POLICIES FOR ALL EXCURSIONS Refunds: Refunds must be requested 7 business days prior to day trips and 30 days prior to overnight trips. No refunds issued after that time. Effective 1/1/14, there is a $10 service charge for cancellation of each seat. Full refunds will be issued if the Department cancels a trip. Excursions include transportation on a luxury liner coach; tickets are not sold separately. Return times are approximations; actual return times may vary. Staff will make excursion reminder calls as a courtesy, if time permits, prior to scheduled trip. However, it is the participant s responsibility to be aware of the dates and times of excursions in which they are registered. For more information call: (909) 394-6290 Boarding the bus on time: Tour itinerary will be adhered to in order to ensure prompt departure and arrival times. In general, please arrive at least 15 minutes before every departure time. Individuals who are not at the boarding location at the specified times, both upon departure or return time, shall forfeit the cost of the trip and will be responsible for their own transportation arrangements and all related expenses. For Adult Excursions, participants will board bus 15 minutes prior to departure, in order of registration, and sit in preferred available seats. Bus seats will not be selected prior to boarding the bus. DEPARTURE AND RETURN INFORMATION Adult Excursions: Depart from and return to the west side of the CVS Pharmacy parking lot, unless otherwise noted. INFORMATION FOR ADULT EXCURSIONS San Dimas residents: Registration begins at 8 am at the Senior Citizen/Community Center, 201 East Bonita Avenue, on the date specified for each trip. Proof of residency is required. Each resident may register themselves and one guest per trip. Residency is not required for the guest of a San Dimas resident, but non-resident fees are applicable. Non-Residents: Registration begins at 8 am at the Senior Citizen/Community Center on the date specified for each trip. Resident/Non-Resident Mail-in Registration: No more waiting in lines. Register from the comfort of your home. Resident and Non- Resident mail-in registration is processed after the first day of resident/non-resident walk-in registration on the dates specified for each trip. Complete a registration form and you will receive a receipt via mail or email. Travel Pack: Each traveler receives one bag containing a beverage, light snack and candy as they board the bus. Medication: Tour personnel are not responsible for administering medication and may only assist by providing water. Therefore, participants must administer their own medication. Individuals are accountable for emergencies and consequences that arise as a direct result of their non-compliance to prescription directives. Please remember to take your medication! Request for Meal Accommodation Adult Excursions: Request must be made at the time of registration. The City of San Dimas Parks and Recreation Department makes every effort to offer vegetarian and vegan meals (V meal) on City sponsored excursions. Please know that the City cannot guarantee that every establishment will provide V meals or that vouchers issued will accommodate the purchase of V meals. The City will advertise when V meals are available but cannot guarantee the establishment s compliance with established vegetarian or vegan dietary guidelines. Upon arrival at the establishment it is responsibility of the participant to discuss with a representative of the establishment, V

meal ingredients, and come to agreement on the V meal option being served. Whenever possible the Department will advertise in advance the V meal option that is available. If the meal cost is included as part of the overall package, discounts are not given for individuals who choose not to eat the meal. It is the responsibility of the participant to notify staff at the time of registration of a V meal request. Disclaimer: Excursion information is correct and up-to-date as of the time of publishing. The Department reserves the right to change any information as necessary Activity Level Ranking: The definition of each activity level is listed here. If you have additional questions regarding a specific trip, please inquire prior to registration. Level 1: Easy activity level with light/minimal physical activity, no required stairs or walking may be at leisure. Level 2: Average physical activity. You should be able to climb stairs and tolerate light walking. Level 3: Moderate physical activity. Walking tour or walking slightly longer distances, up stairs or on uneven walking surfaces. Level 4: Intense physical activity. May require continuous walking or difficult access with stairs. Walking on uneven terrain or periods of standing. Extended excursions. Level 5: Very active physical activity. May include extensive walking, uneven or dusty terrain or high altitudes. Early or late hour activities. Participant should be physically fit to fully enjoy the excursion. (S) Special Recommendations/Requirements: Special clothing, footwear, gear, identification or luggage may be needed. See specific tour details for more information. At time of registration, please specify meal choice or any special accommodations you will need. Disabled Accommodations Available: Inquire with staff for special accommodations or specific needs at time of registration. Sites may or may not be wheelchair accessible, have special seating, or elevators. LIABILITY RELEASE STATEMENT I fully understand that my participation, or that of the minor in my custody as registered, in the above mentioned activity, presents exposure to the risk of personal injury, death or property damage. I hereby acknowledge that participation in this event/class is voluntary and agree to assume any such risks. I hereby release, discharge and agree not to sue the City of San Dimas, its officers, agents, employees or volunteers for any injury, death or damage to or loss of personal property arising out of, or in connection with, participation in the event/class from whatever cause, including the active or passive negligence of the City of San Dimas, its officers, agents, employees or volunteers or any other participants in the event/class. In consideration for being permitted to participate in the event/class, I hereby agree, for myself, my heirs, administrators, executors and assigns, that I shall indemnify and hold harmless the City of San Dimas from any and all claims, demands, actions or suits arising out of or in connection with my participation in the event/class. I also agree and acknowledge that participant may be photographed while participating, and release use of the photographs for reproduction in City-sponsored publications. Travel to the beautiful Lawrence Welk Resort Theatre in Escondido for a matinee performance of Mamma Mia and lunch. The fun, romantic comedy, Mamma Mia is sure entertain and leave you laughing! You may also visit the museum on site, which contains memorabilia highlighting the life and career of legendary bandleader, Lawrence Welk. In 1964, Welk took a drive in north county San Diego, planning to invest in a grove of orange trees. Instead, he bought a motel and golf course. He began staging shows there and over the years it expanded to become the iconic Welk Resort and Theatre. The theatre is known for producing award winning entertainment featuring top-notch actors, musicians and performers in popular Broadway-style musicals. Spend a day in Palm Desert at the unique Living Desert Zoo and Botanical Gardens, which focuses on educating visitors about the deserts of the world. The botanical gardens include geographic replicas of areas as diverse as Baja California s Viscaino Desert and the exotic deserts of Madagascar. Spend the day touring two scenic and historic locations. First, we will arrive at Gardens of the World in Thousand Oaks for a guided walking tour of the Japanese garden, French garden and water feature, Mission courtyard, Italian garden, English perennial and rose garden and so much more. Then we will travel to nearby Lakes of Thousand Oaks Mall for lunch (on your own). Meal: On your own. After lunch, our next stop will be the Leonis Adobe Museum in Calabasas. Built in 1844, the Leonis Adobe is one of the oldest surviving buildings in the San Fernando Valley. It was occupied by the wealthy rancher, Miguel Leonis, until his death in 1889. By 1961, the adobe had fallen victim to vandalism and its owner applied for a permit to raze the structure and erect a supermarket in its place. Preservationists succeeded in having the adobe declared a Historic-Cultural Landmark (the first structure in Los Angeles receiving the designation) in 1962. Now, generations of visitors can step back in time and visualize life in early California. Special notes: This is a small group tour, limited to 30 participants.
