Welcome On behalf of Northern Queensland Primary Health Network (NQPHN), I would like to invite all health practitioners and their teams to Mackay to attend the third annual myphn Conference. The theme of the conference for this year is Connecting our health care together, which focuses on working together to provide seamless care for patients across our landscape. It provides an opportunity to continue discussions and find solutions to ensure peoples health needs are better met across all our communities, and where patient care is the top priority, whether it be in primary or acute care. We want to create meaningful care connections to improve the patient journey, ensuring that consumers receive the right care, at the right time, and in the right place. We ll collaborate on better ways to engage across the whole health platform to ensure seamless care, including the use of My Health Record, appropriate discharge planning, and more preventive focused health care aimed at wellbeing and resilience. In a dynamic health environment, our key conference themes will focus on working together to establish a shared vision for community need, to improve health systems and technology, to build workforce capacity and future capability, and to deliver innovation, research, and reform. myphn Conference 2018 allows plenty of time for discussion during the plenary and concurrent sessions, as well as during the NQPHN Health Professional Awards 2018 evening, and the Men s Health Breakfast. We want you to be inspired to share ideas, to engage in conversation, and to help inform NQPHN about how we might improve the primary healthcare space. After sell-out events in 2016 and 2017, I know myphn Conference 2018 will deliver a program to prepare you for the future. We look forward to seeing you this September in Mackay for high quality professional skilling and an engaging tropical retreat. Mr John Gregg Chief Executive Officer Northern Queensland Primary Health Network 2 myphn 2018 Conference: Connecting our health care together

Health is complex and many of its core attributes are anchored in the social context. myphn Conference 2018 embraces this diversity and invites a cross-section of service planners and providers to participate in the exploration of connecting health. Across the weekend of 1 2 September 2018, delegates and exhibitors will have access and opportunities to engage with over 40 world-class speakers and industry professionals spanning multiple professions and services. Delegates will also enjoy keynote addresses by esteemed speakers such as former Australian Prime Minister and beyondblue Chair, the Hon Julia Gillard AC, and the Hon Ken Wyatt AM MP, Minister for Aged Care and Minister for Indigenous Health, as well as attending the prestigious third annual NQPHN Health Professional Awards.
