Presented by ASD Oregon Thank you for registering your child for Camp Quest. Listed below you will find more information regarding the camp session, what to bring, how & when to check-in, health forms, and other important information. If you have any questions or concerns, please feel free to contact Misti at 503-320-3103. Prior to Camp Arrival: The HEALTH FORMS must be received at Camp Quest two weeks prior to the camp start date. The Medication Sheets should be filled out and brought with you on the day of registration. These forms are required for attendance at Camp Quest. Complete the medication form, copy insurance card, sign the form. Check In: Check in starts on Sunday at 4:00 PM. Check-in occurs at Camp Tapawingo. Please do not arrive before 4:00 to allow us time to be fully ready for your camper. If you will be arriving late or other complications arise, please notify the camp. Check In Involves: 1. Final payment of any outstanding camp fee balances. 2. Cabin assignments. 3. Health screening & head lice check. 4. Drop off any medications to Medication Manager. 5. After you have gone through the check-in process, you and your child may move into your cabin. 6. Dinner will begin at 5:30 (BBQ) and families are welcome to stay through dinner. Medication and Check In Policy Our medical staff will only dispense medication, including herbal remedies or vitamins, according to a physician's written instructions. You must present medications in their original prescription packaging at camp check-in. If the current dosage differs from prescription label, you must present the physician's written instruction to alter the dosage on the label. All campers who will be taking medication or supplements must bring the medication form completely filled out with the medication in original bottles, with original labels in a large Ziploc bag. Check Out: Check-out time is Thursday at 10:00 AM. First Aid: The camp medication manager conducts daily sick calls and dispenses all medication. All medications must be deposited with the medication manager at check-in. Parents will be notified if a camper has an illness or injury that requires more than first aid care. Parents will also be contacted if questions regarding a health issue arise.

Behavior Management Camp programs will be planned and operated to employ best practices in working with children with autism spectrum disorders. To meet individual needs, the Camp Director(s) will adjust staff ratios and camp sessions, use proactive strategies, use individual communication systems, use sensory accommodations, and modify program and environment whenever possible. Parent/guardian must indicate in writing the proactive strategies used to prevent problems with maladaptive behaviors. This should include environmental strategies, refocusing strategies, deescalation strategies and any other interactive strategies that work. Camp Quest reserves the right to deny camp attendance to any camper whose principal behavior management plan relies on the use of physical restraints or locked time-outs. No persons employed at camp will use physical restraints or locked time-out as part of behavior management strategies. Any aggressive or self-injurious behaviors that have been exhibited by the camper in the past three months should be reported in writing. Program planning and staff assignments shall be made based upon the information furnished through the parent packets and IEP s if applicable. A phone call shall be made by camp staff at least one week prior to camp to identify up-to dateconcerns. The staff person making the call will report any concerns to the Camp Director(s). All camp staff participate in a mandatory training covering all aspects of camp including the use of proactive behavior management strategies. Extreme Behavior If, while at camp, an individual camper exhibits extreme out of control behavior, the following will occur: 1. Camp staff shall use emergency procedures to ensure the safety of the individual camper and other campers. The camp staff shall report the behaviors to the Camp Director(s). 2. The Camp Director(s) shall assemble a team meeting to determine how to adjust the camper s program, environment, or staff support. 3. A written plan shall be prepared describing the problem, suggested strategies, and results of implementing the strategies. 4. If the Camp Director(s) determines that (1) the plan has been implemented, (2) the individual camper s behavior poses a threat to the health, safety, and well-being of the camper and /or other campers, and (3) the camp lacks the staff resources, space, and skill to serve the camper, the Camp Director(s) will call the parent or responsible person to take the camper home. Camper Mail If you would like to mail your camper a letter or package, you can send it to Camp Tapawingo at P.O. Box 189 Falls City, Oregon 97344 Make sure to put your campers name on the outside, and please do not send food items. Phone Calls Campers may call home during the week, particularly if doing so helps alleviate camper anxiety. We encourage campers to call during the hours of 5:30-7:30PM, but we can remain flexible based on the need of the camper. There is a phone on the porch of the dining hall that our staff will answer when we are hear it. The phone number is 503-787-2806. There is also a camp phone that the Camp Tapawingo directors and staff will answer. If you need to get a hold of one of us, they will find us. Camper Packing List Please label all items with campers name It is strongly suggested that all clothing and equipment be marked with your camper s name. Lost and found articles with names attached are much easier to return to owners. Lost and found articles left at camp will be taken home by the staff and an effort will be made to return those items. Campers should bring enough clothing with them to last for the session. When packing, keep in mind that campers should dress for comfort. Camp Tapawingo Driving Directions Do not use internet directions, they are wrong! Exit 260A Hwy. 22 Salem CAMP TAPAWINGO Dallas I-5 99 W Falls City Rd. Black rock Rd. Monmouth Monmouth Hwy. Directions to Camp Tapawingo: (503)787-3828 From the North: Take I-5 south to the Salem Parkway (Exit 260A). Follow signs to Hwy 22-Dallas while traveling along Salem Parkway. Drive over the Marion Street Bridge and stay on Hwy 22 for about 11 miles. Just past the over pass is the exit for Dallas (left lane). Stay on Hwy 223 through Dallas following signs to Falls City-Kings Valley to the Fall City Junction. (At the large 3-way intersection in Dallas be sure to stay to the left as you head into the downtown area. This becomes Main Street. You will then make a right hand turn on to Washington...continue to follow the signs to Falls City. At the Falls City Road junction, turn right on to Falls City road. Travel 4.4 miles into the town of Falls City. One block after the grocery store, the road splits, stay to the right traveling up the hill (this is marked Mitchell Street). When the pavement ends and the road splits again, stay to the left. You are now on Black Rock Road. Travel 2.9 miles until you reach Socialist Valley Road. Turn right on Socialist Valley Road and proceed over the bridge and through the gate into camp.

All medications must be in their ORIGINAL containers. Place all medication containers in a Ziploc bag with this completed form detailing instructions for the use of each medication your child is to receive at camp. A medical attendant will receive medications at the nurse s table during camp check-in. Inhalers are the only medications that can be kept with the camper. (Please send two in case one is lost; one will be checked in with the nurse.) All camper medications must be administered by medical attending personnel. NO PRESCRIPTION MEDICATION CAN BE ADMINISTERED UNLESS LISTED ON THIS FORM WITH PARENT OR LEGAL GUARDIAN SIGNATURE.
