INFORMATION PACKET Camp Dates, Hours & Fees Registration Fee: $100 Due at Registration (Per Camper) Day Camp Sessions 10:00am - 4:00pm Session 1 June 3rd - 28th, 2019 $250 per camper Session 2 July 8th - 26th, 2019 $185 per camper Extended Care Sessions Day Break Care 8:00am - 10:00am Session 1 June 3rd - 28th, 2019 $120 per camper Session 2 July 8th - 26th, 2019 $90 per camper AfterCare 4:00pm - 5:15pm Session 1 June 3rd - 28th, 2019 $100 per camper Session 2 July 8th - 26th, 2019 $70 per camper July 4th Mini Week Session 8:00am - 5:15pm Mini Week - July 1st - 5th, 2019 $120 per camper (No Camp July 4th) <Limited Space for Mini Session> Program Eligibility - Homewood Residents Only Camper Eligibility: Children entering 1st grade through 6th grade Note: Must be age 6 on or before September 1, 2019 in order to be eligible. Day camp does not have unlimited availability; No one is guaranteed a spot in camp. Extended Care Sessions are limited to the first 100 paid campers; and must be enrolled in the corresponding Day Camp session to enroll in the Extended Care Sessions. Registration Procedure Homewood Residents: Registration forms can be dropped off at the Homewood Community Center beginning: Monday, March 4, 2019 at 8:00am. Registrations cannot be dropped off before: Monday, March 4, 2019 at 8:00am Non-Homewood Residents: Not eligible for camp registration * Registrations must contain fully completed forms and registration fee for each camper. Registration fee can be included via check or money order to HOMEWOOD PARKS. Please do not fold registration forms; place in a large flat envelope. If registering multiple children please include all in the same envelope, but use separate checks for each child. Should registration fee not be included, registration will be deemed incomplete and not accepted for consideration. * $100 Registration Fee due for each camper with their registration packet; remaining balance for each camper is due on or before Wednesday, May 15, 2019. Please do not include full payment with registration packet. * Registration fee will be deducted from each campers final balance due. If your child is not admitted to camp registration fee will be returned. * Summer Camp does not have unlimited availability. Registrations received the week of March 4th will receive notification by Friday, March 15th; if they have (or) have not been admitted to camp.

INFORMATION PACKET Camp Location & Amenities Homewood Community Center & Homewood Central Park 1632 Oxmoor Road Homewood, AL 35209 Homewood Community Center & Homewood Central Park Amenities 2 Gymnasiums 8 Lane - Competition Pool Splash Water Playground 4 foot leisure pool with vortex feature 6 Pavilions 2 Tennis Courts Large Playground Open multi-use greenspace Program Description Campers will participate in traditional and non-traditional sports, fun games, swimming, field trips, arts & crafts projects, and more. Trained staff will supervise participants while emphasizing the importance of teamwork, good sportsmanship, and cooperation through games, sports, and group activities. Camp Staff The Summer Camp staff is mostly comprised of college students with experience in recreation, education backgrounds and youth leadership. All staff are background checked, certified in CPR, AED, basic first aid and child abuse prevention. The Summer Camp staff is dedicated to providing a fun and safe environment for each participant. The staff is responsible, qualified, and trained to work with children of all ages. Camp Age Groups Homewood Parks Summer Day Camp will be divided into groups based on the campers upcoming school grade. Campers are placed in the following groups by their grade. (*Groups are subject to change; to allow the most participants an opportunity to attend camp.) 1st Grade Coed 2nd Grade Coed 3rd Grade Coed 4th Grade Coed 5th & 6th Grade Coed What Your Camper will be doing at Camp Normal Daily Activities: may include, dance, arts & crafts, volleyball, basketball, kickball, movies, soccer, table games, playground, outside sports, relay races, four square, arts & crafts and much more. Pool Activities: Lifeguards and the camp staff will be on duty during swim time. Campers will be restricted to certain areas of the pool according to their ability. Campers will be given a swim test prior to participation in any pool activities. Field Trips: Each camp group will have 2 to 3 age specific field trips per camp session. Some examples of past field trips are: McWane Science Center, Birmingham Zoo, Bowling, Pump it Up, Chuck E Cheese and more In addition to field trips we will also have several Special Event Days at Camp! Day Camp Orientation Once registered, campers will be informed of the date and time for our Parent/Camper Orientation. This event will be a chance for parents and campers to come meet their counselors before camp begins. Children with Special Needs Homewood Parks and Recreation will accept children with special needs into our programs, and makes a reasonable effort to accommodate the child without fundamentally altering the camp program. Homewood Parks is not able to provide personal assistants for children with special needs. If a child needs an aide please contact the Camp Director for directions in placing an assistant in our program to aide your child. CONSENT & WAIVER Permission for Fieldtrips, Excursions, and use of Public Facilities I hereby give consent to Homewood Parks and Recreation and its designated staff to take the above named child on walking trips in the neighborhood, public park facilities, special excursions to places of interest in Homewood Parks vans, buses, commercial vehicles, public schools transportation, or rented vans or buses, with the understanding that such trips are under supervision of authorized personnel of Homewood Parks & Recreation and that all possible precautions are taken to insure the health and safety of my child. Initial Child s Health Statement I, the undersigned parent/legal guardian, understand that at a Homewood Parks Day Camp Program, physical activity is a regular part of the program. To the best of my knowledge, my child is in excellent physical health and needs no restrictions (except what is listed on the Registration Form) from strenuous physical activity. If I have any questions regarding my child s health, I understand that it is my obligation to seek professional medical advice and to inform Homewood Parks & Recreation of any restrictions on my child s activities. Initial Important Information Homewood Parks and Recreation strives to conduct its recreation programs and activities in a safe manner and holds the safety of participants in the highest regard. Participants and parents registering their child in recreation programs must recognize however that there is an inherent risk of injury when choosing to participate in any recreation activities. Homewood Parks and Recreation continually strives to reduce such risks and insists that all participants follow safety rules and instructions which have been designed to protect the participant s safety. Please recognize that the City of Homewood / Homewood Parks and Recreation does not carry medical accident insurance for injuries sustained in its programs. Therefore, each person registering themselves or family member/ward for a recreation program/activity should review their own insurance policy for coverage. Homewood Parks and Recreation requires execution of the following Waiver and Release. Your cooperation is greatly appreciated. I further agree to indemnify, hold harmless, and defend Homewood Parks and Recreation, the City of Homewood, its officials, agents, servants, representatives, employees and board members from any and all claims for injuries, damages or loss sustained by me or my ward arising out of, connected with, or in any way associated with the activities of the program. In the event of any emergency, I authorize program officials to secure from any licensed hospital, physician and /or medical personnel any treatment deemed necessary of my or my ward s immediate care and agree that I will be responsible of repayment of any and all medical services rendered. I fully understand that traveling by bus involves risks and dangers of serious bodily injury, including permanent disability, paralysis, and death; these risks and dangers may be caused by my own actions or inactions, the actions and inactions of other drivers or passengers; there may be other risks and social and economic losses either not known to me or not readily foreseeable at this time; and as between myself and HPRB, I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND RESPONSIBILITY FOR LOSSES, COSTS, and DAMAGES I incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue the City of Homewood or The Homewood Park and Recreation Board, its administrators, directors, agents, officers, partners, volunteers and employees ( Releases ) from any and all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the Releases, or others and I further agree that if, despite this release and waiver of liability, assumption of risk and indemnity agreement, I, or anyone on my behalf, makes claims against any of the Releases, I will indemnify save and hold harmless each of the Releases, from any litigation s expenses, attorney fees, loss, liability, damage, or cost which may incur as a result of such claim.
