Provider Scholarship Information The scholarship program is a way for the business partners/vendors to continue to help and support educational events for providers. We appreciate each company that has assisted in funding the scholarship program. Applications must be received by 7/07/15. Scholarship recipients will be selected randomly from all applications received. Successful applicant(s) will be notified by email on or before 7/10/15. Successful applicant(s) must accept or decline the scholarship within one week of notification. In the event the successful applicant(s) is unable to accept the scholarship, an alternate recipient will be selected from the pool of applicants. Only one application will be accepted per applicant.
