The purpose of this supplement is to cancel Supplement No. 5 issued October 3, 2002 under Transmittal No. 84. Supplement No. 5 was filed pursuant to the Commission s Order DA 02-2472 released September 30, 2002. On September 30, 2002, as noted in Suspension Order 02-2472, the Pricing Policy Division of the Wireline Competition Bureau suspended for one day, imposed an accounting order, and set for investigation the aforementioned transmittals. The transmittals addressed in that order sought to establish rates for Universal Service Fund charges billed to residential and business customers to reflect the companies fourth quarter 2002 contribution obligations. After release of the Suspension Order, Division staff met with SBC to discuss the tariff filings. SBC provided additional explanations in support of its tariff filings that satisfied Division staff concerns. Accordingly, the investigations and accounting orders imposed in Order 02-2472 are terminated. Supplement 12 This filing is made pursuant to the Bureau s Suspension Order DA 16-805 (Order), released July 15, 2016. On July 1, 2016, PBTC issued its Transmittal No. 539, which proposed to grandfather the DS1 High Capacity Service Portability Commitment and discontinue renewals. The DS1 High Capacity Service Portability Commitment was to continue to be available to existing subscribers for the remainder of each subscriber s 3-year commitment term. Additionally, PBTC proposed to remove the current charges for calculation of shortfall and early termination charges and replace those with a lower charge. Supplement 13 This filing is made pursuant to the Wireline Competition Bureau (Bureau) Order DA-16-1339A1 to terminate the investigation initiated on July 15, 2016, regarding the tariff revisions filed on July 1, 2016, by Pacific Bell Telephone Company (PBTC), under Transmittal No. 539. On July 15, 2016, the Bureau suspended the tariff revisions and initiated an Investigation by its Order DA 16-805 (Suspension Order). In response to that Suspension Order, PBTC filed its Supplement 12 under Transmittal No. 541 to advance the effective date of Transmittal No. 539 to July 15, 2016, and then suspend it for one day until July 16, 2016.
