I declare that the applicant has a reliable administration/monitoring system for this aircraft, so it is ensured that all maintenance tasks and instructions from the authorities are performed within the fixed deadlines and that the configuration of the aircraft is clear. An acceptance review can only be executed by ARC-staff of a Dutch CAMO with an I-privilege on the concerning aircraft or by the ILT. Fill in the name of the company performing the review with the approval number of the CAMO. Fill in the name of the person representing the reviewing company. Tick the box with the classification of the kind of certificate of airworthiness this aircraft should have according the reviewing company. More information can be found in the Basic Regulation 216/2008, article 1, article 4 and Annex II. Fill in the name of the reviewing inspector. In addition to this report the holder has to send a signed application form to ILT. This application form may (preferable) send together with this report. The application form can be downloaded from the website of ILT (www.ilent.nl) On importing of a new aircraft to the Netherlands, a copy of the conformity statement needs to be handed over. With this statement it is indicated that the aircraft is accepted within one of the EASA-member states. For an aircraft coming from an EASA member state the statement can be through a Form-52 which is given to the owner at delivery. For aircraft coming from the USA, Canada or Brazil an export CofA / statement is necessary from the exporting authority. When importing of an existing aircraft to the Netherlands from an EASA member state, the aircraft has to have a valid ARC. If the aircraft has no valid ARC additional requirements can be made by the authority. Send along a copy of the statement / export which is not older than 60 days. Depending of where the aircraft is coming from one should realise that maintenance regulations can differ from the Netherlands. Therefore it must be assured that the quality of the performed maintenance is adequate. By means of a physical inspection of the aircraft it must be checked that there are no apparent defects. If defects are found they have to be fixed or deferred in a controlled manner (DDL, HIL etc.) The inspection can, on request from the inspector, include engine run-ups or even a test flight Check that before a copy is send whether possible changes have been calculated in the weight & balance report. Also check whether a new weighing has been performed when necessary, for example after maintenance of repaint. Is the weight & balance report valid within the deadlines of EU-OPS? Does the aircraft flight manual meets the configuration of the aircraft? For example are all modifications, SB s, AD s etc introduced? The registration marks on the aircraft have to be applied in accordance with the regulations. It concerns the PH-registration on the aircraft and the fireproof identification plate. More information can be found in the Regeling inschrijving Nederlandse burgerluchtvaartuigen paragraph 9. Also the aircraft, engines and propellers have to be correctly identified according Part 21.A.801. ILT.040E.03 5 of 6

Deviation of this is only possible if documents cannot be obtained in former mentioned languages and the users understand the language of the used texts. Randomly check physically on the aircraft whether the configuration of the aircraft complies with the configuration as mentioned in the administration and flight manual. Think about TCDS, STC s, SB s but also repairs and AD s. Every change has to be accepted/approved by EASA. A copy of the maintenance program which complies with the regulations must be submitted. In contrast to the past the ILT will only issue a CofA after they have received an approved maintenance program, or, in case of ELA1 aircraft which are not used for commercial operations, a statement (Declaration) from the owner. In case the ILT has to approve the maintenance program one has to take into account that this might take some time. Check if an administration/ surveillance system is present conform M.A.305. If the aircraft is not managed by a CAMO then the administration/surveillance system must be checked. Make sure that:: Tasks from the maintenance program are taken over correctly (task content and intervals). AD s are correctly addressed. Repairs are correctly addressed and provided with a reference to approved data. Modifications are correctly addressed and based on approved data. Check whether all documents related to the work performed on the aircraft and service life limited components are present. This includes that all proof of releases (CRS) have to be present unless a comparable performed task overrules the former CRS s, and are older than 24 months. Also from all On-Condition components that have been installed in the past 24 months the releases have to be present. These documents have to be handed over at export to the new owner. Take into account the requirements in EASA Part M.A.305 To make this statement all questions applicable have to be answered positively. Also one should request the authority of the country of import whether there are any additional requirements on top of the EASA requirements. It is necessary to attach the letter with the answer from the authority of the country of import to the report. Also in case the authority agrees to any deviations, this also needs to be announced in a letter. These deviations will be mentioned on the Export CofA with a reference to the letter (additional requirements and possible waivers). ILT.040E.03 6 of 6
