Before distributing surveys, BE SURE THE FOR RESEARCHER USE ONLY BOX ON THE SURVEY FORM IS FILLED OUT. 2. Fill out one log form for each individual site 3. Each group gets its own record number. 4. Record the number of children (under age 16) in groups contacted. 5. Record the total number of adults in the group. 6. Record the number who agree to complete the survey, the number who refuse, and the number who have previously completed the survey. Anyone who does not do the survey who heard your intro should be considered a refusal (unless there are obvious extenuating circumstances, like in the middle of cooking or taking care of a kid or they have previously completed the survey). 7. IMPORTANT Record the Survey Numbers of all windshield surveys left on vehicles. 8. Record any relevant comments on the back of this form, noting group # (e.g., about activity, comments, or unusual situations). 9.
