3 Awards Page 1 of Air Safety Award Winner Captain Ray Brice (United) with his family and President Duane Woerth The Air Line Pilots Association, International, (ALPA) has awarded its 2000 Air Safety Award to United Airlines Capt. Raymond Brice. The award, ALPA s highest honor for a volunteer in the air safety field, was presented on August 16 during the Association s annual Air Safety Forum. "Capt. Brice is uniquely deserving of this award the greatest honor ALPA can bestow on an air safety volunteer," said Capt. Duane Woerth, ALPA president. "He has proven time and time again his dedication to ALPA's United Airlines pilot group and our union, as a whole, and has made a real difference in improving the safety of commercial air travel for flight crews and their passengers. Capt. Brice is a consummate air safety volunteer and activist. " Capt. Brice, who flies the B , has been an employee of United Airlines since He has volunteered in ALPA's air safety structure since Capt. Brice currently serves as the United pilots' central air safety chairman, a post he has held since He previously was the United MEC's central air safety vice chairman, and was the United pilots' local (SFO) air safety chairman for seven years. Capt. Brice has led, coordinated and overseen numerous safety improvement efforts at the MEC, ALPA national, and international levels. During Capt. Brice's many years of service to his union, he has been involved in ALPA air safety initiatives involving aircraft maintenance, training, air traffic, certification, wildlife hazards, airport surface incidents (incursions), and Flight Operations Quality Assurance (FOQA). Capt. Brice also has been an active participant in several accident investigations. Capt. Brice attended the College of San Mateo. Prior to his career at United, he spent three years in the U.S. Army. Capt. Brice and his wife, Jan, live in San Francisco, Calif., and have two adult sons, Shawn and Nick. 4 Delta Crew Honored with ALPA s Superior Airmanship Award Page 1 of 2 News Release Release #00.66e August 16, 2000 Delta Crew Honored with ALPA s Superior Airmanship Award WASHINGTON, D.C. A Delta Air Lines crew that landed its aircraft safely despite an inflight fire with multiple system failures was honored with a Superior Airmanship Award by the Air Line Pilots Association, International (ALPA). Association president, Capt. Duane Woerth, presented plaques to each crewmember on August 16 during ALPA s annual Air Safety Forum. In November 1998, Capt. Douglas Kasemeier of Lilburn, Ga., First Officer Marty Cavato of Oviedo, Fla., and Second Officer Henry Turner III of Landrum, S.C., were the crew of Delta Flight 225, from Honolulu to San Francisco. After an uneventful take-off, a fire erupted in the flight deck behind one of the circuit breakers. Second Officer Turner quickly put out the fire using a fire extinguisher. After assessing the situation, Capt. Kasemeier determined the aircraft had lost control of cabin pressure, which was reestablished using a back-up system. At the same time, Second Officer Turner noticed another small fire emanating from the same area as the first; it too was extinguished. The flight deck crew then determined that the aircraft's ground/air sensing system was damaged by the fire meaning that some systems thought the aircraft was in the air, and others thought on the ground. The crew surmised that upon landing they would have no reverse thrust, no spoilers, and brake problems. The plight of Flight 225 continued as the aircraft approached San Francisco Airport when it began to experience flight control problems. By continually resetting the flight control channels, the flight deck crew was able to make a successful emergency landing. However, the challenges of this particular flight were not over yet upon landing, they could not open the aircraft doors so that the passengers could exit. After investigating a variety of means to open the doors, they found they were able to open the doors by shutting down the engines and pulling the fire handles, allowing the aircraft to depressurize. Without the superior airmanship, teamwork, and flight deck management of Capt. Kasemeier and his crew, an uneventful and successful conclusion to a very

6 TWA Crew Honored with ALPA s Superior Airmanship Award Page 1 of 1 News Release Release #00.66f August 16, 2000 TWA Crew Honored with ALPA s Superior Airmanship Award WASHINGTON, D.C. A TWA crew that landed its aircraft safely despite suffering an uncontained engine failure at take-off was honored with a Superior Airmanship Award by the Air Line Pilots Association, International (ALPA). Association president, Capt. Duane Woerth, presented plaques to each crewmember on August 16 during ALPA s annual Air Safety Forum. On July 27, 1999, Capt. Robert Laux of Cherry Hill, N.J., First Officer Eric Drennen (now a B727 captain) of Chicora, Pa., and Relief Officer Kurt Anderson (now an MD-80 captain) of Ann Arbor, Mich., were the crew of TWA Flight 885, from Tel Aviv's Ben Gurion International Airport and New York's John F. Kennedy International Airport. At take-off, the flight deck crew noticed the signs of a massive engine failure. After assessing the situation, Capt. Laux with the concurrence of the control tower elected to return immediately to Ben Gurion. During the landing, the aircraft struck several birds, with at least one of them striking the plane's radome. Emergency equipment was waiting at Ben Gurion to monitor the aircraft, cool the brakes, and remove the passengers. The 239 passengers and 12 crew onboard deplaned safely no one was injured. The entire flight lasted only nine minutes. Subsequent inspection of the engine revealed that a yellow-legged gull entered the engine and was ejected out of the back of the fan and through the lower engine casing. The encounter with the gull also caused two fan blades to tear loose and travel through the engine the inlet spinner and engine also were damaged. Because Capt. Laux and his crew performed flawlessly and as a team, a successful end to a potentially disastrous situation occurred. 7 Florida Pilot Honored With ALPA Safety Volunteer Award Page 1 of 2 News Release Release #00.66b August 16, 2000 Florida Pilot Honored With ALPA Safety Volunteer Award WASHINGTON, D.C. Jack Harris, a TWA captain and resident of Jax, Fla., was awarded a Presidential Citation for Outstanding Service in Air Safety from the Air Line Pilots Association, International (ALPA). The award was presented on August 16 during the Association s annual Air Safety Forum. ALPA honored Capt. Harris for his heroic actions to avert disaster during an August 1999 TWA flight from San Jose, Calif., to St. Louis, Mo.. The first officer on Flight 326, Bill Tiedge, also received a Presidential Citation. After the MD-83 pushed back from the gate at San Jose Airport, the crew experienced a violent explosion. The aircraft continued to take off, but once it was airborne, the flight crew could smell burning rubber. They also noticed that the left "gear unsafe" light was illuminated and there was a failure of the right hydraulic system. Once Flight 326 was safely airborne, Capt. Harris requested a safe area where the aircraft could hold so the flight crew could determine its condition. They attempted to check the landing gear, but their view was blocked. After completing emergency checklists, Capt. Harris spent three hours briefing the company, flight attendants, and passengers, while the aircraft burned fuel. San Francisco International Airport was selected as the diversion airport, and Capt. Harris received a visual check of the landing gear before attempting to land the plane. Although the control tower staff noted that both left main tires were completely destroyed, the landing and rollout were uneventful, and all 142 passengers and crew deplaned safely. Postflight inspection revealed that one of the tires failed during takeoff while the tire was coming apart, pieces of it caused the other tire to disintegrate. Capt. Harris currently flies the B767, and is based out of John F. Kennedy International Airport in New York. He has been with TWA since Capt. Harris began his aviation career with the U.S. Navy in 1963, and retired as a captain with the U.S. Navy Reserve in He received both his bachelor's degree in math education and his juris doctor from the University of Florida. In addition to being a consummate airman, Capt. Harris was a Florida assistant state attorney in Jax from , and has maintained a private legal practice since 1980.

11 California Pilot Honored With ALPA Safety Volunteer Award Page 1 of 1 News Release Release #00.66d August 16, 2000 California Pilot Honored With ALPA Safety Volunteer Award WASHINGTON, D.C. York Hird, a United Airlines captain and resident of Huntington Beach, Calif., was awarded a Presidential Citation for Outstanding Service in Air Safety from the Air Line Pilots Association, International (ALPA). The award was presented on August 16 during the Association s annual Air Safety Forum. ALPA honored Capt. Hird for his heroic actions to avert disaster during a November 1999 flight from Los Angeles International Airport. He was acting as the first officer on the flight; the captain, Jeff Sakuda, also received a Presidential Citation. After the B757, with 130 passengers onboard, received takeoff clearance from the LAX tower, Capt. Sakuda initiated the takeoff roll. At that time, First Officer Hird observed another large aircraft taxiing onto Flight 204's runway, and immediately informed the captain of the situation. Capt. Sakuda observed the other aircraft, evaluated the situation, and made the decision to continue the takeoff. He initiated rotation earlier than normal, and at a higher rate, to avoid a collision on the runway. The actions of the crew of Flight 326 resulted in their aircraft safely clearing the top of the other large aircraft by approximately feet; no one was injured, the aircraft was not damaged, and the flight proceeded normally. Capt. Hird currently flies the B737/500, and is based in San Francisco. He was hired by United Airlines in 1995, and was promoted to captain in February He previously was a captain on the Shorts-360 for American Eagle, based in San Juan, PR. Capt.
