Pacific Airways, Inc., (Pacific Airways) a corporation formed pursuant to the laws of the State of Alaska, hereby applies for a certificate of public convenience and necessity to transport passengers, property and mail in interstate air transportation pursuant to the provisions of 49 USC 41102. Consistent with the requirements of 14CFR302 and 14CFR204.3, PM Air provides the following information. a. The name, address and telephone number of the applicant is: Pacific Airways, Inc.. P.O. Box 5 158 Ketchikan, AK 99901 (907) 225-3500 b. An Affidavit of Citizenship is attached as Appendix 3

c. A list of all key personnel with the information required by 14CFR204.3 (0 is attached as Appendix C. d. The ownership of Pacific Airways, Inc. consists of: Michael Rhoads Stephen Montanus Josh Murdock 33.3% 33.3% 33.3% e. Pacific Airways, Inc. has no subsidiaries. f. Pacific Airways, Inc. holds no shares or ownership in any air carrier, foreign air carrier, common carrier, or person substantially engaged in the business of aeronautics. g. Pacific Airways, Inc. has been operating scheduled and on-demand air service as an intra-state air carrier since 2002. Financial results for CY S 2003 and 2004 are attached, as well as financial reports for the nine months ended September 30,2005 as Appendix F. h. There are no actions or outstanding judgments in any amount against any relevant corporation, key personnel employed (or to be employed) by any relevant corporation, or person having a substantial interest in any relevant corporation. 2

i. A description of the fleet of aircraft available to Pacific Airways is attached as Appendix D. All aircraft will be leased. An affidavit of aircraft compliance is attached as Appendix E. j. There are no pending investigations, enforcement actions or formal complaints filed by the Department, including the FAA, involving the applicant or any relevant corporation, any personnel employed (or to be employed) by any relevant corporation or person having a substantial interest in any relevant corporation, regarding compliance with the Statute or orders, rules, regulations, or requirements issued pursuant to the Statute. k. There are no charges of unfair or deceptive or anticompetitive business practices, or of fraud, felony or antitrust violation brought against any relevant corporation or person having a substantial intcrest in any relevant corporation, or member of the key personnel employed (or to be employed) by any relevant corporation in the past ten years. 1. There have been no accidents or incidents experienced by the applicant, its personnel or any relevant corporation which occurred either during the year preceding the date of application or at any time in the past and which remain under investigation by the FAA, the NTSB or Pacific Airways itself. 3

m. Narrative Description of Pacific Airways, Inc. Pacific Airways was originally formed in February, 2000 by four equal shareholders, including the current three owners and Bradley Schrepel, who has since left the company and his shares purchased by the remaining owners. The business was formed at a time when the number of scheduled air carriers in Ketchikan had been reduced to two with the bankruptcy of Taquan Air/Air One which was the largest air carrier in Ketchikan at the time. By operating as an intra-state carrier, it could tailor its operations to the needs of the markets. Ketchikan, which is on an island, depends on air service to provide connections to other communities in the area, all of which are on other islands. Like all air carriers, Pacific Airways suffered severe financial reverses as a result of the 9/11 attacks and subsequent operations changes. Since 2002, Pacific Airways has recovered strongly and been consistently and increasingly profitable. Because of consistent community support, Pacific Airways has gown steadily and receives numerous requests for services that are essentially interstate in nature. To meet this growing demand, and be able to continue to operate its current intra-state scheduled service, Pacific Airways is seeking federal certification. n. Pacific Airways, Inc. currently holds no federal economic authority. It holds an Operator Certificate from the Federal Aviation Administration which allows it to operate intra-state scheduled and on-demand air service. Pacific 4

Airways is in the process of obtaining an Air Carrier certificate with commuter operations specifications from the FAA. A copy of the certificate and associated operations specifications is provided as Appendix G. The Flight Standards District Office in Juneau will be handling the application. 0. A description of the scheduled air service to be provided is attached as Appendices F. These appendices contain all data required by 14CFR204.3(t). p. A signed counterpart of Agreement 18900 (OST Form 4523) will be provided. 5

Mr. Rhoads owns 33.3% of the capital stock of Pacific Airways, Inc. He holds no other stock in any air carrier, foreign air carrier, common carrier or person substantially engaged in the business of aeronautics except Radial Power, Inc., an aircraft and engine leasing firm that leases aircraft and engines to Pacific Airway, Inc. After graduating from Lake Stevens High School in Lake Stevens, A in June, 1985, Mr. Rhoads entered Everett Community College (A) with a major in Aviation Technical Arts. During this period he also worked at Regal Air at Paine Field (Everett, A) from 1985-88. He graduated with his Associates degree in June, 1988. He was immediately hired by the Boeing Commercial Airplane Company and worked as a Quality Assurance Inspector and aircraft delivery customer liaison on Boeing 747 and 767 aircraft. Mr. Rhoads earned his private pilot license in June, 1986, and his commercial and instrument authorities in November, 1989. He left Boeing to work as a line pilot for L.A.B. Flying Service in Haines, AK in February, 1991. He returned to Boeing after the summer flying season in October, 1991, and worked there until March, 1992 when he was hired as a pilot and later became a check airman at Markair Express in Bethel and Kodiak, AK. In March, 1993 he left Markair to accept a job as line pilot with Ketchikan Air Service where he was a check airman and Chief Pilot until March, 1997 when he was hired as Chief Pilot at Promech, Inc. in Ketchikan. In February, 2000 he joined Josh Murdock and Steve Montanus to form Pacific Airways, Inc. Mr. Rhoads lias been approved in his position as Director of Operations by the F.A.A and also held both the Chief Pilot and Director of Operations for a period of approximately one year until a sui tab le replac emen t was hired. Mr. Rhoads has one accident, no incidents and no violations on his record. In March, 1995 while flying a Cessna I72 land plane as a private pilot, the aircraft suffered an engine failure and Mr. Rhoads ditched the aircraft in the ocean about 16 miles from Ketchikan. The cause of the accident was determined to be engine failure, and resulted in

There are no pending investigations, enforcement actions or formal complaints against Mr. Rhoads regarding compliance with the Statute or orders, rules, regulations, or requirements issued pursuant to the Statute. There are no charges of unfair or deceptive or anti-competitive business practices, or of fraud, felony or antitrust violation brought against Mr. Rhoads.

Box 5158 Ketchikan, AK 99901 General Manager Steve Montanus is a natural born citizen of the United States having been born in Springfield, OH on February 14, 1968. Mr. Montanus owns 33.3% of the capital stock of Pacific Airways, Inc. He holds no other stock in any air carrier, foreign air carrier, common carrier or person substantially engaged in the business of aeronautics except Radial Power, Inc., an aircraft and engine leasing firm that leases aircraft and engines to Pacific Airway, Inc. After graduating from Chaminade-Julienne High School in Dayton, OH in June, 1986, Mr. Montanus entered Ohio University with a major in Aviation Management. He received his Batchelor of Arts degree in August, 1990, and lived with his family while seeking jobs in the aviation field. In November, 1991 he went to work as a Customer Service Representative at Citizens Federal Bank in ihington, OH. He left in January, 1992 to work for Customer Contact Printing in ilmington as a printer and binder. In February, 1993 he went to Anchorage, AK on a vacation and job hunting expedition. He returned to ilmington in March, 1993 and continued to work at Customer Contact Printing until December, 1993 when he was hired as a dispatcher with Baker Aviation in Kotzebue, AK. He worked in Dispatch and accounting until May, I995 when he was hired at Ketchikan Air Service as a dispatcher. In March, 1997, Ketchikan Air was purchased by Taquan Air Service, and Mr. Montanus went to the Airline Dispatcher Training Center in Dallas, TX and completed the F.A.A. dispatcher training course (2 weeks). He returned to Ketchikan where he was employed as a dispatcher at Taquaii Air Service until May, 1997 when we was hired as a dispatcher by the joint operation of Seaborne Aviation and Promech, Inc. in Ketchikan. He left that operation in February, 2000 when he joined Josh Murdock and Mike Rhoads to form Pacific Airways, Inc. Mr. Montanus has no accidents, incidents or violations on his record. There are no actions or outstanding judgments against Mr. Montanus in any amount. There are no pending investigations, enforcement actions or formal complaints against Mr. Montanus regarding compliance with the Statute or orders, rules, regulations, or requirements issued pursuant to the Statute. He holds no other stock in any air carrier, foreign air carrier, common carrier or person substantially engaged in the business of aeronautics except Radial Power, Inc., an aircraft and engine leasing firm that leases aircraft and engines to Pacific Airway, Inc. After graduating from Prairie High School, in Three Hills, Alberta, Canada, Mr. Murdock entered LeTourneau University in Longview, TX, majoring in Aircraft Maintenance and Business. During this period he independently earned his private pilot license. After completing the curriculum in aircraft maintenance in August, 1993, he returned to Ketchikan to work as an aircraft mechanic for an aircraft sales and repair business owned by Terry ills. He left Mr. ills in April, 1994 to work for Taquan Air Service in Ketchikan as a line mechanic. He earned his commercial pilot license in February, 1996, and began working as a pilot and mechanic for Taquan in May, 1997. In May, 1999, Mr. Murdock was hired by Promech, Inc. in Ketchikan as a summer pilot and winter mechanic. He left in February, 2000 to join Mike Rhoads and Steve Montanus to form Pacific Airways. Mr. Murdock has been approved by the FAA. as Director of Operations, and has served in that position since the formation of Pacific Airways. Mr. Murdock has no accidents, incidents or violations on his record. There are no actions or outstanding judgments against Mr. Murdock in any amount. There are no pending investigations, enforcement actions or formal complaints against Mr. Murdock regarding compliance with the Statute or orders, rules, regulations, or requirements issued pursuant to the Statute. He graduated in August, 1986 and received his Airframe and Powerplant license in November, 1986. He enrolled at Laramie County Community College in September, 1986, and received his Associates in Science in Aviation Technology in December, 1986. He was hired as a line and repair mechanic at Evergreen Air Center at Marana, AZ in January, 1987 and worked on inspections and heavy maintenance under contract for American Airlines and Continental Airlines. In July, I987 he was hired by Continental Airlines to work at their maintenance base at Denver, CO. hile at Continental he entered Metropolitan State College in Denver where he majored in aviation. In July, 1989, Mr. Robertus went to work for the Boeing Commercial Airplane Company in Everett, A as a pre-delivery mechanic on Boeing 747 aircraft, and then as a final assembly mechanic and until he was laid off in March, 1990. He returned to work in the faniily construction business in Colorado, and the occasional aircraft maintenance job until he was hired as an airframe mechanic Rocky Mountain Straight Flight repairing damaged aircraft in July, 1991. He re-entered Metropolitan State College in 1991, and earned his private pilot certificate in March, 1993. He earned his instrument rating in Febuary, 1995 and commercial certificate in May, 1995. He completed his B.S. degree in Aerospace Science in December, 1995. From August, 1994 through July, 1996 he worked as a mechanic in the corporate aircraft division of Paragon Ranch, CO until he was laid off. He worked his own construction business until April, 1998 when he was hired as a line mechanic for Taquan Air of Ketchikan, AK. In April, 2000 he joined Sky Signs in Conway, SC as a pilot to build flight time. He worked the summer season towing banners, and returned to Nebraska in September, 2000 returned to work in the family business. In February, 2001 he was hired by Tanana Air Service of Fairbanks, AK as a line pilot. In April, 2002 he was hired as a line pilot by Pacific Airways, and was promoted to Chief Pilot in May, 2904. The F.A.A. has approved Mr. Robertus as Chief Pilot.
