Existing condition of Highway The condition of existing road required following improvements 1. The work of overlaying the bituminous concrete is under progress at site from CH: 50+000(LHS) onwards as per the pavement conditional survey. At present the work of overlaying is completed at site. 2. The work of Micro surfacing is also started in this month at site from CH: 76+330(LHS) onwards as per the pavement conditional survey. At present the work of Micro surfacing is in progress at Chainage : 87+000 RHS in sarwadi village. 3. The work of rectifying the pot hole in the stretch before overlay is under progress at site. 4. The work of Road marking with thermoplastic paint is under progress at site from CH: 76+000(LHS) onwards. 5. The work of white washing of old crash barrier is under progress at site from CH: 50+000 onwards. 6. During the rainy season the vegetation and grass developed in the median and the side shoulder of the road way and creating obstruction in the visibility of the road furniture and inner end of the road so that it requires to remove the obstructions and clean the road furniture for clear visibility to avoid the accidents. 7. The earthen shoulders also being damaged at various location required to repair with specified material up to the desired level with required compaction and maintained in proper line and level particularly at Ch 69+300, 98+800 (RHS) and 82+800, 91+900 (LHS). 8. The reflector of delineators and metal crash barrier have been damaged/vanished at various location particularly in the Ghats section is required to replace urgently. Page 11

9. The waste and useless plants developed on the side slope and on the earthen shoulder of the road way on both sides is under progress. 10. The maintenance work of avenue plantation is also required to provide the tree guards at required location. 11. The maintenance work of grade separator is also pending to execute at site regarding this point which we have already informed to concessionaire by letter. 12. The painting work of Median kerb is completed from ch : 59+000 to 71+000. The balance work is pending. 13. The work of painting is pending of the road edge marking along the median kerb edge along the entire length of the Project highway as per the IRC 35 1997. The NHAI PIU, Amravati also wrote letter to complete this work on priority. This work would be complete after the completion of Overlay work at site. 14. The light arrangement at required place should be sufficient on the project highway. In this matter we have given letter no. PF/KT/TL/OP/70 to the concessionaire to rectify the deficiency. In this month he has rectified Fourteen out of seventeen high mast of the project highway. 15. The metal crash barrier has been damaged at various location of the project highway and required to replace at site. In this matter we have given letter no. PF/KT/TL/OP/68 dated 10.04.2013 to the concessionaire to rectify the deficiency and the list of location of damaged crash barrier attached with that letter. The repairing / replacement work of crash barriers is still pending at site. 16. The barricades at Bus lay bye at CH 56+259 (LHS), 87+000 (both side) and 91+000 (both side) required to rectify earliest. 17. During the month of June 2014 Fifteen numbers of accidents has been occurred. The list of accidents attached in the report. We are taking required steps at site to reduce the number of accidents on the Project Highway. Page 12

ADVICE: The main purpose of operation and maintenance of the highway for the safe and easy movement of vehicles and also the safe crossing of pedestals hence it is necessary to observe each and every point related to safe and smooth driving. The concessionaire shall comply with the O & M requirements set out as per the manual during the concession period. Specification of routine maintenance activities will be follows as under: The carriageway should be pot holes free so that the vehicles can moves smoothly in specified speed. The road furniture should be sufficient at required places to give information to the road users to drive safely. The marking/painting work should be bright and clearly visible to the road users. The light arrangement at required place should be sufficient. Cleaning and repairing road side line drains. Disposal of sediments, extraneous debris or vegetable growth blocking flow of water. Safety devices and information/caution sign boards shall be placed as per MORTH clause no. 112 and maintained regularly. The work shall consist of the washing of signs, delineators, removal of posters etc. and repairing timely. All the sign boards and road safety fixtures arrangements should be clearly visible to the road users. Line marking with thermo plastic paint on toll road shall be carried out timely. The concessionaire must do the regular routine and periodic maintenance as per contract agreement and follow all the instructions/specifications as per the Operation and Maintenance manual. The repairing work should be done in a perfect specified manner so that repairing should not be redeveloped at the same place. It is also necessary to find out the correct reason of development of fault and required to rectify so that the work of maintenance would be reduced. The action should be taken earliest to rectify the fault at site in all respect. The concessionaire should submit the operation and maintenance program to follows accordingly. During the patrolling every day the road way should be checked thoroughly and try to rectify the problem earliest. Page 13

MINUTES OF MEETING: The meeting was held with Project Director Mr. N. Y. Wadetwar and Mr. Brahmankar (Manager Technical) NHAI, PIU on dated 05/06/2014 at office of NHAI, PIU, Amravati regarding work progress about maintenance at site. The site visit of Project Director Mr. N. Y. Wadetwar and Mr. Brahmankar (Manager Technical) NHAI, PIU on dated 20/06/2014.During the visit instructed to maintain the tree guards for the avenue plantation at site and progress improvement in micro surfacing work to complete earliest. The training/workshop program on PMIS tool for PPP projects at Mumbai on 17/05/2014 and 19/05/2014 attend by Team leader Mr. H.K Pandya organize by NHAI RO Maharashtra. The meeting was organized on dated 18.06.2014 at Karanja Toll Plaza with Mr. Pankaj Gautam (senior pavement specialist), Mr. Bargi, Project Manager and site Engineer of the concessionaire to discuss about the matter raised in the joint review meeting regarding to complete the balance work of Operation and Maintenance work of the project highway. During the review meeting the concessionaire agreed to complete the balance work of micro surfacing before 15 th August 2014. The periodic maintenance work of overlay (BC) has been completed throughout the highway where it was required and the remaining carriageway will be improved by micro surfacing work. During the last meeting with the concessionaire Mr. Raman, executive director of Oriental Pathways Pvt. Ltd agreed to complete the following maintenance work of the project highway. He assured that the work will be completed in all respect within the given time schedule. The points were discussed in the meeting as follows: Page 14

PROGRESS REPORT OF OPERATION AND MAINTENANCE WORK IN THE MONTH OF JUNE 2014 The maintenance work of tree guards to the avenue plantation is in progress at site. The work of overlaying the bituminous concrete from Chainage : 50+000 to 100+000 has been completed at required location on site. The work of micro surfacing from ch: 76+330 to 100+000 (LHS) and 100+000 to 80+000 (RHS) have been completed and remaining work (RHS) is in progress at site. The work of Road marking is in progress from ch: 76+900 to 98+050 has completed (shoulder edge and center line only) remaining work is in progress at site. The work of white washing of old crash barrier is completed at site. The Pot holes treatment work completed at site. The work of trimming the median plantation is in progress from ch: 85+000 to 100+000 has completed and remaining work is in progress at site. The maintenance work of earthen shoulder is in progress at site The work of grass cutting on earthen shoulder is in progress at site. The work of repairing/rectification of high mast at CH: 64+000 and CH 65+000 have been completed at site.
