A practical approach to SMS implementation Colombian Civil Aviation Authority Colonel (r) Miguel Camacho Martinez Chief, Safety Management Systems Regional Aviation Safety Group Pan America (RASG-PA) Workshop Miami, May 2013 1
Objective To share with Seminar some Colombian experiences around Safety Management Systems implementation, and their positive effect on operational safety of the country 2
Agenda The particular Colombian civil aviation environment Assimilation and implementation of SMS Some remarks on SSP Accident investigation and statistical summary Conclusions 3
The country strategic position 4
The Colombian operational environment Big territorial extension 1 160.000 Km sq. Diverse geographical characteristics Andean mountains, wide plains, Amazon jungle, coasts and islands Different type of air operations Ranging from regular international transportation to agricultural 5
Dynamically changing situation Second tower, third runway and alternate airport for Eldorado? Air activity flies high... LAN Colombia: expansion is coming Young businessmen creat aeronautical cluster Colombia and USA approve air transport agreement AviancaTaca extends flights to regions 6

Continuous commercial aviation growth 10
A renewed concept Safety management Air safety Flight safety Accident prevention 11
What was done? Adequacy of the organization of civil aviation authority Evolution from Accident Prevention Group into the Safety Management Systems Group 12
What was done? Rule making Part 22 of Colombian Aeronautical Regulation (RAC): Rules for SMS implementation 13
What was done? Coaching, promotion accompaniment and advice ICAO SMS Courses SMS Phase I and Phase II workshops Seminars Supervision of implementation 14
SMS has to be implemented by: Organization type Qty. Training organizations 41 Approved maintenance organization 83 Aircraft operators 136 Aerodromes 45 ATS 1 Total 306 Nine SMS has been implemented and approved Most of the organizations are on Phase II Reactive processes completion 15

Key issues in SMS Implementation Good attitude from organizations One person in charge Highly motivated With a good knowledge of the organization To attend formal SMS training To be original and keep simplicity To have a clear concept that SMS is neither a manual nor a computer program To permit guidance and advice from Safety Management Systems Group 17
Proven benefits from SMS implementation It generates business profits Guarantee of continuity or the organization Lower insurance costs It creates favorable conditions for competitiveness A better representation before international markets A higher demand and business opportunities It complements properly other quality and safety-security processes In order for the organizations to perform as a system It reduces air accidents As the number of operations and passengers increases, costs due to accidents are reduced: organization sustainability 18
What do we have ahead? To go forward on implementation phases: To reach all the big dimension and variety of air operations in Colombia Non regular, agricultural and general aviation requires special efforts and processes for implementation To manage information flow resulting from SMS implementation Among organizations Between organizations and the State From the State to international organizations To improve risk management processes To Sustain SMS already implemented Stability and continuity of people in charge and processes Maturity of proactive and predictive methods 19
The State Safety Program, Integrated regulations, guides and activities for safety improvement: To identify shortages To adjust structural organization To issue safety regulation To identify and involve service providers (not only flight operators) To investigate accidents and incidents To establish a hazard report system, voluntary and mandatory To analyze and share safety data To promote safety SSP 20
The State identifies its shortages: The State Safety Program, SSP Gap analysis, SSP ICAO audits Other audits: IASA EASA State controls 21
TODAY SSP: the State adjusts aviation authority structure and processes SAFETY SECRETARY SUPERVISES: OPERATIONS AIRWORTHINESS TOMORROW SAFETY DIRECTION WILL SUPERVISE: OPERATIONS AIRWORTHINESS ATS AIRPORTS 22
SSP: the State issues regulation and support documentation 23
SSP: the State investigates accidents and incidents ECCAIRS European Coordination Centre for Accident an Incident Reporting System 24
Safety 13% Others 16% Scope Operations 26% Airports Private 10% SSP: the State promotes safety ATC 14% Maintenance 8% Flight and grnd. schools 5% Airports State 8% Aeronautical Organizations 687 45% Universities 32 2% Other State organizations 8 1% Participation CAA 503 33% Armed Forces 283 19% 25
SSP: the State promotes safety 26
SSP: what are we working on? To computerize hazard report system To look for an effective risk barrier implementation, and to close the loop of risk management To improve safety analysis and to share data To publish a formal document with all the SSP regulations and support documentation 27
Accident investigation in Colombia Civil Aviation Authority General Director Secretary of Flight Safety Accident Investigation Group Technical Investigations Technical support Legal support 28
Fulfillment of Annex 13 and ICAO documents Up to date national regulation (RAC 8) Key issues in accident investigation Use of ECCAIRS Training and competence of investigators Total technical independency for investigating Reliability and confidence from aviation community on investigative processes Investigation is linked to SMS and SSP implementation 29
Strengths Experience in accident investigation Human resources of Investigation Group Systematization of investigation processes Support received from universities and from the Colombian Air Force Prompt and valuable support from NTSB, TSB and other organizations Assistance received from a lawyer regarding legal issues 30
Difficulties Lack of specialized laboratories for analysis Difficulty in arriving at remote regions in the country Requirements from other organizations for legal purposes Use of reports for other than safety purposes 31
Our objectives and projects To strength regional Groups for on site investigation Foundation of Investigation Center, with the Colombian Air Force Acquisition of flight registering decoding and simulation capability To move Investigation Group to a higher level, autonomous from Civil Aviation Authority 32
Constant decrease of accidents And constant decrease of rate of accidents 33
Consistent low number or no accident on regular operation (scheduled) And zero (0) fatalities on on demand operation in 2012* * There were 14 fatalities on general and agricultural aviation 34
Significant reduction of accidents on on demand operation and on helicopter operation in 2012 35
Comparison with Latin- America and Caribbean* Colombia shows less accidents compared with the Region average * Based on 3rd. RASG PA Annual Safety Report 36
What do we need to improve? 37
Conclusions Operational safety is a matter of interest for all kind of aeronautical organizations SMS implementation creates confidence and competitiveness Some of the key elements for a successful SMS are originality, simplicity and applicability to particular operational characteristics of a particular organization It has been proven that nevertheless the challenges, doubts and difficulties, SMS implementation is possible Accident investigation, as a reactive method, continues been a valuable process in implementation of SMS 38
