OHIO STATE WOMEN S SOCCER 2018 DAY CAMP CHECK-IN PROCEDURES Dear Day Camper, Congratulations on selecting The Ohio State Women s Soccer Day Camp! This letter is to notify you we have received and processed your application. To complete the registration process, follow the steps below. 1) WEBSITE INFORMATION Prior to camp, please re-visit our website. We will accept walk-up registrations. Be sure to pre-order your camp balls for $25.00 as they will be $30.00 at day of registration. Supplies will be limited. Please do not drop off a camper or leave unattended in the mornings before 8:45 a.m. or without our camp staff visible. OSU CAMPUS DIRECTIONS WOMEN S SOCCER CAMPS CONFIRMATION PACKET OHIO STATE WOMEN S SOCCER 2018 DAY CAMP CHECK-IN PROCEDURES Maps and specific directions to the Jesse Owens Memorial Stadium are available on-line at : http://www.osu.edu/map/building.php?building=092 When you arrive at the Stadium, park in the Jesse Owens/Buckeye Field Lot (see enclosed map, number 14) and look for Ohio State Women s Soccer Camp Staff at the south gate entrance. Registration will occur in the concourse under the stands. General Parking Instructions at Ohio State: The use of University parking lots requires a permit at all times. Unless instructed otherwise, you should plan to purchase a temporary parking permit when you park on campus, including when using the parking lots near our camp athletic fields. When parking, please make sure your vehicle is in a legal, marked space. Hourly and short-term permits also may be purchased using the self-service Pay-n-Display machines located in these lots. Please note that these machines only accept coins or credit cards (no paper bills). When you purchase a temporary permit from a Pay-n-Display machine you may park in A, B, or C parking space (see signs in the lot) in the parking lot where the Pay-N- Display pay machine is located. For more details on the use and location Pay-n-Display machines, visit: http://www.campusparc.com/osu/visitors-patients. Parking Instructions for Day Camps: Campers, coaches, and parents should park in the Jesse Owens/Buckeye Field Lot (see map, number 14) during camp check-in and check-out. Parking will be FREE in this lot during the camp s check-in, drop-off and pick-up. Campers and their parents should park in the designated lot for check-in, drop-off and pick-up. Parents and guests observing camp will need to purchase a parking pass for the day(s) they attend. Passes should be obtained at the Pay-and-Display machines located in the parking lots. Passes must be visible from the dash or rearview mirror, and cars must be parked in designated area(s). Any vehicles that are not parked in the area designated for Soccer Day Camps will be subject to parking tickets. Parking fines are the responsibility of the vehicle owner.

CHECK-IN PROCESS STATION 1: PAYMENT/ TEAM ASSIGNMENT: Your first stop will be to verify registration, clear any balances and receive your camp team assignment. STATION 2: SAFETY MONITORS: You will proceed through the line to see the safety monitors to verify your Parent Consent Form. At this time, please inform the safety monitors of any medications, allergies or injuries you have incurred prior to camp. STATION 3: CAMP T-SHIRT: You will continue through the line to receive your camp T-shirt. WOMEN S SOCCER CAMPS CONFIRMATION PACKET WOMEN S SOCCER CAMP 2018 DAY CAMP CHECK-IN PROCEDURES STATION 4: CAMP BALL: You will pick up your pre-ordered camp ball or you may purchase a ball for $30 while supplies last. Bring a credit card or at least one blank check for any balance due or the purchase of a camp ball. Introductions and team roll call will begin at approximately 9:00 a.m. the camp followed by the first session. Parents are welcome to stay for any sessions. WHAT TO BRING WITH YOU TO CAMP EACH DAY LUNCH: Lunch will NOT be provided; Full Day Campers should bring a packed lunch. Water and Gatorade will be available through the duration of the camps. SNACK: Snacks will not be provided; There will be a 15-20 min break halfway through camp in which campers should bring a snack to refuel before the second session. Water and Gatorade will be available through the duration of the camps. BAG: Bring a bag to hold and carry all your belongings. CLOTHES: Be prepared for any type of weather. A sweat shirt and rain gear is recommended. Be sure to label personal items. For Full Day Camp, bring a bathing suit and towel for swimming at lunch time. SHOES: Bring soccer shoes (molded soles) for the grass, flat-soled shoes to play on the indoor Field Turf (pending availability). Shoes/sandals must be worn at all times. SHIN GUARDS: All campers must wear shin guards for every session. WATER BOTTLE or JUG: We do not provide water bottles and do not recommend sharing water bottles or jugs with other campers. Please bring your own and have your name on it. SOCCER BALL: Camp balls are available for pre-order purchase of $25.00 or at the time of camp enrollment for $30.00. Supplies are limited at check-in. If you do not purchase a camp ball, please bring your own ball, have your name clearly marked in permanent ink, and properly inflate it each day. SUN SCREEN: It will be hot and sunny so we recommend that every camper bring and wear sunscreen.

WOMEN S SOCCER CAMPS CONFIRMATION PACKET WOMEN S SOCCER CAMP 2018 DAY CAMP PROCEDURES DEPARTURE PROCEDURES HALF DAY CAMP: Daily Pick-up is at 11:30am at the Upper & Lower Practice Fields (see enclosed map, letter C) just west of the soccer stadium. FULL DAY CAMP: Daily Pick-up is at 4:00pm at the Upper & Lower Practice Fields (see enclosed map, letter C) just west of the soccer stadium. *Pick-up location may change - parents will be notified of any changes at check-in. ADDITIONAL INFORMATION MORNING DROP OFF: Camp supervision begins at 8:45am each morning. Please do not drop your daughter off unsupervised before 8:45am. If special arrangements are necessary, please speak to the camp administrators at check-in. MEDICATIONS: Medications are generally not permitted at camp. Players may only carry medications to the event if such medications are intended to treat a potentially life-threatening condition. With prior written approval from the Camps Office, a player may carry an EPI-pen and/or an inhaler for asthma. In order to bring an EPI-pen, asthma inhaler or any other medication to the event, a parent or guardian must make a written request for approval to the Camps Office. An EPI-pen, asthma inhaler and any other approved medications must be in the original prescription container with the child s name clearly marked. Children are required to self-administer medications whenever possible. Camp staff members, however, are able to administer these medications in an emergency. To make a written request to bring medication to camp, please contact Makena Lynch (Asst. Director Buckeye Sports Camps Office) by email at lynch.400@osu.edu. In order to adequately assess your information, please contact us at least 14 days prior to the start of the event. ACCOMMODATIONS: If you have questions about accessibility or you need to request assistance to participate in any of the Ohio State Athletics Department s sports camps, including accommodations for dietary restrictions, please contact Makena Lynch (Assistant Director - Buckeye Sports Camps Office) by email at lynch.400@osu.edu. In order to adequately assess your request, please contact us at least 14 days prior to the start of the camp. SPECTATORS: Parents and other spectators are welcome to observe the camp(s). In order to create a safe environment for our participants and spectators, parents should follow the instructions of Ohio State personnel and only watch the camp from the designated public areas at the fields. Please use caution when standing at - or walking around - the fields during the course of the camps. Dogs or other pets: Animals are not permitted at our facilities. Supervise younger children: Parents should closely supervise any children who accompany them to watch the event. Unsupervised children risk being injured during the course of the camp. All participants should refrain from using, moving or otherwise handling any Ohio State sports equipment that is located at the athletic fields.

CAMPUS CONSTRUCTION: Campus construction is occurring in phases and will be changing through the summer. For the latest information, visit http://go43210.osu.edu/. For the campus construction map, visit http://www.osu.edu/map/construction/. EMERGENCY CALLS ONLY: Phone Lisa Davis at the Soccer Stadium (614) 292-9017 or Buckeye Sports Camp at (614) 247-CAMP. You may leave a message at the soccer office 614-292-8482 but messages will only be retrieved during lunch break. REFUNDS: A full refund less a $50 administrative fee will be issued for any cancellation received by at least 2 weeks prior to the start of the clinic. The 3% online processing is not refundable for ANY reason. Because we have to guarantee our number of campers in order to prepare for camp, any cancellation received less than 2 weeks prior to the start of the camp will not be eligible for any refund except in the case of injury, illness, or mandatory school event, in which case supporting documentation from a doctor or school is required. A camp fee may not be transferred to any other Ohio State sports camp. A camp fee may not be transferred to any other camper. All cancellation notices must be received by the deadline above and submitted by mail, email, or fax - no phone calls or voicemails. All supporting documentation required for a refund to be approved and processed must be submitted within 10 days after the conclusion of the clinic session. Refund requests must be submitted on the Refund Request Form available at OhioStateBuckeyes.com/camps. Campers who are injured while at camp but remain at camp will not be eligible for any refund.

CAMPER RULES AND EXPECTATIONS Campers adherence to the following rules and expectations is critical to making our camps safe, rewarding and fun for all participants. The Ohio State University is a large campus in a major metropolitan area. The safety of our campers is a primary concern. Expectations for camper behavior: Follow all instructions of camp staff and immediately consult the camp director if you are uncertain about these instructions. Bring all equipment that is required to participate in camp. Label all personal belongings and equipment. Notify a camp counselor as soon as any problem arises, big or small. Camp staff are available to help you at all times. Follow the instructions of police, fire and other emergency personnel. Behaviors not permitted Harassment, hazing, and bullying of other campers or staff. This includes verbal, physical, and cyber. Using profanity or inappropriate language whether it is written or verbal. Leaving the camp area as designated by the camp director. Campers are not permitted to walk freely around the campus or surrounding areas. If your parent is picking you up prior to the completion of any camp session (day or term), you must give the camp director a note from your parents in advance. Never travel alone. Visitors are not permitted in the camp dorm or dining areas. Spectators are only permitted to watch camp sessions from designated public areas. Possession and/or consumption of alcohol, tobacco, or illegal substances. Possession of matches, lighters, hot plates, kettles, fireworks, lethal weapons or other objects that the camp director considers dangerous. Possession and/or distribution of indecent literature or images. Tampering with fire alarms, safety, or security equipment. Inappropriate use of a cell phone during camp phone may be confiscated for the duration of camp. Excessive noise or horseplay. Violations of federal/state laws. Campers and parents should review this important information prior to arriving at camp. Questions may be directed to the Buckeye Sports Camps Office at (614) 247-CAMP. While unacceptable conduct is not anticipated from campers, violation of any camp rule or expectation could result in disciplinary action, up to and including dismissal from camp. Refunds will not be issued to campers dismissed from camp for disciplinary reasons. We expect that parents or guardians of campers will be available (by phone) during the duration of the camp in the event the camp director or other camp staff needs to speak with them about a camper s behavior.
