Source: Highway Rail Pipeline Fixed Facility Air Vessel UST Unknown Other: Transformer Reported Cause: Transportation Accident Operational Error Equipment Failure Natural Phenomenon Dumping Unknown Other: Vehicle hit transformer. Medium Affected: Within Facility Only Air Land Groundwater Water (Name): Storm drain to Arkansas River and the Arkansas River. List Materials and Volumes: Non-PCB mineral oil (amount in transformer). Brief Incident Description: Vehicle hit transformer and non-pcb mineral oil flowed into storm drain and to the Arkansas River. Damages: Injuries #: Deaths #: Property Damage >$50,000 Evacuations: Corrective Action Taken: Entergy contracted TAS Environmental (7825 Fourche Road, Little Rock, AR 72209) to remediate spill. TAS Environmental placed boom around the outfall of the storm drain outlet where it flowed to the Arkansas River. Boom was also placed further out in the river from the stormwater outlet discharge point. Cellulose based absorbent was placed on top of the water. The cellulose based absorbent (Cell-U-Sorb) requires removal from the top of the water and appropriate disposal. Per Patrick Mehran, TAS Environmental was informed of this via phone on 03/10/2014. Responding Agencies/Parties: ADEQ Hazardous Waste and Water Divisions. Additional Comments: I received the information regarding this spill on 03/10/2014. Subsequently, I entered the site at approximately 11:45 am on 03/10/2014. The on-site coordinator for TAS Environmental, Greg Jackson, informed me that Hazardous Waste Division (Patrick Mehran and Thomas Hunting) had already visited the site previous to my arrival.
