Registration Forms Health Records Tour Agreement Baggage Tags As the Tour Organizer, you re responsible for collecting and returning completed and signed forms from all tour participants, including all chaperones, to Gerber Tours. Only ORIGINAL double-sided forms will be accepted for Registration. If the participant is a minor, the form must be signed by a parent or guardian. We recommend collecting Health History and Medical Release forms from everyone in your group. Bring them along on your trip in case of emergency. For your convenience, we have enclosed a sample form. After careful review, please sign and return both copies along with the Registration Forms and the non-refundable booking deposit by the date specified in the Tour Agreement. We will then send back to you a counter signed copy for your records. We have enclosed a pre-addressed envelope for your convenience. Cancellations occurring within 60-15 days of departure will be subject to cancellation fees of 20% of the Tour Cost, the nonrefundable booking deposit as specified in the Payment Schedule on the following page and any amounts paid to suppliers to confirm Group's reservations. 7. In any such arbitration the substantive (but not procedural) law of New York will apply. The arbitrator and not any federal, state, or local court or agency shall have exclusive authority to resolve any dispute relating to the interpretation, applicability, enforceability, conscionability, or formation of this contract, including but not limited to any claim that all or any part of this contract is void or voidable. Nicknames or shortened forms of names may not be used. For this reason, please make sure that the names on all Registration Forms are full, legal names exactly as it appears on their ID so that we have accurate information for ticketing. Passengers with incorrect names on their tickets may not be permitted to board their plane. Once Gerber Tours tickets the group, any name change from names submitted on Registration Forms is subject to a $150 fee. Fares Airfares are not guaranteed until ticketed and are subject to price adjustments as a result of tax changes, fuel surcharges and/or airport fees. Baggage Many airlines are now charging fees to check baggage. Please see attached Baggage Notice for more details. Identification Domestic Flights: All passengers 18 and older are required to present valid and unexpired government-issued photo identification (federal, state or local). For example: driver s license or passport. Name on ID and ticket must be an exact match. Children under the age of 18 traveling with an adult are not required to show ID at time of check-in. However, we recommend all passengers carry some form of identification and children close to 18 years of age should also carry proof of age (ie. birth certificate). IMPORTANT AIRLINE BAGGAGE NOTICE: As you are probably aware, many airlines are now charging fees for checked bags. Due to the constantly changing nature of these fees and the difficulty in pre-paying them, these fees are not included in the price of your airfare. As a result, many clients have advised their trip participants to limit their luggage to carry-on items only. Most airlines allow one carryon bag plus a personal item such as a purse, briefcase or small backpack.
