Warsaw, July 2013 Dear colleagues and friends, The Law Offices of RGW Rocławski Graczyk i Wspólnicy Adwokacka Sp. k. has the honour of inviting you to the CONSULEGIS Autumn Conference 2013, which will be held in our hometown of Warsaw, Poland. Warsaw, which has been the capital city of Poland for more than four centuries, is also the largest city in Poland. It is the national centre of business, political and cultural events. The symbol of the city is the Siren (mermaid) that can be seen on Warsaw s coat of arms. Warsaw is a vibrant metropolis and a city with unique history. It is a city where the green belt covers ¼ of its total area, with many scenic and spacious parks. We are confident that Warsaw will undoubtedly leave a lasting impression on our guests. Historic palaces and churches lie next to modern office buildings and skyscrapers, and sheltered cafes and trendy clubs resound with music. We will follow the romantic paths of Frederic Chopin s music, who, for the first half of his 39 years called Warsaw his. Here he was brought up and educated and here his heart found its final resting place in Holy Cross Church. Our gala dinner will be an unforgettable celebration in the ballroom of the Polonia Palace Hotel. This ballroom is unique in that it is the only hotel ballroom in Warsaw to have survived the Second World War-in fact, although the rest of the hotel was destroyed, the hotel was rebuilt around the ballroom, leaving this historic venue intact. We are sure that our Autumn Conference will be an opportunity for you to experience the unique atmosphere of Warsaw, a city of both triumphs and tragedies, and one which is the centre of the most vibrant and growing economies in Europe. CANCELLATION CONDITIONS AND GENERAL CONDITIONS ACCOMMODATION The rooms have been reserved for the CONSULEGIS Autumn conference and are subject to special conditions group sales. Please note individual cancellation conditions and policy on your separate Hotel confirmation! All Hotel reservations must be guaranteed by a credit and direct payment on the spot. Given the limited number of rooms and the fact that preferential rates cannot be guaranteed for more than the number of rooms reserved for the CONSULEGIS conference, we recommend that you will send us your reservations as soon as possible. Once the number of rooms reserved for the CONSULEGIS conference are sold out, or for any stay before or after these dates, CONSULEGIS will make a separate, specific request for availability in the hotel concerned (reservations will be made upon availability and subject to new rates). PRE AND POST-CONFERENCE PROGRAM/ACTIVITIES/TOURS If the number of registered participants is inferior to the minimum indicated, tours may be cancelled. The cancellation conditions for individual participants have been established as follows, entailing the payment of the following charges for the programs/dinners/tours: No cancellation fees for the tours will be charged up until October 4 th 2013 (10 o`clock). After that: 100 % will be charged. In case of no show for any tours you have registered for, no reimbursement shall be made. In order to avoid any reimbursement problems, we earnestly advise all those who need a visa to proceed with their registration as soon as possible. We can not accept cancellations over phone. GRAPHICAL MATERIAL Participants are aware that their image be photographed throughout the duration of the conference. They hereby assign to CONSULEGIS, with their signature of this form, the right to use the material within the official CONSULEGIS communication tools (Internet, Intranet, Newsletter, etc.) for an unlimited term and free of charge. 13
INSURANCE (at your own charge) If you are taking part in a conference program you have the obligation to acquire an assistance/repatriation insurance. With regard to your participation in the congress, we recommend that you purchase an insurance policy providing hospitalization and medical expenses coverage. For your information, the insurance policy may be included if you pay by credit card (please check with your bank to see the details about such coverage). CONSULEGIS accept no liability in case a congress participant fails to purchase such an insurance policy. GENERAL CONDITIONS CONSULEGIS reserve the right to cancel or postpone the congress to a later date, change the congress venue and/or program, make any corrections or modifications in the information published in the program and cancel any invitation to participate in the conference, at any time and at their entire discretion, without having to provide any reasons for the same. Neither CONSULEGIS nor any of its managers, employees, agents, members or representatives shall be held responsible for any loss or damage, of any nature, suffered (directly or indirectly) by a conference delegate, accompanying person or a third party following any cancellations, changes, postponements or modifications. FORMALITIES It is the responsibility of conference participants to ensure compliance with police, immigration, customs and health formalities for their journey. Congress participants unable to take part in the congress because of their inability to take a flight or any other means of transportation due to being unable to provide the documents required (passport, visa, vaccination certificate, etc.) cannot claim any reimbursement. HEALTH The organizers decline any responsibility in case of any health problems existing prior to the conference that may lead to complications or be aggravated during the entire period of the stay: pregnancy, cardio-vascular problems, any allergies, special diets, any disorders under treatment and not yet consolidated on the day the congress starts, psychic, mental or depressive illness, etc. (Non exhaustive list).
