D.1 Frmal Request t Add an Aircraft The fllwing letters and frms shuld be cmpleted as early in the prcess as pssible s that the apprpriate resurces can be engaged in a timely manner. Frmal Request Letter: This is a frmal request letter n cmpany statinery frm the Directr f Operatins stating that the cmpany wuld like t add a specific aircraft t the certificate. The letter shuld include the fllwing infrmatin: Aircraft Registratin (US registratin required) Aircraft Serial Number Number f Passenger Seats Owner f the Aircraft Schedule f Events. Aircraft Cnfrmity Checklist: The aircraft cnfrmity checklist must be cmpleted in full and submitted t the Principal Maintenance Inspectr. This infrmatin shuld be prvided as early as pssible because the data is used by the t prepare fr the cnfrmity inspectin and t prepare the paperwrk t add the aircraft t the certificate. Cmmn errrs include incmplete r incrrect data. The checklist template can be btained frm the Principal Maintenance Inspectr. Envirnmental Impact Study Wrksheet: This wrksheet is abut three pages, is simple, and shuld als be submitted as sn as pssible. It takes abut tw weeks t get the results back, which must be received prir t adding the aircraft t the certificate. The FSDO Aircraft Cnfrmity Checklist will usually include a cpy f this wrksheet. Frms OST 4507: The DOT frm 298 / OST Frm 4507 shuld be submitted t the Department f Transprtatin. The submissin shuld be made (a) in duplicate, (b) 30 days prir t perating, and (c) include the fllwing: Cmpleted Frm Insurance frm 6410 (cmpleted by insurance cmpany). The DOT will return ne f the stamped/apprved frms. A cpy f the apprved frm shuld be prvided t the Certificate Hlding District Office (CHDO) Principal Operatins Inspectr (POI). D.2 Manual Submissin The fllwing manuals typically take a few weeks t prepare and shuld be started early in the cnfrmity prcess. Minimum Equipment List (MEL): The MEL, Nn-essential Equipment and Furnishings (NEF), and the Operatins and Maintenance (O&M) Prcedures must be dcumented and submitted. The MEL will be apprved upn cmpletin f their review. An external review prir t submissin will favrably impact the review and schedule. The dcument needs t cmply with the mst recent Master Minimum Equipment List (MMEL) and include the MMEL Plicy Letters applicable t the aircraft. The MEL must include a crrect value fr number f each item(s) installed. MEL prcedures must be specific and cannt include the phrase as required by FAR.

The MEL shall cntain required changes f the RVSM as stated in the MMEL. Prvide dcumentatin and recrds fr each cntract instructr and cntract check pilt that cnducts training, testing, r checking has been trained, qualified, and authrized t cnduct the apprpriate training, testing, and checking in accrdance with the applicable FARs and apprved Cmpany training prgram. Prvide dcumentatin that describes the Cmpany aircraft cnfiguratin(s) and -apprved prcedures are effectively supprted by the cntract training rganizatin s equipment, training, testing and checking. Differences between the Cmpany equipment and the training rganizatin s equipment must be addressed by cnducting apprpriate differences training. Cnduct a standardizatin review and prvide the results t the Principal Operatins Inspectr. Cnduct an initial and recurring audit f the training facility. Each audit must include an evaluatin f the items listed abve, including an in-persn evaluatin f actual training, testing, and checking being cnducted by the training rganizatin. The first audit must be cmpleted and submitted t the POI within 60 days f the cmmencement f cntract training, test, and/r checking.

The date f the mst recent audit must be entered int Table 1 f Operatins Specificatins A-031. Facilitate access by the cntract training facility t the Cmpany aircraft fr the purpse f maintaining their line-perfrmance/line-bservatin currency as cntract instructrs and check airmen. The applicatin fr apprval t utsurce training must be submitted a minimum f 60 days prir t the prpsed training and cntain the fllwing infrmatin: A cpy f the standardizatin review including an analysis f the training prviders curriculums, curseware, prcedures, equipment, facilities, and persnnel that will be used in the cnduct f the peratr s training; A detailed utline, by regulatry reference, f the training elements prpsed t be utsurced; If center persnnel will be used as cntract instructrs t cnduct the USAC Airways 694 LLC training, the applicatin must cntain an apprpriate training mdule develped t qualify centers instructrs/tces n the peratrs curriculum; If center persnnel are being requested t act as cntract check airmen, the applicatin must cntain an apprpriate training mdule develped t qualify centers instructrs/tces as cntract check airman fr the peratr. A cpy f the USAC Airways 694 LLC instructr/check airman standardizatin prgram; A cpy f the USAC Airways 694 LLC prpsed surveillance plan t ensure the center cntinues t prvide the agreed training; Prpsed methd t maintain required crewmember, cntract instructrs, and cntract check airman training recrds including the methdlgy prpsed t ensure curriculum revisins and an apprpriate instructr/check airman read file are maintained; Other data that the POI may require t evaluate the applicatin. D.3 Pilt Recrds Pilt Recrds: Pilt resumes must be submitted fr a minimum number f pilts as specified by the AFM and the air carrier certificate. Alng with the resumes, evidence f training and qualificatin must be submitted. The fllwing dcuments are required: Pilt Resume Pilt Certificate and Medical Pilt Training Recrds and Certificate Check ride frm, if applicable. The pilt must cmply with the USAC Airways 694 LLC hiring plicy, GOM requirements, and PRIA Manual. D.4 Dry Lease Agreement Dry Lease Agreement: If the aircraft is wned by a legal entity ther than USAC Airways 694 LLC, a cpy f the lease agreement with the wner shuld be submitted t the POI. D.5 Maintenance Recrds Review The fllwing data shuld be printed (paper r electrnic), dated, and saved as a recrd f cmpliance. It shuld be cnsidered an aircraft recrd fr the perid f time that the aircraft is n the air carrier certificate. Maintenance recrds shall shw that the all maintenance and preventive maintenance is up t date. A cpy shuld be submitted t the PMI fr review. Inspectins Status (CAMP): Prvide a printut f the current inspectin status. A printut frm the cmputerized maintenance prgram listing the status f each required inspectin. Be sure t check the fllwing: The inspectin items frm the apprpriate sectin f the maintenance manual (r AAIP) have been entered int CAMP. There is a crrespnding lgbk entry fr the phase inspectins and wrk assciated with life limited cmpnents. Incmplete r inaccurate data must be crrected and will nt be acceptable. Ntes frm previus USAC Airways 694 LLC cnfrmity reviews: The aircraft recrds can best be checked by first ging t CAMP. Fr each item listed, find the crrespnding maintenance lgbk entry. This verifies the data in CAMP as being accurate and prvides an pprtunity t verify that each lgbk entry is prperly written up. Begin the review with the majr inspectins and life limited cmpnents. The lgbk entry shuld have a descriptin f the wrk perfrmed, return t service, date, flight time, signature, and certificate number. Perfrm this review fr each and every entry in CAMP. The will lk at all majr inspectins and majr life limited cmpnents. They will then d a spt check f the remaining items including AD s. If they find ne incrrect time r ne imprper lgbk entry then the cnfrmity inspectin may be terminated. Alternatively, the may lk deeply int the detailed recrds lking fr a systemic prblem. If there are serius items that are incrrect r missing, the culd grund the aircraft until the prblem is reslved. A well prepared cnfrmity inspectin shuld take the team abut 2 hurs t cmplete their review. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Airwrthiness Directives: Prvide a printut f the recurring airwrthiness directives and their status. Airwrthiness Directives An AD list summarizing the AD number, subject, date cmplied with, and the persn signing it ff; r, a new set f AD s signed ff. Be sure t check the fllwing: AD s are prperly signed ff and a lgbk entry has been made. AD s that are due in the future are prperly entered int CAMP. Recurring AD s alng with their interval are prperly entered int CAMP. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Majr Alteratins and Repairs ( Frm 337): Prvide a list and cpy f all the 337 s and STC s fr the aircraft. Be sure t check the fllwing:

Revisin: Original Page D-6 Each majr alteratin has a lgbk entry. Instructins fr cntinued airwrthiness are addressed. Included in the Equipment List. Addressed in the Weight and Balance, as apprpriate. Assciated inspectins are included in CAMP. A missing lgbk entry is nt acceptable. Incrrect listing in the Equipment List is nt acceptable. It is a gd idea t request a cpy f all 337 frms that are filed with the in Oklahma City. Cmpare this list t the 337 s t ensure that the list f 337 s is cmplete. Review all 337 s fr any Instructins fr Cntinued Airwrthiness (ICA). If an ICA applies t the aircraft, ensure that the aircraft is in cmpliance with the limits. Dcument the methd in which cmpliance will be assured. If the 337 frm required a Field Apprval by the, make sure that the frm has been signed as required. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Equipment List: A current and accurate equipment list must be abard the aircraft. A cpy f the equipment list must be prvided in the cnfrmity recrd. Additins t the equipment list must have an accmpanying 337 and lgbk entry, as apprpriate. The weight and balance must reflect the added equipment. Check specifically fr the fllwing: Each additin t the equipment list has a 337 and lgbk entry. Instructins fr cntinued airwrthiness are in the aircraft maintenance manual. Additinal inspectins assciated with cntinued airwrthiness are listed in the manufacturer s maintenance manual and included in CAMP. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Weight and Balance: A current weight and balance has been perfrmed with a cpy in the maintenance recrds and a cpy in the aircraft flight manual kept abard the aircraft. All previus weight and balance recrds must be prvided. Check specifically fr the fllwing: A current weight and balance is abard the aircraft and in the maintenance recrds. A scale calibratin in additin t the fllwing: Type and serial number f each scale used; Airplane weight, residual fluids; and Scale tare weights. Lk n the W&B frm t make sure the aircraft was weighed with the calibrated scales. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. This includes verifying, dcumenting, tracking, and mnitring the fllwing weights: Empty weight. Maximum zer fuel weight. Maximum structural weight. Maximum taxi r ramp weight. Maximum takeff weight. Maximum landing weight. Date f last weighing.

Revisin: Original Page D-7 Fire Blck Certificatin Paperwrk: Prvide the paperwrk dcumenting that the materials used in refurbishing the interir has passed DOT requirements fr fire blcking. Be sure the check the fllwing: The materials used t refurbish the interir has been DOT tested and certified. There is a lgbk entry fr each interir change. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Engineering Orders: Prvide a cpy f any engineering rders assciated with maintenance perfrmed n the aircraft. Life Limited Cmpnents: Prvide a printut f a summary f life limited cmpnents. A printut f the status f all time limited cmpnents identified in the manufacturer s maintenance manual. Be sure t check the fllwing: There is a time limited cmpnent list and the times are current. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. RVSM Manual and Maintenance Prgram: Submit the aircraft RVSM Manual and Maintenance Prgram. Be sure t check the fllwing: The prcedures are applicable t the Cmpany. The MEL incrprates all aspects and any changes in the RVSM manual. A brief statement shuld be included at the frnt f this data frm the DOM certifying that the data is accurate and FAR cmpliant. Time Limits Manual: The life limits that are nt in Chapter 5 f the Aircraft Maintenance Manual are listed in Sectin 9 f the GMM. In lieu f this, a separate Time Limits Manual can be created and apprved separately (it typically wuld cntain nly a few items). In rare instances, an AAIP wuld be required. Special Emphasis Items: A review f the maintenance recrds t determine cmpliance with the apprpriate sectins f FAR 91 (paragraphs 403, 405, 407, 409, 411, 413, and 417). Bridging dcument and CAMP manual that includes wrk cards, time limits, etc. The MRB and MPD fr the aircraft, if applicable. List f passenger accmmdatins fr the interir. Additinal diagrams that might include lcatin f emergency equipment, if nt n LOPA. Skin mapping and repair assessment, if applicable. Recrds required by FAR 135.439. Flight Attendant Operatins Manual, if applicable. D.6 Physical Cnfrmity Inspectin Review the placards and check all serial numbers. The passenger briefing cards, required manuals, life preservers, life raft, flares, first aid kit, and ther required items must be abard. The mst cmmn prblems are missing items r items with an expired due date. Make sure that a scale is abard with a calibratin sticker, which is necessary t weight baggage that is expected t be ver the average standard weight fr a heavy bag. Briefing Cards: Nte that there are specific requirements fr briefing cards t include emergency equipment and instructins fr selectin and peratin regarding emergency rw seating and exits. There must be ne briefing card fr each seat. See AC 121-24C, Appendix 3.
