OneCare Pastimes Guide, November 2010 This guide contains a list of OnePath s underwriting ratings for 124 sports and pastimes. Pastimes This guide details the loadings and exclusions that apply if the life insured participates in certain sports and/or pastimes. Some of these sports and pastimes require the life insured to complete supporting questionnaires and/or provide full details in the Application Form. You can access the supporting questionnaires for the following pastimes in OnePath s Illustrator by clicking on Forms, then Underwriting and then Sports and Pastimes : Abseiling/rockclimbing/ Boat/sailing/yacht racing Diving Hang gliding Motor sports Parachuting For applications submitted through OneCare Express, any relevant questionnaires will automatically be produced. Acceptance rating key $1, $2, $2.50, $3, $5 The sport and/or pastime is eligible for this cover type; however, an additional $ amount per $1,000 amount insured will be added to the base rate (e.g. additional $2 per $1,000 amount insured). This will also be applied if the life insured chooses a monthly instalment benefit payment type. For the lump sum equivalent, it will be applied as an additional $ amount per $1,000. This is applicable for Life, Trauma and the Accidental Death Benefit under Extra Care only. 25% The sport and/or pastime is eligible for this cover type; however, a 25% loading of the base rate will apply. This is applicable for Income Secure, Business Expense and Living Expense only.
IC The case needs to be referred to a OnePath underwriter for individual consideration. NA The sport and/or pastime is ineligible for this cover type. OR Standard rates apply. Exclusion The participation in or preparation for the sport and/or pastime is excluded from cover for as long as the life insured is actively involved in it.
