Terms and Conditions Eligibility The holiday clubs are for young people aged 11-17 and are specially designed for young people with additional and complex needs. All sessions are 25.00 per day and transport is included with the typical day being 10am-3pm. All places are non refundable once confirmed. Once the place has been confirmed, please pay the remaining balance within 7 days. Stockport CP has the right to offer the place to another person if payment is not received within this time. Please provide suitable clothing for the location, activity and weather. Pick up and drop off point for all sessions is Granville House, 20 Parsonage Road, Heaton Moor, Stockport, SK4 4JZ. Medication We can only dispense pharmacy labelled medication. All customers must adhere to Stockport CP s medication policy which will be detailed in your information sheet. Insurance Stockport CP has a comprehensive public liability insurance policy. I m interested, how do I book? If you currently access any Stockport CP services, please complete the booking form in this leaflet and return to the address specified along with your cheque. Places will be allocated subject to an up to date assessment. If you are new to Stockport CP, please call our dedicated customer pathway team on 0161 432 1248 and they will handle your enquiry and booking. This will involve a Stockport CP Support Planner coming to your home to put together a support plan to make sure the young person is well supported on the day.
