Health Management Group Program Guide Spring / Summer 2019 Ready to improve your health? Our programs can help! We offer a range of programs that can help you with specific health conditions. We also offer programs that can help you quit smoking, start exercising, or eat a healthy diet. Partners and support persons are welcome to register and attend. If you are an adult who can speak English or French and you are ready to improve your health, our programs are for you. Programs are free of charge. Note: Some class spaces may be held for patients of local MyHealth Team physicians. Programs are subject to change and may be cancelled at any time. Only people registered will be notified. Staff at all of these programs can help you link to additional resources in your community. Health Education Programs Programs to help you quit smoking, eat healthy, start exercising, or manage a specific health condition. Get Better Together A 6-week self-management workshop for you to better manage any health condition. Learn and practice skills to manage symptoms, reduce fatigue, problem solve and set goals. Community-Based Rehabilitation Cardiac Rehabilitation is offered at 2 community locations for those with a heart condition. COPD Essentials One 1 ½ hour session Learn what COPD is and how to manage COPD. This includes controlling flare-ups, anxiety, stress and steps to stop using tobacco. Introduces practical mindfulness exercises in order to reduce stress. Diabetes Series Separating Fact from Fiction This 4-part series will help you understand what diabetes is and how it affects the body. There is a lot of conflicting information about diabetes, and these classes provide an opportunity to separate fact from fiction with a focus on prevention, management and support. To gain the most from these programs, it is suggested that you start the series with Class #1 (Diabetes Essentials). The other classes may be taken in any order. Cardiac Rehabilitation Cardiac Rehabilitation is a 16-week program for those who have been diagnosed with any type of heart condition. This includes persons looking to return to better health after a heart attack or diagnosis of coronary artery disease, peripheral artery disease or other heart condition. In this program, you will: Learn the level of activity you can safely handle, and work on an individualized exercise program. Improve your diet and lifestyle, reduce cardiac symptoms, and reduce stress and anxiety. A referral from your doctor or nurse practitioner is required. There is a fee of $255 for this program. Financial assistance may be provided based on individual need and the availability of funds. Some extended health insurance plans cover all or part of the fee. You can ask your doctor to contact one of the sites below. This includes chronic obstructive pulmonary disease (COPD), emphysema, chronic bronchitis and pulmonary fibrosis. In this program you will: Learn the level of activity you can safely handle, and work on an individualized exercise program to improve your strength, breathing and sense of wellbeing. Learn to use medications effectively, prevent infections and get strategies to help stop smoking. Reduce respiratory symptoms and the risk of an exacerbation/ lung attack to slow the worsening of your condition. A referral from your doctor or nurse practitioner is required.
