19 Terms and Conditions 19 SSES Supported Holidays 'Day Tripper' Breaks is a service provided by Small Seeds Educational Services having its registered office address at 90, Lockwood Scar, Newsome, Huddersfield, HD4 6BA. The Contract The contract between you ( the client ) and us ( Small Seeds Educational Services ) is subject to these booking conditions. The contract shall be governed by and construed in accordance with English law and is subject to the jurisprudence of the courts of England. You should read these conditions carefully before you book. You must be at least 17 years old at the time of taking your 'Day Tripper' Supported Break with us. The person booking the supported break is responsible for ensuring the accuracy of the personal details and any other information supplied in respect of you the client on the booking and for passing on any information regarding the booking or any changes made in relation thereto. Our supported breaks are suitable for adults with mild to moderate learning disabilities and 1:1 or small groups of adults displaying challenging behaviour. We provide basic practical support and supervision. In order for Small Seeds Educational Services (SSES) to provide the best possible support, the Health & General Questionnaire ( H&G Questionnaire ) must be fully completed and be a true reflection of you the client. If, at any time between completing the H&G Questionnaire and the client s supported break commencing, there is any change to the information provided on the completed H&G Questionnaire, this must be communicated to SSES in writing. This applies to every supported break the client is booked on. Challenging Behaviour SSES are able to offer supported breaks to people who present with behaviours that challenge and have a positive behaviour plan in place. on or chriscockroft@smallseedseducationalservices.co.uk to discuss how we can best support you on one of our 'Day Tripper' Breaks. Where challenging behaviour has not been 'flagged up' and a positive behaviour plan is not in place, in the event of you, the client, developing or displaying physically challenging, threatening and/or disruptive behaviour, we will reserve the right refuse any further business with you. What your 'Day tripper' Supported Break includes: The 'Day Tripper' supported break is inclusive of practical support, travel to destination and travel at destination if required and admission to venues (if requested). Transport is provided from a pick-up point, designated by us, to your 'Day Tripper' supported break destination. A Door to Door Service can be arranged and this may incurr an additional cost dependant upon location. Our standard support provided

20 is currently: 1 support worker to maximum of 3 clients. NB. Should demand for our service increase, resources will be employed to increase the number of clients we are able to provide our service for. 20 Transportation SSES may use the following means of transport during the 'Day Tripper' supported break: Minibus/Coach/Car/Ferry/Train. Means of transportation for a particular break can be confirmed via the Small Seeds Educational Services office. Brochure Content All details are correct at the time of going to print. Supported break and prices are subject to change without notice. If we are required to do this the client will have the option of cancelling the booking and have full payment reimbursed. Some images are used for illustration purposes only. Bullet-pointed activity information are indicative of what options are included on the corresponding 'Day Tripper' supported break: it is not a comprehensive list or itinerary. How to Book To make a booking please feel free to telephone in the first instance to discuss what you require. Alternatively, chriscockroft@smallseedseducationalservices.co.uk. with any queries or questions. 1) Fill out the 'Booking Form' and the 'Health and General Questionniare' (if booking for the first time). 2) If you require door to door pickup and drop-off could you please call on before making any payment. Depending upon your location there could be a supplementary cost to add to your 'Total Supported Break Cost'. 3) Send the full payment with the Booking Form and H&G Questionnaire. If you are ing the Booking Form and Questionnaire, let us know to expect the payment by post. At the moment we can only accept payment by cheque. 4) No booking is confirmed until the 'Total Supported Break Cost' is received. 5) We will send you a receipt which confirms the payment and booking. All written correspondence and receipts will be sent directly to you unless it is clearly indicated on the Booking Form that this is to be sent to a third party. If We Cancel 'Day Tripper' Supported Breaks are based on an economic minimum number of passengers which is usually 3. One to one and 1:2 supported breaks are available at an extra cost. If you do not pay the 'Total Supported Break Cost' on time we reserve the right to cancel your booking. We reserve the right to cancel your supported break in any circumstance, but if we do so, you can either have a refund or accept a replacement supported break of corresponding value from us, subject to availability. We will not be liable to pay any compensation if we are forced to cancel or in any way change your trip as a result of unusual or unforeseen situations outside our control, the consequences of which could not have been avoided even with all due care. These include unavoidable technical problems

21 with transport, war or threat of war, civil strife, industrial disputes, natural disaster, bad weather, epidemic or terrorist activity. 21 If you Cancel Should you wish to cancel your supported break with us you should notify us directly as soon as possible by telephone, confirming by . When written notification has been received by SSES Day Tripper Supported Breaks, a fee of 10 pounds will be levied to cover administration costs. You are entitled to a full refund only if you give us 14 days notice of your intention to cancel. If you choose to cancel within 14 days before the start of your 'Day Tripper', a 70% refund will be granted. You can choose to change your 'Day Tripper' to another date (subject to availability). Permission to do this up to 48 hours before departure is subject to a 10 pounds fee and is entirely at the discretion of the Management. Holidays booked 'last minute' within the 14 days prior to a day trip, are subject to the same cancellation policy. If cancellation is due to sudden illness, you can re-book with a replacement supported break of corresponding value from us, subject to availability. NB. If we can find someone to take your place there will be an admin charge of 20 and you will receive the balance of your payment. Alternatively, if you can find someone to take your place there will be an admin charge of 10 and you will receive the balance of your payment. Liability We do not accept responsibility for our failure to perform the contract for the supported break or for the improper performance of the contract, as such failure is due neither to any fault on our part nor to that of our employees, agents, sub-contractors or suppliers if it is: attributable to you the client, or is attributable to a third party unconnected with the provision of the services for and is unforeseeable or unavoidable. Unusual and unforeseeable circumstances beyond the control of Small Seeds Educational Services, the consequences of which could not have been avoided even if all due care had been exercised. An event which Small Seeds Educational Services, our employees, agents, suppliers and subcontractors could not, even with all due care, have foreseen or forestalled. Our liability shall be limited in accordance with the international conventions which govern such services and the conditions of carriage of the carrier.
