INSTRUCTIONS FOR COMPLIANT UNITS The following instructions are to assist units and attendees with completing the Convention Registration Form. Credentials must be submitted to the National Office for each name listed. If there are any questions, you may contact the NAACP Finance Department at 410.580.5712. This form, the original, signed credentials and all monies owed must be mailed together. The order must be postmarked no later than May 20, 2013, all additional orders must be placed on-site. Section I CONTACT INFORMATION The contact name should be the person responsible for retrieving the Convention Packet. All information should be completed in this section. Please note: only the person listed as the contact will be able to retrieve the packet. Portions of packets will NOT be disbursed nor will a different individual be allowed to retrieve a packet. Section II UNIT FEES Refer to the NAACP Constitution and By-Laws for Units (Articles 3,, Section 4(a)(1) for details regarding submission of Fundraising Assessments. Only Units that intend on registering delegates are required to pay Convention Assessments. NO PacKages ACCEPTED AFTER MAY 20th. There are three packages offered: Labor Luncheon, Membership Luncheon and W.I.N. Luncheon. These packages include the following: one adult convention registration, one Gospel Explosion ticket, one Freedom Fund/Spingarn Dinner ticket, and one ticket for the package chosen. Be sure that events for the tickets that are purchased do not conflict. Write the total number of packages and the total cost of this section in the spaces provided. Please note: When a package is selected, unless you need additional tickets, do not indicate any tickets in Section V. Also, unless additional registrations are needed, do not indicate any information in Section IV. SECTION IV: REGISTRATION In this section, please indicate the number of adult and youth registrations. The names of the attendees must be provided on the registrant listing. ALL REGISTRANTS PURCHASING DAILY PASSES ARE OBSERVERS. Please note: Do not include registration fees for package orders. SECTION V: TICKET ORDERS This section lists all the Convention s ticketed functions. All pre-ordered tickets must be retrieved at the Convention Center in the Advanced Registration window. No tickets will be mailed. Please note: If you have requested a package (Section III), do not indicate any tickets in this section unless there are ADDITIONAL tickets needed beyond what your package provides. SECTION VI: TOTALS The purpose of Section VI is to tally the grand total of monies owed for the entire order. Add the total monies from all sections and insert that amount into the space labeled Order Form Total. This is the amount to be remitted to the NAACP FINANCE DEPartment with your completed registration. ***PAYMENT METHODS*** We accept Visa, MasterCard, American Express, Discover as well as personal, business and/or Unit checks as payment. Also, the cardholder must print and sign his/her name in the spaces provided. If the cardholder s signature is not provided, payment and registration will not be processed. ***REFUND POLICY*** There will be NO refunds distributed during the Annual Convention. All refunds will be returned within 4-6 weeks following the Convention. The only refunds allowed are: 1. Tickets returned prior to 48 hours before the scheduled event. 2. Overpayment by a Unit Refunds will only be considered upon presentation of a valid original receipt. There are no refunds for advance registration purchases. ***CANCELLATION POLICY*** Written cancellation requests received, postmarked and mailed to the NAACP National Office by May 31, 2013 will be fully refunded.
