Fairlee Vacations 2018-2019 Fairlee Vacation Program Description Easterseals provides exceptional services, education, outreach and advocacy so that people with autism and other disabilities can live, learn, work and play in our communities. Participants with disabilities experience safe, healthy and success-oriented recreational opportunities. The philosophy at Camp Fairlee is to provide a nurturing environment with: 1) Choice of age appropriate activities the meet the interest and abilities of our participants; and 2)The support which enables all to participate to their fullest potential that supports independence. Our Travel Trip program provides outdoor recreation, fun and friendship in addition to seeing new places for adults with disabilities. The trips allow parents and caregivers a short interval of rest and relief from the 24-hour responsibility of direct care or service. Travel Trip activities vary by trip and location. Check-in is the evening prior to trip departure between 6:00pm and 7:00pm. Camp Fairlee cannot accept responsibility for participants before scheduled check-in time. Fairlee Vacations Program Fee The program fee is based on each Travel Trip and listed on the registration form. In order to register for our Fairlee vacations, a completed registration form, a current up-to-date health form and a $400 deposit must be received by our office at least 4 weeks prior to the trips departure date, so that arrangements can be made in a timely manner. If an organization, club, or agency will be funding the trip, a Letter of Intent for Funding must be received and on file in order to complete the registration process. A letter of intent is attached, or the form can be faxed or emailed by request to the agency/ organization who will be funding the trip. You must call camp and make this request. Be sure to have a fax number or email address for the agency/organization. If you are a new participant, you will not receive an acceptance packet until you have completed the pre-camp interview. Refund Policy: All cancellations must be made at least 14 days before the date of the trip (someone may be on the waiting list). There will be no refunds for late cancellations. Fairlee Vacations Eligibility Participants must have the ability to: 1. Be compliant with staff directions. Health Forms & Medication Info A current (within 12 months of attending program) completed health from is required at all times and must be turned in 2 weeks prior to camp date. This form must be completed, signed and dated by the participant s physician. All prescribed medications must be in their original bottle or blister pack from pharmacy with the original script from the prescribing physician. All over the counter medications must be brought to camp in their original bottles. Any altered prescription label will not be accepted. The dosage and schedule on the pharmacy label must match the information on the health form signed by the physician. Camp Fairlee staff will not accept pre-poured medication or anything that does not match with the physician s order. If any changes are made to the participant s medication (e.g. dosage, time, route, etc.) after the health form has been submitted to Camp Fairlee, a new, signed physician s order must be presented at the time of check-in. Camp Fairlee will not accept a participant at check-in if any of the procedures listed above are not followed; if a participant has an elevated temperature (100.5 F or more); and/or an untreated or unstable illness or condition. For the safety of all participants and staff, NO exceptions will be made. 2. Be ambulatory or able to transfer to a passenger van and/or plane. 3. Be able to participate in a 3 to 1 participant to counselor ratio. 4. Exhibit socially appropriate behaviors in group settings. Inappropriate behaviors, such as refusal of medication, running away/elopement, hitting, spitting, biting, kicking, hair pulling, setting fire and swearing directed at other participants and/or staff, are not acceptable. If such behavior occurs, the caregiver will be contacted. If the behavior /continues the participant will be sent home at their own expense. Camp Fairlee reserves the right to send a participant home due to behavioral or medical issues, at the participant s expense. This decision will be made by the Director. Wheelchairs must be easy to fold. Power chairs cannot be accommodated. Wheelchair space is limited to 2.
