April 2014 2 nd Floor Lakeside Offices University Hospital Wales Dear Colleague, The Cardiff and Vale University Local Health Board is continuing to operate a scheme assisting employees of the UHB to purchase Annual To Go, or To Go Plus tickets for travel to and from work. This scheme enables employees to obtain an annual ticket and make payment via UHB payroll, and is run in association with Cardiff Bus. You will be issued with an iff card which will be loaded with either an annual Cardiff To Go, or Cardiff To Go Plus ticket. You pay for the ticket by weekly or monthly instalments deducted via payroll. If you already have an iff card this can be swapped over onto the UHB scheme please complete the iff card number section on the application form. The cost of the current bus pass/annual tickets are attached indicating both the types of ticket and prices. Please select which you require on the application form and return to the Transport Office. Remember, the use of your iff card is NOT restricted to and from work, it can be used at any time - so the more you use it the greater the value you get from it! The iff card may also be used by your family and friends when you are not using it. You can also use the e-purse facility to upgrade your journeys/add additional credit to your card, should you wish to travel outside the zones your ticket covers. Please note: Should your details change, please contact the Transport Dept. in writing so we can update our records If you wish to leave the scheme please contact the Transport Dept. either in writing, by email or by telephone detailing your wish to withdraw and to cancel payments. Frequently Asked Questions What happens if I terminate my employment with the UHB? You must inform the Transport and Sustainable Travel Dept, 2nd Floor, Lakeside Offices, UHW, immediately on or before termination. Your card will then be de-activated by Cardiff Bus What happens if I lose my card, it is stolen or damaged? As soon as possible, contact the Transport Office. We contact the Sales Office at Cardiff Bus, the card is stopped, a replacement issued and the balance transferred. Please ensure you complete the security questions on the application form to get this benefit. The Transport Office at UHW will inform you when your replacement card is ready; you then collect the replacement from the Cardiff Bus Customer Service Office, Wood Street. You must take your UHB ID Badge with you as proof of your employment, and ensure you remember your answers to the security question. The first replacement card is free, subsequent ones will incur an administration fee. Lost cards may be handed - please contact Cardiff Bus either on 02920 666 444 or in person at Head Office, Sloper Road, Leckwith, Cardiff CF11 8TB (routes 1, 2, 92, 93, 94 stop close by) to see if it has been handed in before applying for a replacement. I will be of State Retirement Pension Age during this period of the ticket and will be remaining with the UHB do I still pay full rate? If you are a resident of Cardiff (including Pentyrch and Creigiau) and fall within Cardiff Council s categories you are entitled to a free bus pass. Remember, you must return your iff card to the Transport Office upon receiving your free pass. This ensures your deductions are cancelled from your wages as soon as possible. I am a Registered Disabled Person- do I still have to pay the full rate? Not necessarily - it will depend on whether your disability conforms to the criteria laid down in the Transport Act, and your local Council. Address: 2 nd Floor, Lakeside Offices, UHW Please note: Monthly Payroll monitoring is undertaken and failure to notify the Transport Office that you no longer work in the UHB, are on a career break, have transferred out of the UHB to another service or are to be in receipt of a free bus pass, will result in your UHB scheme iff card being de-activated and hot listed by Cardiff Bus. You must therefore inform us of any changes in your work situation as soon as possible. 3
