THE WEEKLY OPERATIONAL NOTICE DATE OF ISSUE The Weekly Operational Notice is issued every Tuesday. An acknowledgement of its receipt is not necessary. Every person whose duty requires them to have a copy is held responsible for obtaining one of each issue, and for communicating to their staff any instructions applicable to them. If not received at the usual time or in doubt as to your obligations, contact your Supervisor. TEMPORARY REDUCTIONS OF SPEED OF TRAINS (a) (I) Repairs and renewals are being effected at the following places and, until further notice, the speed of trains must, if required, be reduced as shown. Each location will be protected when necessary, either by hand signals, or by special permanent way signals. Train Crews must keep a good look-out at these locations for such signals, which must be exhibited in accordance with these rules. (ii) In the absence of such signals, trains may be run at the ordinary rate of speed. (b) Drivers are reminded that repairs of lines necessitating reductions of speed, of which it has not been able to give notice, may be necessary at other places beside those mentioned in this notice. They must, therefore, be always on the lookout and be prepared to stop or to run at reduced speed whenever and wherever hand signal are exhibited. (c) Work Group Supervisors and others in charge of works at the following places where the requirements render it necessary that the speed of train be reduced, must understand that this information does not in any way relieve them of the responsibility of seeing that Competent hand signallers, furnished with the necessary Hand Signals and Audible Track Warners are stationed at the places named for the purpose of signalling in accordance with these rules, or that the special way Warning and Caution signals are exhibited, as each case may require. Special permanent way warning and Caution signals are exhibited at the locations contained in the following tables. For instructions regarding the use of these signals see Rule 7, Section 15 of the Rules and Operating Procedures 1994. (d) Unless otherwise specified, all reductions of speed shown in this notice apply until the issue of the next Weekly Operational Notice. Any temporary reduction of speed which appeared in the last issue of the Weekly Operational Notice, and which is not included in this edition, may be regarded as cancelled. (e) Subject to any temporary modifications in the foregoing or other current instructions, the permanent way reductions of speed and the speed limitations shown in the Working Time-tables, and all additions to and amendments thereof, must be carefully observed. Signallers, Train crews, Work Group Supervisors, Gangers and others must promptly report their Supervisors, any case in which they observe trains running in excess of the speed laid down at or between any of the points where speed is appointed to be reduced. SAFEWORKING NOTICES NEWPORT WORKSHOPS NO. 2 GARDEN PLATFORM TRACK RESTORED TO SERVICE On Tuesday 23rd May 2018, No. 2 Garden Platform Track at Newport Workshops will be returned to service. The lockable point clip on the CCW type hand points between No. 2 and No. 3 Garden platform tracks will be removed and the points will be unspiked. Safeworking Circular SW.053/2011 is cancelled. Metro SW.135/2018 NORTH MELBOURNE POINT MACHINE RENEWAL WORKS Between 21:00 hours on Wednesday 6th June 2018 and 04:00 hours Thursday 7th June 2018, points No.427 will be provided with M3A type point machine. The Following Will Be Affected During These Works: Points: 427 Signals: NME526, NME527, NME528 & NME567. The above works will be conducted under cover of an Absolute Occupation. HEIDELBERG MACLEOD COMMISSIONING OF ST. JAMES ROAD PEDESTRIAN CROSSING WESTRACE DATA UPDATE DECOMMISSIONING AND REMOVAL OF KEY SWITCHES AT MACLEOD Between 21:00 hours on Friday 25th May 2018 and 04:00 hours on Monday 28th May 2018 the following alterations will be implemented: Commissioning of the St. James Road Pedestrian Crossing, with the Express/Stopping selection interlocked with Automatic signal S468 on the Up approach. Westrace Data update to fix the route locking issues associated with signal HDB140. Decommissioning and removal of the 11P keyswitch and associated point handle from the Macleod Panel. Decommissioning and removal of the 5P keyswitch from signal post MCD111. The above works will be conducted under cover of an Absolute Occupation. Amend signal diagram No. 25/18 titled Dennis to Macleod. An updated signal diagram will be issued at a later date. NOTE: A Network Safety Representative will NOT be in attendance to sign the Certificate ofsignalling. Metro SW.131/2018 CAULFIELD - OAKLEIGH AUTHORITY TO PASS HOME SIGNALS (NOT YET IN SERVICE) DEFINED AREA Effective from 21:00 hours Tuesday 29/05/2018, in conjunction with Absolute Occupations O.697/2018 & O.698/2018, Home Signals within the area as defined hereunder, are to be considered not yet in service and do not require authority for the passage of rail traffic: The Down Line between Signal Posts: CFD712 & OAK606 (inclusive). The Up Line between Signal Posts: OAK719 & CFD733 (inclusive). The arrangements outlined herein will remain in place only until the commencement of Signal Commissioning activities. The Operational Safeworking Supervisor will provide advice to the T.F.P.C. that the provisions of this circular will no longer be in effect and that authority must now be granted prior to the passage of rail traffic. PROTECTION OF POINTS At Caulfield, the Track Force Protection Coordinator (T.F.P.C.) must ensure that a Permit to Foul Limit Board is affixed to the rail head, on both the Up & Down Dandenong Line Leads, on the Down Side of Points No. 612, at a point clear of the Fouling Point. WORKSITE RAIL TRAFFIC WITHIN DEFINED AREA Rail traffic must not pass over points inside the defined area until a suitably qualified project representative has ensured that the track is fit for traffic, the points blades are in the correct position and secured for the intended movement. Prior to the commencement of any worksite rail traffic movement, points must be secured by a point securing device and padlock. Should the movement require the points to be altered, the Track Force Protection Coordinator must request a suitably qualified project representative to operate the points into the required position. The TFPC must re-apply the point securing device and padlock prior to the movement. When the relevant conditions herein have been met, the person in charge of the movement may permit the passage of rail traffic past the relevant Home Signals. WORKSITE RAIL TRAFFIC OUTSIDE DEFINED AREA Prior to commencing any worksite rail traffic movements outside of the defined area, the T.F.P.C. must obtain permission from the relevant Signaller (either Caulfield or Westall Panel) to undertake the movement. The Permit to Foul Limit Boards on the leads to the Up & Down Dandenong Lines at Caulfield, as required herein, may only be removed by the T.F.P.C. after authority has been granted by the Signaller Caulfield to commence the rail traffic movement. Metro SW.134/2018 OAKLEIGH SIGNAL CONTROL PANEL ABOLISHED Commencing 21:00 hours on Tuesday 29th May 2018 until 06:00 hours Saturday 16th June 2018, construction activities will be undertaken between Caulfield and Huntingdale as part of the CTD Project (Area 1) works. These works will be conducted under the cover of Absolute Occupation (O.697/2018 & O.698/2018). After the Absolute Occupations have been granted by the Signaller at Oakleigh, the interlocking must be disarranged by the MTM Signal Maintenance Representative in accordance with the provisions of Section 5 Rule 10 of the 1994 Book of Rules and Operating Procedures. At this time the Signal Control Panel and all associated signalling infrastructure at Oakleigh will be abolished. The details of the new signalling arrangements at Oakleigh will be described in a SW Circular which will be issued prior to the completion of the Absolute Occupation. Metro SW.162/2018 HAMPTON REPLACEMENT OF PEDESTRIAN & EMERGENCY GATES HAMPSTON STREET LEVEL CROSSING Civil works have been undertaken to replace the existing pedestrian and emergency gates exit gates with magnetic pedestrian gates and electromagnetically latched emergency exit gates. The Down side emergency exit gates will be commissioned into service on Friday 25th May 2018. Issued: 25 May 2018 Number L&D-BUL-76-2018 Metro Academy Training Calendar & Extensions Requests The purpose of this bulletin is to alert Suppliers and Worksite Protection personnel of the process for extension requests over the upcoming recertification window. As Metro is the only provider approved to deliver recertification training the following must be adhered to. What do I do if my Lookout, Handsignaller, TFPC or TVO competency is about to expire OR has expired? 1. You need to log in to the Platform and book a course (If you are new to the Platform see below) Once you ve booked in you will be sent an email confirming your attendance. 2. Your employer will need to send the following to competencies@metrotrains.com.au no later than 10-days before your competence is due to expire: a. The name, RIW Number, role competency and expiry date for each worker, b. A statutory declaration confirming how many occasions over the last 3-months each worker has been practicing their role on the MTM network, and c. The confirmation email from the Platform that each worker has been booked into the course. Note. The request and statutory declaration can be for multiple workers. 3. On review, if your extension is successful MTM will upload an extension letter into your RIW profile with the date set to 10 days past course completion. Note. Extensions will be granted at the highest level of competency that you are required to extend, given your evidence supports the extension level requested. This will automatically extend the subordinate levels. MTM competency extension will not be accepted for other rail operators. 4. Within 48hours of course completion, you will be able to access your certificate of completion within the Platform. This will need to be uploaded into your RIW for verification. Engcon Sleeper Grabber Failure Background On Sunday 13 May 2018 a worker was injured when a Sleeper Grabber attachment, commonly known as an Octopus, disengaged from the Engcon unit. The worker suffered serious personal injuries and is recovering. The incident is still under ICAM investigation, however the following two causal factors have been identified: 1. The incompatibility of the excavator mounting pins with the Engcon Quick Hitch Unit. 2. Plant and people exclusion zones. Please see attached contractor s Safety Alert for more information. L4 SQE FOR 068(2) Immediate Actions 1. For all plant currently in use on site, ensure all plant attachments are compatible with Engcon Quick Hitch Unit size, and verify with manufacture or suppliers. 2. Managers in charge of work are to advise contractors to undertake assessment of compatibility prior to future use. 3. For all works supervisors to reinforce the adherence to plant and people exclusion zones, and that no worker should be in the proximity of a suspended load. SAFEWORKING NOTICES PERSONAL ISSUE OF MASTER KEY Permission is granted to Mr D White, for the personal issue of an extended period of a Master Key applicable to the Bendigo - Deniliquin Train Order Corridor, by the Signaller at Echuca from Friday 25th May 2018 to Monday 28th May 2018. Permission is granted for the Master Key applicable to Bendigo - Deniliquin Corridor to be issued for these works to be utilised on the Seymour Tocumwal Corridor. Mr White will be personally responsible for the security of the Master Key during the period it is issued to him. Prior to the initial issue of the Master Key, permission must be obtained from the Train Controller at Centrol. The Signaller at Echuca must complete an entry in the Train Register Book stating that the number of the Master Key that has been issued. Both Mr White and the Signaller must then sign the entry. Prior to the operation of any Master Key locked points, Mr. White must advise and obtain authority from the Train Controller, Centrol. In addition, Mr. White must endorse the Track Warrant form, Occupation form, or Track Protection Record Book (Depending on the method of track protection the Track Machines will be working under). When the Master Key is returned, the Signaller at Echuca must complete an entry in the Train Register Book to this effect. Both Mr White and the Signaller must then sign the entry. The Train Controller is to endorse the Master Key Log of the above details. The Master Key will be used for locking-away of Track Machine s at Toolamba during Track Works being conducted between Toolamba and Echuca.
