Please be mindful, school tuition and financial obligations must be paid up to date before registering your child/ren for camp. Dates : Time: After Care: LOCATION: LUNCH: SNACK: ATTIRE: SUPERVISED ACTIVITIES: FEES: June 15th Aug. 4th Monday Friday 8:00 am 3:00 pm 3:00 pm 6:00 pm Annunciation Junior High - Drop off and pick-up Lunch is Not provided. You must send lunch and a drink, unless otherwise noted. Snack and drinks are provided in the mid morning and again in After Care. Cool and comfortable play clothes with SNEAKERS. Qualified staff of Annunciation School offer age appropriate activities daily: Arts & Crafts, Pottery, Exercise, Basketball, Volleyball, Kickball, Soccer, Croquet, Bingo, Board Games, Karaoke, Zumba, and Free Play. WHEELIE DAYS - Tuesday & Thursday, (bring bikes, scooters, skates & helmets, a must!) WET Wednesday!, FIELD TRIP Friday or Bar-B-Q Cookout and the Works! Registration Fee - $100 per child or $150 per family Weekly Camp Fees: 1 child $140, 2 Children $200, 3 children $250 After Care Fees are per day: 1 child $10, 2 Children $17, 3 children $20 RESPECT AND PROPER BEHAVIOR IS EXPECTED AT ALL TIMES Please fill out and sign the attached application form. AUTHORIZATION AND RELEASE FROM LIABILITY I grant permission for my child/ren to participate in all off campus field trips. I grant permission for sunscreen to be applied to my child when needed. I grant permission for my child to be photographed during camp activities. I agree that no reduction in the weekly fee will be made for late arrivals or early departures. I understand that Camp Registration is not refundable. I understand personal items should be left at home. In the event, they are brought to camp, the Summer Camp Staff is not responsible for lost or broken items. I understand that if my child becomes disruptive, disobedient, or disrespectful to the staff or any other child in camp, I will receive a phone call and be asked to pick up my child, immediately. I do hereby release and hold harmless Annunciation School Summer Camp, its officers, agents, employees and the Archdiocese of Miami of any and all responsibility and liability for any injury or claim resulting from participation in camp activities on campus and off campus and agree to take no legal action against Annunciation School Summer Camp, its officers, agents, employees and the Archdiocese of Miami.
