COMMUNITY NEEDS ASSESSMENT Last year, Deaconess worked with St. Mary s Health System, ECHO Community Health Care, United Way of Southwestern Indiana, and Welborn Baptist Foundation to create a Community Health Needs Assessment action plan. The plan revolves around targeted and consistent efforts to address four major health issues in the Tri-State: TOBACCO USE SUBSTANCE ABUSE OBESITY MENTAL HEALTH Our goal is that by addressing these needs, we will help foster a healthier and happier community. To that end, Deaconess partnered with 65 providers to create an accountable care organization called Deaconess Care Integration (DCI). An accountable care organization, or ACO for short, is a concept that comes from the Affordable Care Act. In an ACO, a collaborative network of doctors, hospitals, specialists and other health care providers agrees to work together as a team to improve patient health and be accountable for reducing health care expenses. Thanks to our programs for diabetes management, high-risk care coordination and medication therapy management, we ve made a real difference in the health of our patients. These innovative programs have helped Deaconess bend the cost curve for our patients, making good health more affordable. IMPORTANT COMPONENTS OF OUR ACO Intensive Outpatient Management In 2013, we created an intensive outpatient management program that identifies the root cause of repeat hospital visits and works to decrease the number of avoidable visits. To select patients for this program, we identified the top 1% of hospital resource users. This includes patients with frequent visits to the Emergency Department, multiple hospitalizations, and long lengths of stay. These patients are referred to a social worker for one-on-one counseling to determine why they come to the hospital more frequently and what resources will help them better manage their health. In the first 12 months of the program, we helped 50 patients better manage their chronic conditions using resources from 70 local agencies. This intervention reduced the group s inpatient days from 953 to 740. Medication Assistance Program The Deaconess Medication Assistance Program (MAP) works with public and private services to help eligible patients receive their medications at no cost or reduced cost. In fiscal year 2013, MAP helped 955 patients get the medications they needed to control their chronic health conditions.
