Indian Valley Boys & Girls Club 2018 Summer Camp Policies & Procedures Form GENERAL INFORMATION: CAMP AGES 6 TO 13 YEARS OLD COUNSELORS IN TRAINING 14 TO 18 YEARS OLD (MUST APPLY PRIOR TO REGISTERING) EACH FULL WEEK IS $110 - $75/ FOR 3 DAYS AND $50/ FOR 2 DAY PER CHILD CASH AND CHECKS ARE THE ACCEPTED FORMS OF PAYMENT IF REGISTERING DURING THE SUMMER, THE LAST TWO WEEKS OF CAMP (TRIPS INCLUDED) MUST BE PAID IN CASH. CAMPERS MAY BE DROPPED OFF AS EARLY AS 7:30 AM AND CAN STAY AS LATE AS 6:30 PM WITHOUT ANY ADDITIONAL CHARGE YOU WILL BE REQUIRED TO IMMEDIATELY PICK UP YOUR CHILD IF HE/SHE IS DROPPED OFF AND IS NOT REGISTERED AND PAID IN FULL PRIOR TO THE START OF THAT WEEK CAMPERS MUST BE DROPPED OFF BY 9:00 AM. CAMPERS ARRIVING AFTER 9:00 AM WILL NOT BE ALLOWED TO ATTEND FOR THE DAY CAMPERS MUST BE PICKED UP BY 6:30 PM UNDER NO CIRCUMSTANCES ARE CHILDREN PERMITTED TO BE DROPPED OFF AND LEFT UNATTENDED OUTSIDE OF THE BUILDING PRIOR TO 7:30 AM CAMPERS MAY BE EXPELLED FROM CAMP DUE TO EXCESSIVE LATE ARRIVALS OR PICK-UPS. LATE FEES MAY BE APPLIED. PARENTS MUST COME INTO THE BUILDING TO SIGN THEIR CHILD IN AT THE FRONT COUNTER. AT THE END OF THE DAY, PARENTS MUST RE-ENTER THE BUILDING AND SIGN THEIR CHILD OUT. WE UNDERSTAND THIS MAY BE AN INCONVENIENCE, BUT WE ASSURE YOU THIS PROCEDURE HAS BEEN ADDED FOR THE SAFETY AND PROTECTION OF YOUR CHILDREN. NO EXCEPTIONS WILL BE MADE IN REGARD TO THIS RULE UNLESS THERE IS A SERIOUS MEDICAL CONDITION AFFECTING THE PARENT/GUARDIAN. ANYONE PICKING UP A CHILD FROM CAMP MUST SHOW A VALID DRIVER S LICENSE AND BE DESIGNATED AS A PICK UP PERSON ON THE REGISTRATION FORM. IF SOMEONE OTHER THAN A PARENT OR GUARDIAN IS PICKING UP YOUR CHILD, PLEASE NOTIFY THE DIRECTOR OR FRONT DESK SUPERVISOR. INCLUDE THE NAME OF THE PERSON AUTHORIZED TO PICK UP YOUR CHILD AND THE TIME THEY WILL BE PICKED UP ALL AUTHORIZED PICK UP PEOPLE MUST BE ADDED IN PERSON BY THE LEGAL GUARDIAN. PHONE CALLS, EMAILS OR FAXES WILL NOT BE ACCEPTED NO PARENTS ARE PERMITTED PAST THE FRONT DESK AND ARE RESTRICTED TO THE FRONT LOBBY AREA. 5

ALL SIGN UPS FOR FIELD TRIPS OPEN ON MONDAY OF THE WEEK THEY ARE HELD. FIELD TRIPS AND POOL TRIPS HAVE LIMITED SPACES, HAVE AGE RESTRICTIONS AND ARE ON A FIRST COME, FIRST SERVE BASIS. PHONE REQUESTS FOR ALL TRIPS ARE NOT ACCEPTED. TRIP FEES AND POOL FEES ARE NON-REFUNDABLE UNLESS THE TRIP IS CANCELLED BY THE CLUB. MONEY WILL BE REFUNDED IN A TIMELY MANNER IN THE EVENT OF A CANCELLATION CHILDREN ARE PERMITTED TO BRING SPENDING MONEY ON POOL AND FIELD TRIPS. HOWEVER, THEY ARE RESPONSIBLE FOR THEIR FUNDS AS WELL AS ALL PERSONAL BELONGINGS. ALL CAMPERS MUST FOLLOW RULES OUTLINED AT TRIP VENUES. FAILURE TO DO SO WILL RESULT IN LOSS OF TRIP PRIVILEGES AND/OR SUSPENSION FROM THE CLUB. POOL TRIPS MAY BE CANCELLED AT THE DIRECTOR S DISCRETION. IF A TRIP IS CANCELLED, MONEY WILL BE REFUNDED IN A TIMELY MANNER. CAMPERS MUST WEAR SNEAKERS OR SHOES THAT TIE, IN ORDER TO PARTICIPATE IN GYM ACTIVITIES. BOOTS/UGGS, SANDALS, FLIP- FLOPS, CROCS AND OTHER EQUIVALENTS WILL BE NOT PERMITTED IN THE GYM. CAMPERS MUST TURN IN ALL MONEY TO THE FRONT DESK EVERY MORNING. EACH CAMPER WILL HAVE A DESIGNATED ENVELOPE WHICH WILL HOLD THEIR MONEY. WE DO NOT PROVIDE CHANGE FOR MACHINES OR REFUND MONEY LOST IN MACHINES. CAMPERS MUST BRING THEIR MEMBERSHIP CARD EVERY DAY. CARDS ARE TO BE SWIPED AT THE FRONT COUNTER UPON ARRIVAL AND UPON LEAVING THE CLUB. CAMPERS WHO FORGET THEIR CARDS REPEATEDLY MAY HAVE THEIR MEMBERSHIP SUSPENDED. STAFF WILL DISTRIBUTE MEDICATIONS TO THE BEST OF THEIR ABILITY. STAFF WILL NOT BE HELD ACCOUNTABLE FOR MISSED DOSES. MEDS FOR THE WEEK SHOULD BE BROUGHT IN EVERY MONDAY AND GIVEN TO THE FRONT DESK SUPERVISOR. PARENTS MUST FILL OUT THE SHEET AT THE FRONT DESK. IF A CHILD IS NOT ATTENDING CAMP, ANOTHER CHILD MAY NOT TAKE HIS OR HER PLACE IF A CAMPER WOULD LIKE TO BRING A GUEST FOR A DAY, THERE IS A $30.00 FEE. ALL REQUESTS TO BRING A GUEST TO CAMP MUST BE MADE AT LEAST TWO DAYS IN ADVANCE AND CLEARED BY THE DIRECTOR. THE DIRECTOR MAY DENY THE REQUEST TO BRING A GUEST. GUESTS ARE NOT PERMITTED ON FIELD TRIPS OR POOL TRIPS. CAMPERS MAY BE TEMPORARILY OR PERMANENTLY SUSPENDED FROM CAMP AT THE DISCRETION OF THE DIRECTOR FOR ANY BEHAVIOR CONSIDERED INAPPROPRIATE OR UNACCEPTABLE. IF CAMPERS ARE SUSPENDED DURING THE CAMP DAY, A PARENT WILL BE CALLED IMMEDIATELY TO PICK THEIR CHILD UP FROM CAMP. FAILURE TO DO SO MAY RESULT IN ADDITIONAL DAYS ADDED TO THE SUSPENSION. CELL PHONES AND ELECTRONICS ARE NOT PERMITTED AT THE CLUB DURING CAMP HOURS. THE CLUB IS NOT RESPONSIBLE FOR ANY LOST, DAMAGED OR STOLEN ELECTRONICS OR CELL PHONES. BRINGING PERSONAL ITEMS OR VALUABLES TO THE CLUB IS DISCOURAGED. THE CLUB IS NOT RESPONSIBLE FOR ANY LOST, DAMAGED OR STOLEN ITEMS. 6
