February 29, 2016 Dear Pilot: Invitation It is my pleasure to invite you to participate in the Rally Division of the 2016 US National Hot Air Balloon Championship in. A prize fund of $2,000 is guaranteed along with a pole ring toss of $5000. Nationals competitors will also be eligible for the ring toss. The Red River Balloon Rally on-site pilot registration and the mandatory General Briefing is on Thursday July 14, 2016. Flying concludes Sunday morning, July 17. There will be a Rally Awards breakfast at 11 AM Sunday to conclude the event. The organizer is providing a complimentary hotel room for pilots with check in on Thursday, July 14 and check out Sunday, July 17. Arrive full; leave full. A tentative schedule of events is enclosed. The Red River Balloon Rally is open to experienced pilots comfortable with flying in congested areas. The event will be flying under an FAA Waiver. Fun competitive tasks that can be run in conjunction with the ring toss will be planned. The Rally Balloonmeister is Mr. Sean Askren. Application Deadline May 2, 2016 Participation in this year s event is limited to a maximum of fifteen pilots. We encourage you to submit your application early although that will not be the sole selection criteria. You will be notified by email of your acceptance no later than May 6 all applications must be received in the BFA Office on or before May 2, 2016. You can mail your application or complete the attached forms, scan and email it to the BFA office. Acceptance Your application will not be considered complete until we receive your completed application form, digital (JPG) photo of your balloon and yourself (head shot only), $100 entry fee payable to the BFA and a completed balloon related biography. The Red River Rally pilot list will be posted on the Shreveport website, Red River Rally Pilot List, no later than May 6, 2016. A waiting list will be maintained for all applications received beyond the fifteen slots available. Often plans change and last minute slots may be available. Insurance The Organizer and the BFA/HACD requires that each participating pilot name the sponsoring organizations, venues and others directly involved in the organization and sponsoring of this event as additionally insured on their balloon insurance policy. The City of Shreveport and the Sports Commission certificates must include a waiver of subrogation. We have reviewed this requirement with the Schantz Agency and RPS IMC Balloon Insurance and have been assured by those agencies that they have no issues with this requirement. While the additionally insured endorsement provides the named additionally insured no liability coverage, it does provide unlimited defense coverage should they be named in a law suit. We will provide a list of entities requiring additionally insured certificates that you will have to authorize. All pilot packs, tickets, maps, banners, etc. will be distributed at that time. FAA staff from the Baton Rouge FSDO will be present to examine pilot and balloon documents. Be sure you have a current balloon registration. 1

Corporate Balloons A Corporate Balloon is a balloon which carries advertising for a particular product, company, service, or identifiable artwork, logos, etc. Commercial entry fees are $1,500 and are payable to the BFA. Entry fees for commercial special shape balloons are negotiable. Please contact Maury Sullivan at 330.323.4455 for details concerning commercial special shape entries. Please do not contact the hotels directly until you have been accepted and assigned your hotel and notified through your acceptance email. It is imperative that you indicate on the Pilot Application the number of crew members you would like us to provide for each scheduled flight. If you are provided local crew, we ask that you provide them with the complimentary meal tickets that are given to you or purchase additional tickets as needed. Maps Each participating pilot is provided two official full color competition maps of the primary competition area. Maps will be available for mailing to pilots in advance should you choose. Exact cost of postage and handling is not currently known but should be around $10. This information will be made available when the approved pilot list is finalized. Airspace The majority of flight operations are within the Class C airspace of Shreveport Regional Airport (SHV) and Barksdale Air Force Base. Both facilities are fully cooperating with our event and operations personnel to ensure a safe event. In this regard, you are required to carry an operating aircraft radio and monitor frequency 121.4. Due to the sensitive nature of Barksdale Air Force Base all pilots will be asked to fly with the use of a Garmin Dakota 10 logger provided by the organizer. HOTEL ACCOMMODATIONS Pilots receive one (1) complimentary room for July 14-17, 2016 (Sunday check out). When final acceptances are sent you will be advised of your hotel. PILOT ENTRY AGREEMENT Including Acknowledgement of Responsibility and Release of Liability I hereby declare that I am participating in the Red River Balloon Rally (Rally) on of my own free volition. I agree that the organizers and sponsors of this event (including the Shreveport-Bossier Convention and Tourist Bureau and the Shreveport-Bossier Sports Commission, the cities of Bossier City and Shreveport and Caddo Parish Commission, State Fair of Louisiana, Bossier and DeSoto Parish Police Jury, Cumulus Broadcasting LLC, LSU Shreveport, Balloon Federation of America, event race officials, my sponsor or any subsidiaries, employees, officers, trustees, agents and/or members of the above mentioned entities) are providing me with the facilities and means for my participation in this event and in no way do they supersede the responsibility of the pilot in command, as shown in CFR 91.3(a) and elsewhere in the Federal Aviation Regulations (FAR s). I hereby certify that I am aware of and agree with the responsibility legally transferred to me under the Federal Aviation Regulations (FAR s) as regards my personal decision to fly my balloon, and any bodily injury or property damage resulting from there is solely my liability, and that my decision as pilot in command may affect my placement in this event if I choose not to fly. I understand that the pilot of each balloon has full and complete authority over the balloon and is responsible for all decisions to be made associated with the balloon on the ground and in the air. I certify that I am of sound mind and body and I have no medical, mental, or physical limitations that would limit my ability to safely fly my aircraft. I am not taking prescription or over the counter drugs that may impair my ability to safely fly my aircraft. Officials of the US National Hot Air Balloon Rally may supply information, guidance, or data and will use reasonable care to ensure the information is accurate and complete. However, the BFA/HACD and Rally assumes no responsibility for the completeness or accuracy of such information. It is my decision whether to rely on that information or acquire additional information. I agree that none of the flights are mandatory and that I will make all flights at my option. I hereby agree to have in full force during the Rally a balloon insurance policy with minimum liability limits of $1,000,000 each occurrence combined bodily injury and property damage, including passengers, but not less than $100,000 each passenger. I hereby give permission, without any restriction, to the Rally or its assigns, to use still and motion pictures and sound recordings of me, my balloon, equipment, chase crewmembers, employees and agents for news, advertising, promotional media and commercial uses. It is my understanding that the organizers may inspect all balloons entered in the event; but that such inspection is only for the purpose of determining whether the aircraft complies with all rules of eligibility of said event. The approval of the balloon by said officials does not constitute a representation or warranty of any kind or character whatsoever concerning the mechanical condition of the aircraft, or whether or not it is airworthy. I acknowledge that the Rally utilizes GPS loggers for purposes of scoring and monitoring participant s flights for adherence to event regulations regarding landowner relations and Prohibited Zone infractions. I agree to carry this GPS logger on all flights and conform to all rules related to its use. I also acknowledge financial responsibility for the device while under my control. I agree that this Agreement is a Louisiana contract and it is to be governed by the laws of the State of Louisiana, and that I hereby submit myself to the jurisdiction of the Louisiana courts concerning any claims arising out of the Rally or this Agreement. This Agreement, including the Conditions of Entry, General Information, Insurance and Hotel Reservation form, contains all the terms and conditions agreed to by the parties hereto, and no other agreements or discussions, oral or written, regarding the subject matter of the Agreement shall be deemed to exist or bind any of the parties hereto. I, the pilot in command, hereby declare that I will fly in accordance with the event rules and regulations, abide by the FAR s, and fly entirely at my own risk and waive any right of action against the Rally, the owner of the launch site, event race officials, my sponsor or any employees, officers, trustees, agents, and their respective members for any loss or damage sustained by me in consequence of any action or omission on their part, my part or the part of other participants. Thus, I release the organizers and sponsors of this event (including the Shreveport-Bossier Convention and Tourist Bureau and the Shreveport-Bossier Sports Commission, the cities of Bossier City and Shreveport and Caddo Parish Commission, State Fair of Louisiana, Bossier and DeSoto Parish Police Jury, Cumulus Broadcasting LLC, LSU Shreveport, Balloon Federation of America, event race officials, my sponsor or any subsidiaries, employees, officers, trustees, agents and/or members of the above mentioned entities) from liability for their actions or inactions relating to the Rally which may arise out of or result from or in any manner be connected with the balloon flight or activity in which I participate as a pilot in command.
