Make sure that all forms are completed and signed. Attach a copy of your health Insurance Card to the Permission Slip. Should your child require Emergency Medical Help, the Hospital will not even administer as much as an aspirin without it. Pay special attention to the What to Bring letter. Once you have completed all of the forms and attached the Insurance Card, make a check for $175.00 per student payable to: Catholic Schools Band Camp and mail to: Catholic Schools Band Camp P O Box 858 Palatine, IL. 60067-2537 Use a #10 envelope. Do Not send Priority" or Registered or any other form of mail. Last year there was a problem due to objects too large for the box You will be notified by E-Mail of your acceptance. PLEASE NOTE: Last year we had 75 children at Camp. Of that 75 one-third of them registered during the last week of May. This made ordering supplies. Ordering music and finding enough Counselors VERY difficult. This year we have decided that we will hold all checks until May 31 and if we do not have 70 campers registered by May 31, we will destroy all checks and Registration Forms and there will be no more Band Camps. WHAT TO BRING TO CAMP MUSIC STUFF: Band instrument, folding music stand, pencils, and spring clothes pins (about 6). BEDDING: Bunks and mattresses WILL BE PROVIDED. Bring a pillow, pillowcase, sheets and blankets for a single bed OR Bring a sleeping bag and at top sheet. CLOTHING: OLD COMFORTABLE CLOTHES - - shorts, slacks, sweatshirts, etc. extra socks, TWO pair of shoes, FLIP/FLOPS may only be used in the Dorm or At the Beach. a hat, a flashlight, a jacket or sweater for cool evenings, and a swimsuit, No special clothing is required for the concert. Don t bring jewelry or Designer outfits. - THIS IS CAMP. If you must have a watch, buy a cheap one that you won t mind losing, if it happens. If you want a camera buy a disposable one and PUT YOUR NAME ON IT. PUT YOUR NAME ON EVERYTHING! MISC.: Toiletries - soap, shampoo, towels, toothbrush, toothpaste, sunburn lotion, insect repellent, and a plastic bag for dirty clothes. ALL IN PLASTIC BOTTLES WHERE NECESSARY AND WITH THE CHILD S NAME ON IT. RECREATIONAL EQUIPMENT: Some sports equipment will be provided; however you may bring whatever equipment is necessary for the games you like to play. ELECTRICAL: Hair dryers and curling irons are allowed, however, outlets are very limited. Radios, Walkman, MP 3 Players etc. are allowed at YOUR OWN RISK, but they must be battery powered and have a headset for personal use. THERE ARE NO CELL PHONES ALLOWED AT CAMP NO FANS, NO VIDEO GAMES, or DVD VIEWERS, NO EXTENSION CORDS, NO CELL PHONES MEDICATION: All medication will be checked in with and be dispensed by the Nurse. Please send all necessary instructions. List all medications to be taken as well as the times they are to be taken and place all items in a reseal able plastic bag bearing the child s name. DO NOT BRING: Skateboards, roller skate or blades, footballs, large radios, jewelry or any other valuables. There is nothing sold at camp, so there is no reason to bring money.
