Thank you for your registration! Please take a moment to review the enclosed information. If you have any questions or concerns, feel free to contact the spcala for information at (310) 676-1149 X228 or email Humane@spcaLA.com. LOCATION: 7700 E. Spring St, Long Beach, 90815 Enclosed is a map with directions. EXPECTATIONS OF CAMPERS: We expect campers to show respect and kindness toward animals, camp counselors and other campers at all times. Please discuss this with your child before coming to camp so that he/she is ready to participate upon arrival. Our staff will remind campers of the expectations if needed, and any repeated incidents may require a phone call to parents or removal from camp activities. PLEASE HAVE CAMPERS BRING DAILY: 1. A sack lunch. 2. A bottle of water or drink. 3. Snacks will be provided, but you are welcome to send a snack with your child, if they have special dietary needs. DRESS REQUIREMENTS: 1. Campers must wear long pants for dog training. Campers may wear shorts as long as they bring long pants to change into. Campers will not be allowed to dog train without long pants. Capris do not count as long pants. 2. Closed-toe shoes for dog training. 3. A hair tie, if the camper has long hair. 4. Hats and sunscreen are optional, but recommended. Campers will spend a lot of time outdoors.

DROPPING OFF: Camp runs from 9:00 a.m. to 3:00 p.m., Monday through Friday. Drop off time for students is 8:45 a.m. You may drop your child off at the Education Center of the spcala, which is located to the right as you drive towards the building from the parking lot. Enclosed are a map and directions to the Hawthorne facility. Please come into the building to sign in your child. PICKING UP: Please be sure to pick your child up promptly at 3:00 p.m., as you will be charged additional fees if your child remains after the pick up time. Also, if your child remains in our care beyond 3:30 p.m. without any contact from a parent/guardian, it will be considered abandonment, and the police will be called to escort your child home. Please come into the yard to sign your child out at the end of the day. CANCELLATION POLICY: Refunds will be provided if notice is given two weeks before the start date of the session in which your child is enrolled. There will be a 10% processing fee for any cancellation. MEDICATIONS/DISABILITIES: If your child is in need of medications of any sort, please notify the Director of Humane Education, Melanie Wagner, prior to your child attending camp. If your child has a disability, please inform the Director of Humane Education. Many disabilities can be accommodated, but there are a few that cannot. For the safety of the campers and the animals, please inform the Director of Humane Education of any disabilities, illnesses, or medications that could affect your child at camp. FROM 605 NORTH: Exit at Katella Ave./Willow St. Continue on Willow St. Turn right on Studebaker Rd. Turn right into El Dorado Park. FROM 405 SOUTH: Exit at Studebaker Rd. Turn left on Studebaker Rd. Turn right into El Dorado Park. When you enter El Dorado Park, tell the guard that you are visiting the spcala (there will be no charge for parking). Turn left immediately past the guard shack, and follow the paw prints on the pavement around to the right. The road curves to the left before you enter our parking lot. Camp will be held in the building marked Education Center toward the right side of the parking lot.

Parent s Copy of the spcala Friends for Life Camp Liability Waiver Keep this copy. Do not return it to the spcala. The Society for the Prevention of Cruelty to Animals of Los Angeles (spcala) conducts a children s camp program ( Camp Program ). During the Camp Program, participants will have direct contact with domestic animals. The Camp Program is conducted at the spcala animal shelter at at 7700 East Spring Street, Long Beach, CA 90815 ( spcala Location ). spcala takes reasonable care selecting shelter dogs to participate in the Camp Program. Nevertheless, there are potential risks inherent in having contact with animals and contact with animals is unpredictable, which could be dangerous and may result in injury or death. Nonetheless, I/we are voluntarily participating in the Camp Program and assume all risks associated with participating in the Camp Program. I/we agree that spcala may use and license others to use my/our name, voice, photograph, likeness and any biographical material for the licensing, marketing, advertising and promotion of the Camp Program in all markets and media, including the internet. In connection therewith, I/we consent to have photographs and videos, with or without audio, taken of me/us for use in such licensing, marketing, advertising and promotions. I/we agree, for myself/ourselves and any and all heirs, not to sue spcala and/or anyone related to the Camp Program, and not to initiate or assist in the prosecution of any claim for damages or any cause of action which I/we may have arising from or related to my/our participation in the Camp Program. I/we agree, for myself/ourselves and any and all heirs, to indemnify and hold harmless spcala, its agents, volunteers and employees, including but not limited to the dog trainers and Camp Program staff and the animals, from any and all liability, claims, actions, suits, causes of action, expenses, attorneys fees, and damages of whatever kind or nature, in law, equity or otherwise, whether now known or unknown, suspected or unsuspected, arising out of or in any way related to the Camp Program. I/we agree, for myself/ourselves and any and all heirs, to fully and forever release and discharge spcala, its agents, volunteers and employees, including but not limited to Camp Program staff and the animals ( Releasees ) from any and all liability, claims, actions, suits, causes of action, expenses, attorneys fees and damages of whatever kind or nature, in law, equity or otherwise, whether now known or unknown, suspected or unsuspected, arising out of or in any way related to the Camp Program. In furtherance of this intention, I/we hereby expressly waive any and all rights and benefits conferred by the provisions of SECTION 1542 OF THE CALIFORNIA CIVIL CODE WHICH PROVIDES: A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH THE CREDITOR DOES NOT KNOW OR SUSPECT TO EXIST IN HIS OR HER FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH IF KNOWN BY HIM OR HER MUST HAVE MATERIALLY AFFECTED HIS OR HER SETTLEMENT WITH THE DEBTOR. I/we give the spcala authority to seek emergency medical treatment for the participant named below. I/we know of no medical or other condition that would prevent the participant from participating in the Camp Program. I/we understand that during the Camp Program, participant may have opportunities to interact with a veterinarian and his/her staff. During these encounters, participant may have the opportunity to witness a sterilization surgery or may have the chance to see specimen jars that may contain deceased puppies or kittens that have been preserved for al purposes. Participant may leave the room at any time at their discretion. I/we understand that during Camp Program, participant may have opportunities to participate in field trips. Field trips may include visits to a zoo or aquarium. In the event of travel away from the spcala Location, participant will be transported from the spcala Location to the off-site location, and back, by Camp Program staff via an insured bus line.
