BACKGROUND Article 13, clause no. 13.2 of the Concession Agreement states that: 13.2 Inspection During the Construction Period the Independent Engineer shall inspect the Project Highway at least once a month and make a report of such inspection (the Inspection Report ) stating in reasonable detail the defects or deficiencies, if any, with particular reference to the Scope of the Project and Specifications and Standards. It shall send a copy of the Inspection Report to the Authority and the Concessionaire within 7 (Seven) days of such inspection and upon receipt thereof, the Concessionaire shall rectify and remedy the defects or deficiencies, if any, stated in the Inspection Report. Such inspection or submission of Inspection Report by the Independent Engineer shall not relieve or absolve the Concessionaire of its obligation and liabilities hereunder in any manner whatsoever. To comply with the requirement of the above clause of Concession Agreement, a Joint Site Inspection program of the Project Highway was planned for 26-02-2015 by a joint team comprising NHAI,IE, Concessionaire and EPC Contractor and the same was informed to the Concessionaire vide this office letter ref. no. LASA/BK/IE/73426/2015-840, dated 22/01/2015. During site Inspection, the representative of Concessionaire Mr. Amarnath Das (GM), Authorised signatory Mr. Sarwan Kumar EPC Contractor Mr. E. Daragaiah (DPM, structure), Mr Adinarayan (Material Engineer), and other site engineers were available during site Inspection. This Joint Site Inspection Report is based on the findings during site inspection by the whole team of IE (TL, RE, B/SE, SQME, QS, AHE-2, ABE-2, AQME-2 and Survey Engineer). Project Brief The NH-34 Barasat to Krishnagar Section in the state of West Bengal is located in the northern part of Metropolitan city Kolkata. It is part of the proposed Barasat to Raiganj section. The NH-34 is an integral component of the National and Regional road network. The project highway described in this report is under construction and runs from Km 31.000 to 115.000 between Barasat to Krishnagar on NH 34 in the state of West Bengal. The completed NH-34 will connect the traffic from Kolkata to Siliguri as well as facilitating the movements between the State highways (SH), Major district roads (MDR) and other district roads (ODR). The increase in car ownership, changes in land use and travel patterns have resulted in congestion on many parts of the State road network including NH-34. The NH-34 currently not only functions as a main corridor to Nepal and Bangladesh but as an urban collector/distributor between communities, employment and shopping centers. The alignment traverses through two Districts North 24 Paraganas and Nadia. The terrain along the project road is generally plain. The predominant land use along the project road is agricultural with ribbon development near human settlements. At certain locations, encroachments within the ROW have been observed in the form of Shops, Hotels, Dhabas and various other commercial establishments. The road passes through towns of Kamdebpur 1

(22 47 13, 88 30 13 ), Amdanga (22 34 06, 88 30 27 ), Gadamara (22 54 00, 88 32 21 ), Barajaguli, Chakdah (23 04 52, 88 32 35 ), Ranaghat, Phulia, Shantipur (23 15 59, 88 26 50 ), Govindapur and Krishnagar (23 22 40, 88 29 10 ) etc. I. SCOPE OF WORK Map Showing Road Alignment of NH-34 The scope of the Access Controlled Highway Project as detailed in the concession Agreement shall be broadly classified as follows: Construction of Four Laning of NH-34 from km 31.000 to km 115.000 (Existing chainage) or 115.341 (Designed chainage) in the state of West Bengal. Construction of 2 nos of Major bridges, 5 nos of Minor Bridges, 134 nos of slab culverts and 17 nos of Pipe culverts on main carriage way under New/widening/ Reconstruction Scheme as per Schedule B of Concession Agreement. Construction of 4 nos of ROBs, 1 no of RUB, 2 nos of Vehicular Underpass, 12 major junctions, 14 nos of minor junctions, 1 no of toll plaza and others Schemed as per Schedule B and Schedule C of Concession Agreement During the entire construction period, the Project Highway is to be maintained by the Concessionaire at his own cost from the appointed date. 2

Traffic Management and Safety of workers and the road users shall be maintained at all times in accordance with the relevant IRC and relevant Codes as stipulated in the Concession Agreement without any deviations. The scope of the work for the Project Highway also includes but not limited to the following: Road side furniture Pedestrian Facilities Highway Lighting Systems Junctions(Major & Minor) Tree plantation and landscaping Truck lay byes & Bus lay byes with bus shelters. II. SITE INSPECTION Joint Site inspection was conducted on 26 th February, 2015 by the team comprising IE (TL, RE, B/SE, SQME, QS AHE-2, ABE-2, AQME and Survey Engineer); the Representatives of Concessionaire and EPC Contractor were available during site Inspection. This Joint Site Inspection Report is based on the findings during site inspection by the whole team of IE (TL, RE, B/SE, SQME, AHE-2, ABE-2, AQME-2 and Survey Engineer). The visit started at 10:30 AM from Project end point (at Km 115+000) and concluded at Km 51+000 on the same day at 5:30 PM. III. FINDINGS OF JOINT SITE INSPECTION There is some improvement in construction works at site as the Concessionaire is expediting Earthwork and GSB work. Similarly, the Concessionaire is continuing the works of Major bridges, Minor Bridges, RUB and culverts. The works which are under progress were inspected. The following are the findings: 1. Cutting of Jalangi River Bond The Concessionaire has cut the Jalangi River earthen bund for the construction of test pile for bridge on Jalangi River. Similarly Concessionaire has excavated and took away the soil for embankment construction from both side of the NH-34 endangering the earthen bund constructed by Irrigation. Irrigation Department complained about the activity of Concessionaire and requested to stop the excavation work immediately. Concessionaire is instructed to stop excavation work immediately and mend the damaged portion of river bond. Concessionaire representative assured that they will maintain and make the bund continious,when earthwork in this location will be started. 3

Major Bridge at Km 113+525 Concessionaire has mobilised new sub-contractor for piling work. The sub.contractor managed one MAT machine, one crane machine, three Bentonite Tanks and steel sheets for liner etc. at site. The Concessionaire is instructed to submit the methodology for piling.. 4

RUB (Km 112+215) The construction of RUB is going on as planned. The following is the progress status of RUB work: a. Left Tunnels: Unit A E Box completed b. Right Tunnel Reinforcement work for Unit A was in progress Engineer from ITD cementation (Sub-contractor for the construction of RUB) said that they are facing problem for pushing of boxes due to underground communication cables of railways laid right beside the railway embankment. He reiterated that shifting of underground cable is dealt with high priority with the Railway Authority. 5

Planned for FDD test during inspection but the program was relinquished due to over moisture in the bed. The Concessionaire is instructed to plow the bed again for drying and compact the same at OMC. 5. Sub-grade top layer at 102+260 102+350 (LHS) Field density was checked for sub-grade second layer. The density of sub-grade was found not satisfactory. Only 93.21% of dry density was found which is below the required value as per MORT&H Specification. Concessionaire was instructed to continue compaction operation to achieve required density (Report enclosed as Annexure-I) Checking of FDD of sub-grade layer at Km 102 + 300 (LHS) 6. Return wall of box culvert at KM 100+623 Rienforcement work and form work of return wall was complete for this culvert. The work was found satisfactory and allowed for concreting work. Reinforce and formwork checking @ KM 100+623 (LHS) 7

7. FDD checking of embankment at KM 93+375: FDD was checked for embankment at KM 93+375 (LHS). Only 89.60 % of FDD was found. The result failed. Hence, the Concessionaire was instructed to continue compaction operation to achieve required density (Report enclosed as Annexure-II) FDD checking of embankment layer at KM 93+375 (LHS) 8. Concessionaire site laboratory Compressive strength of ite cubes of M35 (Pile) concrete for Major Bridge at KM 77+800 was tested and found satisfactory. Similarly mortar cubes of Ambuja cement 43 grade of 4th week tested and found saisfactory. (Results encosed in the Annexture - III ) Checking of Compressive strength checking of concrete cubes at labrotory 8

9. Major Bridge at Km 77+800: As per the information provided by the Concessionaire, piling work of Major Bridge at Km 77+800 was re-started from 27.01.2015 and was in progress. The Concessionaire was instructed for proper safety arrangement as safety arrangement at site was found unsatisfactory. Unsafe vertical cut of existing embankment at the approach of existing bridge needs immediate attention for protection of slope 10. Box Culvert @ Km 68+224 (RHS): Raft was ready to receive concrete. Reinforcement and form work checked and found satisfactory. Allowed for concreting. 10

Density of sand 2 nd layer was checked and found unsatisfactory. Concessionaire was instructed to continue the compaction operation till required density is achieved (Report enclosed as Annexure-IV). Construction Works From Km 31+000 to Km 51+060 Slush removal and C & G work from Km 43+000 to 44+200 (RHS) are in progress. There are no other substantial works from Km 31+000 to Km 51+060. 12

15. Repair of Project Highway: Slush Removal in progress at Km 43+600 (RHS) As the Concessionaire stopped Repair and Maintenance Work from 16-03-2014 and the road severely damaged, NHAI has appointed M/S Roads (India) International as Repair and Maintenance Contractor at the risk and Cost of the Concessionaire. Roads (India) International has started to repair (potholes filling) work from 27-07-2014. Repair and Maintenance work by Roads (India) International has been completed. Following works are done Potholes Repair: 84.00 Km (100%) (except severely damaged stretch from Km 58+450 to Km 59+850, from Km 96+500 to Km 97+000 and Km 111+650 to Km 111+800) Bituminous Macadam (BM): 72.10 Km/84.00 Km (85.83%) Completed Semi Dense Bituminous Concrete (SDBC): 65.5 Km/84.00 Km (78.00%) Completed IV. RECOMMENDATION: The concessionaire has not yet fully addressed/responded/complied the instructions / comments / suggestions given by the Independent Engineer in their previous Joint Inspection Reports. It is again recommended to address the issues raised by Independent Engineer in this Joint Site Inspection Report along with previous reports and correct/rectify the deficiencies at the earliest to achieve the quality works. 13
