S/K Challenge III Participant Visit Information for the Sophos/Kydoimos (S/K) Challenge III, August 2016 Access to Dugway Proving Ground, Utah U.S. Citizens: All participants must be pre-approved in the visitor database prior to arrival. Participants will be screened through a national database. Participants or their Point of Contact (POC) will be notified by email, only if their visit request is denied. Participants are required to submit attached Dugway Proving Ground (DPG) Visit Request Form to John Gomes (daniel.j.gomes4.civ@mail.mil) and Renee Drochner (renee.k.drochner.ctr@mail.mil). Participants must provide the following required information: Arrival Date (MM/DD/YYYY) Departure Date (MM/DD/YYYY) Full Name Shown On Driver s License Rank/Grade Date Of Birth (MM/DD/YYYY) Driver s License Number Driver s License State Of Issue Cell Phone Number Company Name/Organization CAC Holder (Yes/No) Upon arrival at the DPG Main Gate, participants will veer to the right at the Y and proceed to the Visitor Control Center (VCC) where a Visitor Access Form 5 will be issued. Participants must provide a valid driver s license, vehicle registration and insurance. The VCC is open Monday through Thursday, 0700-1700 hrs Mountain Standard Time. When the VCC is closed, on site DPG security guards will issue all Visitor Access Form 5s. Be aware that your vehicle may be searched upon entrance and is subject to search at any time while on the installation. Foreign Nationals: Foreign National participants much request visitation through their local embassies no later than (NLT) 60 days in advance. A visit request form must be submitted to the DPG Counterintelligence office NLT 30 days in advance. The DPG Counter Intelligence Office can be reached at (435) 831-2910, fax (435) 831-2499. Upon arrival at the DPG Main Gate, Foreign National participants will veer to the right at the Y and go to the VCC, where an U.S. Citizen escort will meet them. A Foreign National Badge will be issued and must be worn at ALL times while on the installation. Foreign National participants MUST be escorted by a U.S. Citizen at ALL times. 1

Directions to DPG: From Salt Lake City Airport via I-80 - travel West on I-80. Take exit 77 Rowley/Dugway. Turn left at the bottom of the ramp onto UT-196 South. Travel South until you reach the end of the UT-196 South road, which terminates at DPG Front Gate (Figure 2, Route A) NOTE: There are no services between exit 99 Lakepoint and Dugway (approximately 60 miles). Please ensure you have enough fuel and needed items. From Salt Lake City Airport via through Tooele - travel West on I-80. Take exit 99 Stansbury/Tooele. Travel South on UT-36 (Main Street, Tooele) through Tooele and Stockton for approximately 16 miles. Turn right on UT-199 West. Continue on UT-199 West through Johnson s Pass until you reach the end of the road, which terminates at DPG Front Gate (Figure 2, Route B). NOTE: There are no services between Stockton and Dugway (approximately 31 miles). Please ensure you have enough fuel and needed items. 2

Respirator: Participants and observers who wish to access the test site within the hazard area are required to have a full-face respirator in their possession. If participants and observers would like to obtain a respirator at DPG, they may choose one of the following options: Fill out the attached Protective Mask Request and send it to the DPG Health Clinic (ATTN: Trina Hendrickson (demetria.t.hendrickson.civ@mail.mil, fax 435-831-2871). The health clinic will review and determine if an appointment is necessary. If an appointment is not needed, the health clinic will notify TAP Laundry that the participant already approved. The participant will go to TAP Laundry, located in Ditto, to obtain a respirator and complete a fit test during operating hours of Monday-Thursday, 0700-1630 hrs. If an appointment is required, the participant will be notified and will need to call the health clinic to make an appointment NLT 2 weeks in advance. The health clinic can be reached at (435) 831-2211, Monday-Thursday, 0700-1700 hrs. Participants and observers are allowed to provide their own respirator without a DPG medical review and fit test; however, documentation must be provided upon request. Respirator should be National Institute for Occupational Safety and Health (NIOSH) approved with an organic vapor/acid gas/particulate filter (3M 60923 or North 7583P100L) and should be fit tested. Participants and observers attending VIP Day only, are not required to acquire a respirator. NOTE: The DPG Command may waive the respirator requirement for participants and observers. If a respirator is not worn, participants and observers must wear safety glasses or safety goggles.
