The Lake Mary Parks & Recreation Youth Summer Camp returns to the Lake Mary Community Center for its fourth year! The program features camp activities, field trips, sports, games, and much more. The program is available to children ages 6 years-old to 11 years-old. THREE FIELD TRIPS PER WEEK! Lake Mary Parks & Recreation Summer Camp RULES & REGULATIONS REGISTRATION City Resident Registration April 2, 2018 April 13,2018 Open Registration April 16, 2018 A City Resident is someone who lives inside the corporate boundaries of the City of Lake Mary. Residency will be verified at time of registration by Lake Mary Parks & Recreation staff. In order to register, the following will be required for each camper: o Completed Registration Form; o $25 weekly deposit, per each week registering for; o Signed Camp Rules & Regulations (signed by both parents); o Completed Liability Release Waiver (signed and notarized by both parents) A child must be between the ages of 6 and 11 years-old at the start of the first session of camp and they must have been enrolled in elementary school (Kindergarten thru 5 th grade) for the 2016-2017 school year. A completed LIABILITY RELEASE WAIVER must be submitted at time of registration, for each camper. A liability release waiver must be completed by BOTH parents/legal guardians AND must be notarized by a Notary Public. Notary Public services are available at the Lake Mary Community Center, free of charge. Any questions regarding waiver requirements or hardships should be directed to staff at 407-585-1416. Only registered campers will be allowed to participate in camp. Visitors (including parents, legal guardians, relatives, and/or nannies, etc.) will not be permitted. The main form of communication for weekly camp information and parent reminders is through email. Please be alert for these communications. CANCELLATIONS Any cancellation that is received prior to the payment deadline for a registered week is eligible for a refund of the fees paid, excluding the initial $25 non-refundable/non-transferable deposit. Once payment has been made and the payment deadline for the registered week has passed, there will be NO REFUNDS/TRANSFERS for cancellations. DROP-OFF & PICK-UP All campers must be checked IN and OUT by their parent/legal guardian or authorized person that has been designated on the camper s Registration Form. PHOTO ID WILL BE REQUIRED FOR CAMPER PICK-UP. The final pick-up time is 5:30pm. An additional $5 late fee will be assessed for every 10 minute window thereafter, per camper. GENERAL GUIDELINES The camp will maintain a camper to counselor ratio of no greater than 15:1 during most field trips. A safe, fun environment will be maintained. Behavior that may cause physical and/or emotional harm will not be tolerated. On-going/continued discipline problems may result in the camper being removed from the activity or the camper s removal from the Summer Camp Program. REFUNDS WILL NOT BE GIVEN FOR CAMPERS WHO HAVE BEEN REMOVED FROM THE PROGRAM DUE TO BEHAVIOR ISSUES. For the health of all campers and staff, we ask that children who are sick do not attend camp. If a child becomes sick at camp, staff will contact the parent/guardian and ask that the camper be picked up immediately. The child must be clear of the illness for a complete 24 hours before returning to camp. FOR ANY AND ALL EMERGENCIES: o 911 will be called immediately, followed by a call to the camper s parent/legal guardian. o Should a child require an emergency epi pen or emergency inhaler, an additional waiver will need to be completed before camp begins. The epi pen will be kept in a secure location either at the camp facility or in possession of the Lead Counselor. NO OTHER MEDICINE WILL BE KEPT AT THE FACILITY OR BY THE CAMP STAFF. o If your child requires daily medicine, please make sure it is taken before arriving to camp, or make arrangements with the Lead Counselor for you to visit the camp during the day to administer the medicine to your child. The City of Lake Mary will not be responsible for any lost or stolen items. Camp staff asks that campers leave all cell phones, tablets, ipods/ipads, video games, toys, or other personal items at home, unless otherwise approved by Lead Counselor. The camp day is full of activities, so these items are not necessary. A phone is always available for campers, should they need to contact a parent/guardian. LUNCHES: o Campers will need to bring non-perishable lunch with them each day, unless otherwise specified. o Lunches will not be stored in a refrigerator. o There will be two specified snack times throughout the day, one in the morning and one in the afternoon. Please pack accordingly for your camper. o Snacks WILL NOT be available for purchase at the camp facility. CAMP ACTIVITIES Most Mondays and Fridays, the camp will remain at the camp facility for the entire day. A variety of activities will be scheduled to provide a diverse mix of indoor and outdoor fun. Activities may include (see camp calendar for full listing): o Arts & crafts, sports clinics, structured outdoor games, board games, movies, and science projects FIELD TRIPS The program registration fee INCLUDES all field trips and camp activities. Campers WILL NOT be allowed to bring extra spending money to camp or on field trips. All camp staff and campers will attend each field trip. If a child is not attending a field trip, their parent/guardian will have to make other arrangements for them that day. Supervision will not be available at the camp facility when the camp is out on a field trip. Campers must be on-time for all field trips. The camp will not be able to wait at the camp facility for late dropoffs. Water Field Trips: o SPLASH PAD Features a zero-depth water playground, no swim test required. Counselors enter the o o splash pad with the campers. SHS POOL - All campers will be given a swim test by the lifeguard before being allowed to enter the deep end of the pool. Life jackets will be made available to those campers who need them. Counselors swim with the campers. DAYTONA LAGOON Campers have the option on what rides/activities in which they wish to participate. Lifeguards are on-site, throughout the park. Counselors swim with the campers. ALL Thursday field trips will include a lunch. Campers who do not want or like the lunch choice will need to pack their own non-perishable lunch (in a disposable bag) for the day. Campers WILL NOT be allowed to be dropped off to or picked up from any field trip location, unless prior approval is obtained from the Lead Counselor or Recreation and Events Manager. Campers will be given two (2) camp shirts and MUST wear the camp shirts on Wednesdays and Thursdays. CITY OF LAKE MARY RELEASE, WAIVER AND INDEMNIFICATION AGREEMENT (ADULT ON BEHALF OF MINOR) NOTICE TO THE MINOR CHILD S NATURAL GUARDIAN READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF THE CITY OF LAKE MARY USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR CHILD S RIGHT AND YOUR RIGHT TO RECOVER FROM THE CITY OF LAKE MARY IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND THE CITY OF LAKE MARY HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM. I, for myself, my heirs and personal representatives, hereby assume all liabilities, risks, injuries and hazards to my child, _, incidental to, or as a result of, participation in YOUTH SUMMER CAMP PROGRAM 2018, including transportation TO, FROM, AND DURING the said activity. I freely acknowledge the fact that this/these Program(s) and/or activity(ies) may have, and/or do involve, physical contact or other conditions or factual circumstances where physical or other injuries may occur, and that transportation to and from said event could involve the potential for an automobile, or other, accident. As legal guardian and/or natural parent of the above referenced child/children, I do hereby warrant that my minor child is in good health and has no physical condition that would prevent him/her from safely participating in the Program(s) and/or activity(ies) identified above. If my minor child has any medical or physical limitation, I have made the Program s staff aware of such limitations in writing in advance of my minor child s participation in the Program(s) and/or activity(ies), which may/may not include swimming or water activities.

As legal guardian and/or natural parent of the above referenced child/children, I do hereby waive, release and agree to indemnify and hold harmless the City of Lake Mary, their officers, agents, employees, the organizers, sponsors, activity supervisors, co-sponsoring organizations and participants for any claim, demand, liability, costs, suits, charges or compensation for loss or injury of any kind arising out of a loss or an injury, including losses or injuries arising from the negligence of the City of Lake Mary, their agents or employees and sponsors or activity supervisors, arising from my child/children's participation in the said activity. I, as legal guardian and/or natural parent of the above referenced child/children, assume all risk of injury, liability, and loss arising from my child/children s participation or presence at said activity. I acknowledge that the City of Lake Mary will not assume any costs relating to any injury while my child/children are involved in this activity, or from transportation to or from this activity. This Waiver, Release and Hold Harmless/Indemnification Agreement is in consideration of the City of Lake Mary permitting my child/children s participation in the activity or program at issue and in further consideration of the City of Lake Mary not requiring self-funded liability insurance coverage on my part as a condition precedent to my child/children s participation in the activity. I, as legal guardian and/or natural parent of the aforesaid child/children, freely and voluntarily assume all risk of loss or injury arising from my child/children s participation in the activity whether due to my negligence, my child/children's negligence, or the negligence or intentional acts of others. I acknowledge that, absent this Release and indemnification, the City of Lake Mary, or other sponsors of the activity would not have offered me, or my child/children, the access to the activity because of unacceptable exposure to civil liability claims and/or lawsuits, or the expense of providing a program that is risk-free. By signing this waiver, I agree to indemnify any and all employees of the City of Lake Mary for any and all damages which result from any and all acts or omissions, including negligence, in whole or in part, on the part of any City employee. I have read and understood this document and sign it freely and knowingly, intending that it shall be fully operative and effective in all respects and that it waives legal rights to which I, or my child/children, might otherwise be entitled if my child/children are hurt or suffer loss during his/her/their participation in that activity.
