Hot Meals delivered to your door 1 Main Course + 1 dessert. Meals delivered every day. A big menu for you to choose from. It costs 4.75 Free welfare check making sure you are okay! Tea packs delivered with your Hot Meal 1 Sandwich or Cream Tea + 1 Snack + 1 Dessert Chilled food for the evening meal. A big choice of snacks and sandwiches. It costs 3.25 1

Breakfast Packs delivered with your Hot Meal 1 Cereal or Porridge + 1 Pastry/Snack + Drink Ready for the next day. It costs 2.75 Hot food for Groups + 1 Main Course + 1 Dessert Good for eating with friends We can deliver to lunch clubs A big menu with lots of choice It costs 4.55 per person 2

Medication prompts with your Hot Meals Someone can remind you to take your medicine. This is free. Ask a health or social care professional to refer you. Emergency Food and Grocery Packs These are bags of food for when you come back from hospital. They can be delivered to your home, to a friend, or to the hospital reception. You can ask for fresh items too. These bags are free. Ask your social worker or hospital staff to refer you. 3

Helping you feel safe in your home Community Alarms and Telecare This is technology that helps you to live safely at home and contact people if you need to. HILS can put alarms in your home. If you have a problem you can press a button and call someone. The buttons are on pendants. You can wear pendants like a necklace or a bracelet. When you press the button, or fall over, it will call people at Careline who will help you. They can contact friends and family, the police, or the fire service for you. It costs 3.25 per week + VAT. Setting up is free. + HILS installs telecare + Press your pendant to speak to someone All day, everyday 4

Pop-in Visits Short, five minute visits to make sure you are okay. We can remind you to take medication or make you a drink It costs 3.50 per visit. Key Safes This is a strong metal box attached outside your house. You can keep a house key in it. The box locks with a code that you can choose. Give the code to people you trust and visit you often. Always check with your carer before giving out your code. There are 2 types of key safe, 1 costs 66 and 1 costs 90. If there is a problem with your key safe we can visit you to fix it. This costs 30. 5

Our Health and Wellbeing Services Nutrition and Wellbeing Our team can come to see you at home. They will check your weight and give you advice about what to eat. If you have allergies or need a special diet they will help you choose the right food. Dementia fun clubs + Groups of people with mild dementia do fun activities. Our friendly team do gentle sport, arts and crafts, games and music. Lunch is included in the price, everyone eats together. It costs 25 to join, and 25 per session. Please pay every month in advance. 6

Hertswise For people with dementia, memory problems, and their carers. There are support groups, activities and other services. Puts you in contact with specialist advisers to help you. Free Home Eye Testing Opticians will come to your home and check your eyesight. This is free for anyone over 60. They can advise you on the best eye treatment. 7

Our community services Advocacy This service puts you in contact with professional people. They can help you understand your health and care options. They can help you make good decisions. They can do research for you, answer letters, and attend meetings for you. Write letters and emails with you. They will listen to you and help you find what you need. If you want this, call HertsHelp on 0300 123 4044. Someone will visit you to see if you are eligible. 8

Jubilee Community Centre This is our Community Centre in St Albans. Lots of groups hire our rooms for different activities. There is a drop-in restaurant where you can have a hot Main Course + Dessert. Lunch costs 4.55. This is when the restaurant is open 9
