of engine anomalies were observed. The localized area where Page 14 Copyright 1999, 2016,

15 the airplane came to rest exhibited discoloration and charred vegetation consistent with a ground fire. The coroner was asked to perform an autopsy on the pilot and take toxicological samples. The airplane was fitted with cameras for the flight. Some of these cameras were found in the area of the wreckage. The recovered cameras were retained and are being sent to the National Transportation Safety Board Recorder Laboratory to see if they contain video data in reference to the accident flight. The student pilot and passenger received serious injuries.
