INFORMATION and NOTICE for GUESTS (Dai-ichi Life Higashi-Totsuka Training Center --- Your Accommodation) Welcome to FALIA and Dai-ichi Life Higashi-Totsuka Training Center! In order to make your stay more pleasant and comfortable, we would appreciate your kind attention to the following information. 1. Please always put your name plate on! Name plate shall be provided on your arrival. 2. Please be very punctual! Please check the seminar room or meeting place in advance and be there at least 10 minutes before the scheduled time. Morning calls are not available. 3. Please keep the manners! Please do not wear a robe (Yukata), pajamas and slippers outside of the room. Please do not take a bath / shower in the early morning (before 6 am) or in the late at night (after 12 midnight). 4. Smoking Smoking is only permitted in the designated rooms located on the 3 rd and 4 th floors in the seminar area of this Center. Smoking in your rooms are strictly prohibited. Please kindly refer to the following sheet for more details. 5. Telephone Call to other rooms in the training center, 9 0 - (Room number) Outside line (domestic & overseas) is not available from your room. 6. Meal Breakfast is usually served between 7:30 and 8:30 in the cafeteria on the 1st floor. Please be sure to put your name plate on. Please make sure to separate your garbage into plastics, papers and others. The leftovers should stay on your plates when you return the tray. Smoking and/or drinking alcohol is prohibited in the cafeteria. Dinner are not served in the cafeteria of this Bldg. 7. Television < Channel Information> (Language used is principally Japanese.) 1 - NHK GENERAL 2 - NHK EDUCATIONAL 4 - NTV 5 - TV ASAHI 6 - TBS 7 - TV TOKYO 8 - FUJI BS is not available.

8. When you leave the room, please check the followings. 1 Shut the window properly. 2 Close the curtain. 3 Turn off the lights, T.V. and other electric appliances. 4 Make sure to lock the door whenever you leave the room. 9. Tea Room (Saloon) Place: 3rd floor Open hours: from 9:30 to 21:45 Last order: 21:00 Light meals and some alcoholic beverages are available after 17:00. Menu in English is available. Credit cards are basically not acceptable. 10. Observation Lounge Observation lounge is on the 22nd floor of this Bldg. The lounge is open until 23:00. Please turn off the lights and air-conditioning when you leave a lounge. 11. Vending Machines A variety types of beverages including mineral water are available. You can drink tap water as well. Vending machine is installed on the same floor of your room. 12. Curfew of the Building The Building is locked at 22:00 in principle. 13. Bed making & Cleaning Bed making, change of sheets and cleaning the room are served once during your stay. Towels and bath mat shall be changed every day. 14. Electricity When using a hair-dryer, please refrain from using other heat- producing equipment that requires a lot of electricity. If you operate several electrical devices at the same time, the circuit breaker may be tripped. In such a case, it takes time to reset and return the power. We ask for your cooperation in NOT using several electrical devices at the same time. 15. Other Information A Japanese-style robe (Yukata) and slippers are equipped but NOT complimentary. An iron and a trouser press are lent at the front desk. After you use them, please return to the front desk immediately. A washing room (laundry) is located on the 4th floor of this Bldg. Washing & Drying machines and detergents are equipped therein. Free Wi-Fi is only available in a PC room. In the toilet, you can wash away used toilet paper by pushing the bar. Please do not put the paper into a disposal case. During your stay, please do not use hair dye, nail polish and shoe polish both in the room and the bathroom. Compensation may be asked if stains remain on objects or surfaces in the room or the bathroom. Please return your room key to the front desk whenever you leave the accommodation. You can keep safety box key until the check out.

If you would like to send your luggage to Narita/Haneda airport, please use delivery service at Seven-Eleven shop. You need to bring the luggage at least 3 days before the departure date. Please ask FALIA staff for delivery slip. FALIA staff are ready to hear the inquiries about sightseeing spot, train route, restaurant, etc. Please do not hesitate to ask FALIA staff. Only the use of an iron and a trouser press is to be asked at front desk. IN CASE OF EMERGENCY such as fire, earthquake, etc. Please follow the instructions of FALIA staff, in principle. Do not use elevators when you evacuate! Please check the emergency exit chart which is posted on the room door and the attached Notice for Emergency. When you check out the Center Tidy up your room. If you damage or lose any equipment in the room, please inform FALIA staff of it. Make sure not to leave your belongings inside. Please return your room key and safety box key to the front desk. As from September 1 st, 2018, the official rule at the accommodation changed. Under the current rule, you cannot smoke in your room and any other places including corridor except smoking rooms on the 3 rd & 4 th Floors. Please be sure to keep this rule, otherwise cigarette butts may cause a fire! Do not dump your cigarette butts into any garbage box at the accommodation. Earthquake Notice for Emergency Do not get panic. Just stay in your room keeping yourself safe (*) and wait for the instruction by FALIA or any staff around you. * Stay away from glass windows and keep a door open to secure an escape route. Put the helmet on to protect your head. Fire If you are the first person who found it, let FALIA or any staff know it immediately. FALIA staff is always cooperating with the security guards of the Center and give you an instruction to evacuate the Center if it if necessary. For your reference, the evacuation place is Shinano-cho Park. Please refer to the map and check the route to the park.
