Welcome to the St. Pius X Parish School Pirates and Princesses 2016 Summer Camp! There s something timeless about kids and summer camp. At camp, children get to experience the hot sun and cool water, the green trees and blue skies, the outdoor games and the indoor activities. Most of all there are the grinning faces of friends, old and new, to look forward to playing with each day. The mission of the St. Pius X Parish School Summer Camp 2016 is to offer a quality, safe, inclusive, and affordable summer experience for our students and families. Our goal is to provide lasting memories, imaginative play, and allow our campers to develop to their full potential. The St. Pius X Parish School Summer Camp is now in its sixth year! The Summer Camp 2016 will be offered to students currently registered for the 2016 2017 school year that will be attending our three year old preschool through fifth grade. We will also accept students who were in three year old preschool through fourth grade during the 2015 2016 school year. We offer full-time camp (from 7:00 a.m. to 6:00 p.m.), regular camp (from 9:00 a.m. to 4:00 p.m.) and part-time camp. Summer Camp will begin on Monday, June 6, 2016 and continue throughout the summer until Friday, August 12, 2016. Space is limited and will be filled on a first come, first serve basis. Students must be completely toilet trained to enter the camp program. Campers will be grouped according to age. Camp days chosen are on a permanent basis, and for staffing and purchasing reasons cannot be changed from week to week or month to month. However, camp days can be added based on space availability and camp attendance can be different each week. Camp Staff: Our Camp Co-Directors, Mrs. Jessica Jarosinski and Ms. Katelyn Kott, will oversee our camp staff and supervise the day to day activities and events of our camp. Camp Counselors will be responsible for the daily supervision of their age group of campers. They will implement the daily activities and are responsible for the supervision of the campers. The Camp Counselors will track daily attendance and have daily parent communication. Camp Counselors will be assisted by Counselors-in-Training based on daily attendance and field trips. Summer Camp will be, for the most part, located on the grounds of St. Pius X Parish School. We will be taking some of our outdoor activities to the park at Madison Meadows as well as field trips each week. For rainy days, we will utilize the Social Center and on the days that are extremely hot, we will use the rooms that have air conditioning. The camp teams will operate on a rotational basis to allow for maximum opportunities to interact and utilize the activities that have been planned each day.

Each week we will rotate the days we offer water games, crafts, science and exploration, sports/games, etc. so that part-time campers get the same opportunities as our full time campers. A weekly calendar will be emailed to parents and will be available to parents on our website at www.stpiuslombard.org/school. The calendar will contain information about each day including what we will be doing each week (activities, crafts, games) and what to bring to camp for each week. We will use our current email program that we use each week to send out our Green Knight Newsletter as our main form of communication with our summer camp parents. You will be asked to provide us with an email address that is frequently checked so that you may be kept advised of any camp changes. Activities, crafts, games and sports will coordinate with the theme of the week. Each day will include a morning snack. All full day campers and regular campers are required to bring a lunch daily following good nutrition. Rest periods will be available for any camper who needs it during the day. Pirates and Princesses Summer Camp Weekly Themes Week 1: The Adventures of Pirates and Princesses (June 6 th June 10 th ) Week 2: Bugs and Animals Adventures (June 13 th June 17 th ) Week 3: Frozen Adventures (June 20 th June 24 th ) Week 4: Let s Celebrate America Adventures (June 27 th July 1 st ) Week 5: Under the Sea Adventures (July 5 th July 8 th ) Week 6: The Adventures of Super Heroes and Star Wars (July 11 th July 15 th ) Week 7: The Adventures of Cowgirls and Cowboys (July 18 th July 22 nd ) Week 8: Imagination Adventures (July 25 th July 29 th ) Week 9: Sports Adventures (August 1 st August 5 th ) Week 10: Video Game Adventures (August 8 th August 12 th )

Field trips will be taken at least once a week and may be offsite or on school grounds. Field trips are only for campers if they take place on their regularly scheduled day of attendance. Field trip expenses are included in the cost of the full-day and regular day campers fees if they are in regular attendance on that day. Half-day campers will need to pay for field trips that extend past the dismissal time of noon and can attend if they are scheduled to attend on that day. Field trip prices will vary depending on the type of field trip. All field trips will require a separate Diocese of Joliet permission slip to be signed by the parent. Each camper must have a medical emergency and health history form on file prior to their first day of camp. This form will list any restrictions to the child s activities due to health reasons, medication, emergency numbers to call, physician number and a release to treat in the event of an emergency. Children who require medication or wish to have medication available to them (over the counter or prescription) must have a Diocese of Joliet Medical Form on file as well. Students may not have medication in their possession. Your child s Camp Counselor will be responsible for keeping and dispensing medication to their campers. Each team will have an emergency first aid kit with them at all times. Camp Directors and Camp Counselors will be aware of the location of the AED s on the premises and will have their cell phones with them at all times. Each camper will need to bring a backpack each day to camp. These backpacks should be brought to camp each day of attendance and should include lunch (labeled with name). All campers must bring a water bottle each day they attend, labeled with their name, as we spend the majority of the day outside. We will refill these water bottles throughout the day. We recommend a change of clothes for our younger campers and a dry t-shirt for spills for all campers. Campers may bring their cell phones, electronic games and cd players (all labeled with their names), but must check them in with their Camp Counselor. These items may be brought out at appropriate times as designated by their Camp Counselor. St. Pius X Parish School Summer Camp 2016 is not responsible for lost or stolen items. All campers will need to bring a bottle of sunscreen, spray-on only, on the first day of camp to be shared with all the campers. Camp Counselors are not allowed to apply lotion to campers. Parents are asked to spray their children with sunscreen before coming to camp in the morning. Camp staff will re-apply sunscreen throughout the day.

All campers must wear gym shoes to camp each day. No flip-flops or sandals. On the days we will have water activities, we ask that you have your child wear their bathing suit under their play clothes and pack a towel and flip-flops for your child. Once the weather is warmer, campers should wear their swim suits each day under their play clothes. Each Camp Counselor will have a log of the students in their room. Campers and parents will be advised of their camp assignments prior to the first day of camp. Please see your Camp Counselor to check in and check out your child each day. If a camper will not be in attendance, a phone call is requested one hour before scheduled check in time. Attendance is not mandatory at camp, however, camp fees will not be reduced due to nonattendance. Camp fees have been structured to include materials, supplies, staff salaries, insurance, crafts, games, field trip fees, and special projects. Please call the Camp Director, Mrs. Jessica Jarosinski at 309-838-9753 or Ms. Katelyn Kott at 630-797-8750, if your child will be absent, picked up early or dropped off late, etc. If you are unable to reach anyone, please contact the school office and leave a message. The summer camp will have a special doorbell outside of the school marked camp bell. Please ring the bell to gain admittance in the morning until 9:00 a.m. or to pick up in the afternoon between 4:00 p.m. and 6:00 p.m. If you are picking up outside of these hours, please call Mrs. Jarosinski s or Ms. Kott s cell phone in case the camp is outside. St. Pius X Parish School Pirates and Princesses Summer Camp 2016 WAIVER AND RELEASE Please read this form carefully and be aware in registering your minor child/children for participation in the St. Pius X Parish School Pirates and Princesses Summer Camp 2016 you will be waiving and releasing all claims for injuries your child/children might sustain out of the above program. I recognize and acknowledge that there are certain risks of physical injury to participants in the above program and I agree to assume the full risk of any such injuries, damages or loss regardless of severity, which my child/children may sustain as a result of participating in any activities connected or associated with any such program. I waive and relinquish all claims my child/children or I may have against St, Pius X Parish School or St. Pius X Parish, and its officers, agents, servants, staff, volunteers and employees as a result of participation in the above program. I hereby fully release and discharge St. Pius X Parish School and St. Pius X Parish and its officers, agents, staff, volunteers and employees from any and all claims from injuries, damage or loss which I or my child/children may have or which may accrue to me or my child/children on account of my participation or the participation of my child/children in the above program. I further agree to indemnify and hold harmless and defend the St. Pius X Parish School and St. Pius X Parish and its officers, agents, staff, volunteers and employees from any and all claims resulting from injuries, damages and losses sustained by me or my child/children and arising out, connected with or in any way associated with the activities of the program. St. Pius X Parish School Pirates and Princesses Summer Camp 2016 SUMMER CAMP CONTRACT I am enrolling my child/children in the St. Pius X Parish School Pirates and Princesses Summer Camp 2016. I understand that payment is due in full prior to the start of each week of camp that my child/children will be in attendance. In addition, I understand that I will be required to pay for all days I register for camp and that there are no refunds given for days missed due to unplanned vacations, illness or other absences. I will pay a registration fee equal to the amount stated below based on the option I choose for my child/children. I understand that these registration fees are non-refundable and must accompany this contract in order to reserve my child/children s space. I agree to pay a $1.00 penalty for each minute past the 6:00 p.m. pick-up time for my child/children for summer camp. I acknowledge that this penalty will be automatically added to my summer camp balance. Space at summer camp is on a first come, first serve basis. FULL TIME CAMPER: Campers who will arrive between 7:00 a.m. and 9:00 a.m. and will be picked up between 4:00 p.m. and 6:00 p.m. Monday thru Friday of each week. Cost for a full time camper is $200.00 per week for the first child. This total breaks down to an approximate average of $4.00 per hour for a child to attend camp full time. Families with multiple children may deduct $15 off the weekly rate for the second child ($185 per week) and $30 off the weekly rate for the third child ($170 per week). Please check here if your child/children will be full time campers. Time I will drop off my child/children each day: (between 7:00 a.m. and 9:00 a.m.) Time I will pick up my child/children each day: (between 4:00 p.m. and 6:00 p.m.) I have enclosed with this contract a deposit of $200.00 which will be applied to my child/children s last week of attendance at camp. REGULAR DAY CAMPER: Campers who will arrive at 9:00 a.m. and be picked up at 4:00 p.m. each day, Monday through Friday. Cost for a regular day camper is $160 per week for the first child. Families with multiple children may deduct $15 off the weekly rate for the second child ($145 per week) and $30 off the weekly rate for the third child ($130 per week). I have enclosed with this contract a deposit of $160.00 which will be applied to my child/children s last week of attendance at camp. PART-TIME CAMPER: A part-time camper may attend camp one or more days of any week/weeks that you choose. A minimum of one day is required for each week of camp. Campers who require 5 days a week (full time or regular) are required to pay the rate of a regular camper or a full time camper.

A part-time camper may choose to attend one or more days a week (up to four days) as a: Full time camper hours (7:00 a.m. to 6:00 p.m.) $40.00 per day Regular camper hours (9:00 a.m. to 4:00 p.m.) $32.00 per day Half Day Camper (9:00 a.m. to noon). $20.00 per day So that we may be able to adequately and safely staff our summer camp and prepare for field trips and activities, we ask that parents pre-register for Summer Camp in advance. Please check here if your child/children will be part-time campers. I have enclosed with this contract a deposit equal to one week of camp based on my days and times chosen below, which will be applied to my child/children s last week of attendance at camp. FIELD TRIP PERMISSION SLIP FORM JOLIET DIOCESAN SCHOOL SYSTEM St. Pius X Parish School February 22, 2016 During the St. Pius X Pirates and Princesses 2016 Summer Camp, our campers will be taking various field trips to destinations that incorporate our weekly themes and activities. Some of these field trips will be at no additional cost, however, any field trip that extends beyond the half day dismissal time of noon will require an additional payment by the half day campers to attend. We will advise parents in advance (via email) if there is any additional charge for field trips. Please sign and return this permission slip by April 1, 2016. This permission slip must be turned in prior to attending any of the Summer Camp field trips and covers all field trips taken during the Summer Camp 2016 dates of June 6 th, 2016 through August 12, 2016. Students without a field trip permission slip will not be allowed to attend any of our field trips. Handwritten or verbal permission will not be accepted per the Diocese of Joliet.
