Disney Trip Give-A-Way Rules & Guidelines Disney Trip Give-A-Way Eligibility Rules: 1. Be a Texas resident. Caregivers/Parents/Legal Guardians and their children must all be residents of Texas. 2. Be a family who cares for a special-needs child(ren). 3. Be willing to share your story via video, photos, and in writing (Be willing to share prior to winning the trip, discuss family s experience during the trip, and share story about how the trip impacted your life to Jeremiah s Voice, Inc. and its affiliate organizations). 4. A: If you are requested to provide proof of residency, then, yes, you must show proof. 3. Does it matter what type of disability the child has? A: No, all families with a child who has been medically diagnosed with any disability can apply. 3. Can I apply if none of my children have a disability? A: No, we are only providing this trip to those families who are caring for a child or children with special needs. 4. Will my information on the application be shared with anyone else outside of Jeremiah s Voice, Inc. or its affiliates? Page 2 of 5 A: No, your personal information will not be shared. Your stories, photos, videos and memoirs of your trip will be shared on our website, social media platforms, and marketing materials to illustrate the impact of taking a vacation. There will be no compensation for stories, photos, and videos shared. 5. When will I be notified if I win? A: Winners will be notified by Saturday, September 15, 2018, and are invited to attend the inaugural Jeremiah s Voice Fundraising Event on Saturday, September 22, 2018 to receive the Disney Vacation Gift. Two (2) guest tickets will be provided to attend the launch event. Please note: Jeremiah s Voice Inc. will not be providing Disney Vacation winners with transportation to and from the event. Disney Vacation winners must provide their own transportation to and from the event. If unable to attend, the winners can participate via video conference or video that will be shared at the launch event. 6. Can we submit more than one application? A: We ask for one application per family. Duplicate applications will not increase your chances of winning. 7. Is there an age limit on the special-needs child? A: No, there is no age limit for the special needs child; however, the child must be accompanied by their parent or legal guardian. 8. What is included in this trip? A. Jeremiah s Voice, Inc. Disney Trip Give-A-Way will include the following for up to Four (4) family members: a. Four-night accommodations at a Disney Resort hotel. (Views are not guaranteed and are based on availability.) b. Up to Four (4) 4-Day park tickets that will include the Park Hopper Option (one per person). For each day of the ticket, guests will enjoy admission to the Magic Kingdom Park, Epcot, Disney s Hollywood Studios, and/or Disney s Animal Kingdom Theme Park. B. Dining Guidelines: a. The Disney Dining Plan will be provided. b. Disney Plan exclusions: Gratuities are not included and private dining (outside of what s include in the Disney Dining Plan) is at your own expense. c. Alcoholic beverages not included. Children ages 3-9 must choose from a children s menu for Table-Service dining where available d. All meal dining options will be provided and approved by Jeremiah s Voice, Inc. and its affiliates prior to taking the trip.
Page 3 of 5 e. Merchandise or photo imaging products that may be offered at Disney Character Dining Experiences or Themes Parks are not included. C. Travel Guidelines: a. Round trip airfare from any major Texas Airport to Orlando International Airport (MCO). b. Complimentary Roundtrip transportation to and from the Orlando International Airport (MCO) to the Disney resort hotel (only). c. You are responsible for your transportation to and from the major Texas Airport to your home. D. Travel Dates a. Packages must be booked thirty (30) days in advance. b. The guests must provide three (3) available check-in dates in order of their travel preference. The final package travel dates will depend on availability. c. Travel must be completed by 10/1/2019. d. There are Disney Block-out Dates. (Travel will not be permitted if it conflicts with Disney Block-out Dates.) E. Disney World Requirements a. Disney Theme Parks and Resorts require that, at least, one member of the party be over eighteen years of age. At Resort Check-In, a valid credit or debit card in the guest s name will be required. Please note: Jeremiah s Voice, Inc. will not provide guides at the Disney Park. Guests will be responsible for planning their schedules while at the park. While the park is fully equipped to accommodate children with special needs, guests will essentially take a selfguided tour of the park for each day of their stay. b. Any incidental charges, such as meals beyond the Disney Dining Plan includes, phone calls, laundry, and room services are not included, and are paid by the recipient upon checkout. If there are more than four guests, we will advise to book a second room. Rooms required for additional guests will not be included. Package amenities such as Park Passes or Park Hoppers or the Disney Dining Plan will not be included for additional guests either. These items must be purchased by the guest. F. Gratuities are not included. This includes but it is not limited to gratuities for servers at restaurants, baggage services, and housekeeping. G. Air Round-Trip Coach Airfare is included and is capped at $500 per person. H. Jeremiah s Voice, Inc and its affiliates are not financially responsible for the following: a. Delayed flights, missed flights, cancelled flights, flight-time changes, seat assignment and/or reassignment, seat changes, seat upgrades, baggage fees, transportation between the guest s home and the airport or gratuities relating to any of these items. b. Flights cancelled by the airline will be handled on an individual basis and best faith efforts will be made to ensure the guest(s) arrives at their vacation destination as efficiently and safely as possible. (Must follow the Airlines guidelines and Jeremiah s Voice, Inc. and its affiliates are not responsible).
I. Terms & Conditions - Guest(s) understand(s) and agree(s) to the following conditions: a. This vacation package is nontransferable and nonrefundable. Once redeemed and the reservation confirmed, no changes will be accepted. b. This vacation package cannot be sold, traded, transferred, refunded, commissioned or redeemed for cash value. Vacation elements may not be substituted. c. If the total package is not used, there is no refund monetary or voucher due. d. Guests are responsible for providing proper citizenship documentation to travel which may include Texas State-issued driver s license, passport, valid alien registration card and/or necessary visas. J. Regarding the air component of travel, the guest(s) will assume responsibility for the following: a. Education, understanding and compliance with the airline carrier s requirements for check in, seat assignments, baggage sizes and weight, as well as proper identification. Other requirements spelled out by the airline carrier not listed here are also at the guest s responsibility. b. Checking in online or at the airport per the airline s recommendations. c. Checking flight status for any changes in departure time, terminal or gate. d. Transportation between their home and the airport on their travel dates. This includes costs or fees associated with transportation between their home and the airport including gas, parking, taxis, Uber and so on. e. Costs or fees associated with seat selection, seat changes, or seat upgrades which can be made directly through the airline carrier. f. Cost or fees associated with baggage including but not limited to carry-ons, checked bags, delayed or lost baggage and weight overage fees charged by the airline carrier. g. Delayed Flights: In the event of a delayed flight, the guest will coordinate with the airline carrier to determine and/or negotiate options. No reimbursement by Jeremiah s Voice, Inc. or its affiliates will be made to the guests for delayed flights. h. Flight cancellations: In the event of flight cancellation on the day of travel, the guests will coordinate with the airline carrier to determine and/or negotiate options. In the event of flight cancellation prior to the day of travel, Jeremiah s Voice, Inc, and its affiliates will work with the airline on the guests behalf to secure alternate flights. No reimbursement by Jeremiah s Voice, Inc. or its affiliates will be made to the guest for cancelled flights. i. Missed Flights: In the event, the guests miss their flight(s), the guests will work with the airline carrier to determine next steps and options. No reimbursement by Jeremiah s Voice, Inc. or its affiliates will be made to the guest for missed flights. j. Flight Time Changes: It s customary for flight times to change prior to travel. This window is usually within 30 minutes or less of the originally booked flight time. The guests assume responsibility for checking flight times leading Page 4 of 5

Most carriers allow guests to request flight status updates free of charge using one or more of the followings: Email, text, or the carrier s app. No reimbursement will be made to the guests for flight time changes. K. After the information above has been provided, allow 7-10 business days, excluding holidays, for an e-mail confirmation containing a reservation number. L. Jeremiah s Voice, Inc and its affiliates will make best faith efforts to secure the best flight schedule for the guests travel dates. Once air arrangements have been finalized, the guest will be provided with a confirmation number (record locator number). The guests will use that information to complete online check in.
